    April 2022 Surgery Buddies

Not gonna lie...I had a food funeral that lasted a few weeks (ok maybe more like a month) so I actually put on weight right before my pre op diet started. On the day of surgery, I was exactly 40. My doctor said my moderate sleep apnea was a co-morbidity (sleep apnea was discovered during sleep study required in my pre op testing) so I could actually be 35. I worried that since I was not doing anything to prevent the sleep apnea (i.e., CPAP), they could still deny me at 35. The hoops insurance puts us through is ridiculous!

Zoloft - 7 months after RNY - weight gain?

I have had some pretty serious mental health issues related to both severe depression and anxiety, and have been in therapy at various points of time. Unlike many, I have never use medication in that time. That was not entirely my decision - surprisingly even as bad as I was, no one ever suggested it and I didn't raise it because I was scared of adding even more weight I didn't need. I kind of wonder if it was the medical establishment not taking my mental health issues as seriously given my weight (since they tend to blame everything on being obese). Given that, I cannot speak to experience with Zoloft. I am completely supportive of medication, so if that's the best course of action for you and your situation, you absolutely should do it. But, I think you should discuss your concerns with the therapist. Would it be possible to try talk therapy without the Zoloft, to see if that helps enough that the medication can be avoided? Or, are there other options either for dosing or the choice of antidepressant? I can understand your concerns, and this should certainly be a discussion with the therapist to reach a course of action you are both comfortable with. Good luck!
  3. I was started on zoloft prior to surgery, I didn't notice any weight gain then. I've continued on it since surgery and to my knowledge it has not had any effect on that aspect of things.
  4. I think it's a personal decision as to which one you care about most, but it's your decision, not that of your doctor. Perhaps you'd want to start with a lower dose and monitor things. Perhaps if you continue to lose weight, but more slowly, that will be acceptable to you. Perhaps you'd like to start with the talk therapy only and see how that goes. Maybe relay these concerns to your doctor - ask if there are other medications that may work better with weight loss or perhaps the Zoloft can be paired with an appetite suppressant. Definitely I think there should be a back and forth with your doctor to reach something satisfactory on both sides.
  5. 18 years ago, I had a RNY. I lost 124 lbs. I kept most of it off until the last 3 years when I gained about 45 pounds. I have been successful in losing weight that additional weight and am back with 5 pounds of my lowest weight! I did not have plastic surgery 18 years ago and it's my biggest regret. Now that I am stable in weight, I'm looking at plastic surgery. I've talked to 2 Docs. And I'm confused and torn. Doc#1 says Fleur. He says that I don't have much back skin/fat. He can pull the skin forward and tighten it. I have a pronounced belly that hangs quite low and I get heat rashes/yeast infections frequently. Doc#1 says that Fleur should be a great solution. He said it will also take care of my sagging mons. (Which I really want gone!) He will also tighten my abs after nearly 50 years of weight gain and loss. Doc#2 says to do a panniculectomy and lipo around the sides. She feels this is the best option as my frame is not necessarily curvy and she could not be sure of getting a flatter tummy. Let's assume, this is all out of pocket cost. I like both Docs. Which would you choose and why? I could use some insight and advice! Thanks
  6. Hi everyone, I'm 7 months out of RNY (Aug 23, 2021) and my weight loss has been great (down 82lbs). I have been struggling my entire life with anxiety and as I get healthier I have been more motivated to deal with my mental health issues as well. I had a conversation with my primary care dr. this morning and she is prescribing 25mg of Zoloft and talk therapy. My question is - have you been on or started Zoloft after RNY? My main concern is weight gain. I am soooo nervous about taking this medication and losing the results I have worked so hard for the last year. I'm also concerned about it slowing my weight loss. I am 100% going forward with the talk therapy and honestly feel I could use the help of a medication, but am so nervous about trying it and then gaining. Thanks all!

    April 2022 Surgery Buddies

This was going to be the topic of my next question. I intended to quote 3 posts but I may have double-quoted one of them by mistake. anyway, my question has to do with the marker BMI and losing weight. I hover between 39.1 and 40 BMI. CIGNA is a stickler about BMI with 35 being a cut off for benefits. I know that I will lose weight when I start the liver reduction diet next week. A good friend who was sleeved in October told me emphatically 'Do not lose any weight.' After the insurance company gives their blessing, is it okay then for me to stop worrying about losing weight. (Such an odd concept)

I agree with @Goldengirl321... I also needed to maintain my BMI prior to the 2 week pre op diet as it was exactly 40. Afraid if I lost weight I would be denied insurance coverage. I lost just over 12 lbs in my pre op diet. You WILL lose weight during it!

    April 2022 Surgery Buddies

Hey there! I am just at a 40 bmi with no co morbidities so my dr actually told me to maintain my weight until surgery. Although I do still have to do the 2 week diet prior and I’m sure I will he said at that point the weight won’t matter. My journey was a little different I did all my pre op testing cardio, pulmonary, nutrition, psych, sleep study and my 12 office visits (per my insurance, Aetna) then I was approved at the end of January. I asked to be scheduled mid April due to some work things I had going on. I now have to do more testing at the hospital on 4/15 an EKG, chest X-ray, Covid test and blood work, I’ll see my primary Mon 4/18 for final clearance and then hospital 4/19. But back to your original question don’t worry too much you will loose in the 2 weeks and be a rockstar!
  10. Golden girl since we are scheduled the same day, I have questions. My preop mtg with dietary snd surgeon is scheduled the first week in April. I finished all my required visits in Oct last year but have been waiting for surgery due to Covid. I am nervous because I haven’t really lost much weight since then. I know I will need to go strict diet for 2 weeks prior to surgery but anyone else who has not lost a lot of weight preop in their own??? I feel like I should’ve done better.

    OOTD

Not an OOTD per se, but so grateful with the weight off I can wear heels again. Rocked these tonight on a date with skinny jeans and a body suit.

    carbs and grains

It will depend upon what you can tolerate. I only eat whole & multi grains. The less processing the better. I only have two serves of grains a day - rolled oats & multi grain crackers as a snack. Every time I try a highly processed grains like in bread, pasta & rice they sit like a dead weight in my tummy. Interestingly I’ve discovered that multigrain brown rice crackers are a slider for me but the white rice ones aren’t which I don’t understand why.

    Negative comments from family members

Trigger Warning: ..............................Yes. My dad is a food pusher. We have a very strained relationship now but growing up he would make 3 different dinners at once and have me eat a bit of all of it and then call me fat and bully me. He use to buy packs and packs of ramen noodles and hide them in my things like my desk/book bag/purse as I started becoming overweight as a child. I still can't eat ramen to this day because it brings me back to those moments. Hed tell me how ugly and fat I was but then push huge amounts of food on me. He's from a country where food is unstable and grew up incredibly poor so when he came to the US and saw how abundant food was he constantly buys food and cooks. He's a big reason why I had a bad relationship to food for so long/why im still working through it and why I still have self esteem issues. He's thin and in shape but has a serious problem with alcohol. I didn't tell him about my surgery because a) he wouldn't understand given a language barrier and he's treated me horribly over the years because I'm fat and the last thing I want is to hear those words but about my surgery. He's picked up on the fact that I had a weight loss procedure done. He tries to mend our relationship but still offers food because he doesn't know how else to show love. He offers me healthy stuff now but I simply just decline because I'm not ready to explain anything to him or become closer. Sent from my SM-G975U using BariatricPal mobile app

@sheribear68 So exciting! Congrats!! I also had a tummy tuck. I had muscle tightening as well. I had good abs as well but having kids and that extra weight that we all carried for so many years makes the center part of the abdominal muscles separate. I def wanted my PS to pull the ab muscles all in like a corset. I love love my TT. I also did a breast lift and Aug I have thought about a thigh lift and have gone back and forth on it. Glad you have worked so hard and kept it off!! This is a big treat to yourself for all the hard work!! Congrats and def show us some pics!!

I love this! im looking for some inspiration for a weight loss celebration tattoo
  16. I only have had bypass so I can share what my team said. At one month I was eating 2 oz of protein. It was still soft like yogurt. I would have a snack like 1 oz cheese and make up the difference with shakes. I needed a lot of snacks of cheese and yogurt to keep my energy up at work. I used the little OXO toddler containers to measure and prep. Now I use the 2oz containers for mustard or dressing or veggies. My team’s goal was for me to eat real food, and 4 oz of protein at one meal, 3x a day. I have a support group for the program with switch, bypass, and sleeve and we all have that goal. The switch people have higher protein goals due to higher malabsorption. I personally was not able to eat 4 oz of real food until 4months. Most folks in the program get to 6 months before eating 4oz. That was 4 oz by volume, at 6 months I explained that I was very hungry and they said to switch it to weighed (with a food scale) 4 oz. When you can eat 4 oz weight of protein then start eating veggies. It’s based upon what you can do individually. Obviously drink 64 oz volume of water, and take the vitamins, the rest is up to you and your body. I had many surprise liquid poops until I got to soft meats like fish and sausage innings. When I ate real food as opposed to the shakes I got regular again.

    Mad at myself

Sorry to hear of your loss and challenges. We all deal with stress differently, so the thought of seeking assistance with that is good advice. I will say from the weight perspective, I was six years out from my sleeve and had gained back about 45 pounds. I found that what worked for me was to go on a 'time restricted feeding' schedule, and to eat a diet that was generally keto when I did eat. I skipped breakfast and only ate from Noon to 6pm. For those two meals I would have a serving of protein, and a green vegetable or salad. I was able to drop about 20 pounds in a couple of months. I stopped because my hiatal hernia had worsened enough that it had to be fixed with revision to bypass, but the diet program was working well. There is a nutrition doctor named Dr. Jason Fung who develops programs for insulin control diets, which is what I based my program on. You might want to Google him and check out his information.

Chi Franciscan - Tacoma, WA thoughts?

I had the duodenal switch by Srikanth in 12/2019. By 7/2021 I was dying of malnutrition. I was taking all my vitamins and drinking my protein shakes - not the pre made ones but the ones you make yourself. I was blacking out, collapsing in parking lots, I couldn’t drive because I was blacking out while I was driving, it got so bad my husband had to bath me. I couldn’t be left alone. I had called Srikanth’s office to schedule an appointment to talk about the symptoms when they first started but was told that I needed to call my PCP so I scheduled an appointment with her. I was put on various medications because my BP was typically around 67/52, I had ear problems in addition to blacking out, collapsing, throwing up any food I could get down IF I could get food down. I eventually ended up in the ER and was admitted for a little over 2 months for malnutrition. Sometimes when you have the duodenal switch your stomach doesn’t absorb enough nutrients and your body will start shutting down. It was awful. Now I’m on TPN and have my vitamins, proteins, carbohydrates, and electrolytes pumped through my blood stream 12 hours a day. I’ve had sepsis 3 times in 2 months. I’m not saying it happens to everyone but the nurses in the hospital said they see this all the time. No one mentioned this before surgery. No one helped me after surgery. Srikanth’s office told me to go see my PCP who is not a specialist in weight loss surgery. I’m so dissatisfied with Srikanth’s office with the exception of Karli. She is wonderful but my issues came on before she started working there but now I have a resentment against there office and don’t ever want to go back. It MAY not happen to you BUT watch out for those symptoms.

    February 2022 Surgery Buddies

I had mine on Feb 14th so I'm 5 weeks and 3 days out. I felt pretty good about fluids and protein until like 2 weeks ago drinking water and feeling pain/restriction, especially when it's room temp. Using a straw and having ice water seems to help me get it down better. I actually gulp in a lot of air when I don't use a straw. Most teams tell you not to, even mine but I feel gas pain in my chest without the straw. I'm doing really well with protein though and get 70-80 grams a day through about 700-800 calories. Rarely do I get to 800 cals though. I noticed on days when I have eaten up to 800 cals I feel nauseated at night so I try to not go over 750. It's a small difference but weird enough I can really feel it. I eat 3 small meals a day, one snack and a protein shake. My dietician has me eat 2 oz of protein every 2 hours during the daytime. I'm taking vitamins well, no hair loss...yet? I do need to take nausea medicine in the morning because maybe 4 times a week I wake up with the instinct to dry heave. I don't know why but I've always felt nauseous in the mornings, even pre op. I find that I can either focus on food or water but not both. When I drink 64 oz of fluids, I eat less and miss my protein goal. When I reach my protein goal, I drink less fluids. I find that balance to be really difficult to deal with and get right. I wonder when that'll be easier? It's also tough with the 2-hour eating rule and then the 10 min before 30 after drinking rule. I'd basically always have to have food or fluid in my mouth all day which isn't realistic for me. I've lost 29 lbs in 5 weeks. I don't really feel it yet/still feel like a big out-of-shape lump LOL. I think I'll feel the weight loss when I hit the 240's. I have my one-month post-op tomorrow with my nurse practitioner. Looking forward to it. P.S. Is anyone still dealing with constipation and ROCK HARD stool? I am on prune juice, Metamucil, colace and occasional senna tea. I go almost every day but it's incredibly forced, usually bleeding occurs or I need to slap on a pair of gloves and take it out (I know TMI I'm sorry ) . I'm going to talk to my team about it tomorrow but I'm so done with it. It also causes me a great deal of nausea. Wondering if anyone else is dealing with this and what you're doing to make it better?
  21. My surgery was on 16/2. It’s been 5 weeks post surgery. My fluid intake has gone up so much and I’m starting to lose weight again now. On the advice of my surgeon and dietician and trial and error, I’ve done these things: 1. Track my water usage using the Waterllama app. it’s cute and easy to use 2. track my food intake using the “see what you eat” app, making note of only the proteins I consume (I don’t count macros otherwise. Just make sure everything is “light” or “Low fat” and mostly “sugar free”) 3. set reminders on my phone to take medications, eat and drink (I don’t feel thirsty or hungry so this has been dangerous for me) 4. use water enhancers and uncaffeinated herbal teas to vary the taste of my fluids I haven’t been hospitalised other than that 1 night post surgery however I’ve been making weekly trips to meet the care team. I’ve lost 25lbs, which isn’t much. I was barely losing any in the previous 2 weeks but according to the body analysis, I was losing muscle and water. I was lethargic all the time but could still move. Once I got the proteins and fluid settled, I’ve been feeling so much better. For movement, I searched for anything “post bariatric surgery” and was doing basic stretches and walking for 15min then moved to 30 min. Now I look for “Low impact/intensity beginners cardio” lasting for about 30min to 45min. Don’t push yourself to the point of pain. Once I feel twinges, I stop. The physiotherapist in the care team suggests that movement be done for 10-15 min, twice a day at a start. For the first 3 weeks, my heart rate doesn’t go beyond 65% of my max heart rate. Now I’m pushing it to about 80%. After week 6 I will incorporate weights again. I enjoy resistance training and can’t weight to get back to it so just do what feels right to you. Find the type of movement that you enjoy. Whilst my body has been changing even without exercise, I am constantly reminding myself this is the same body. WLS is not magic. I have to take care of my body before I can get it to work right. This means eating drinking moving and resting sufficiently. I’ve been on 5 weeks of leave and this has helped me with my new-habit forming as well as allow me to heal gently. I will finally get back to work next Monday. I don’t know how I will be energy and pain wise but we’ll see.
  22. I had my band removed almost a year ago and I'm in the process of getting a bypass due to GERD. I never had any issues like slippage or erosion. Only the reflux that progressed to GERD. Now my esophagus is permanently altered, so my only choice to fix it is bypass. My surgeon wouldn't do bypass at the same time as band removal because she wanted to give my digestive tract a chance to heal from the GERD. I haven't gained any weight back after it was removed. I've actually lost a bit of weight. I'm weird, I know. 😂 I think it was because I was relying on slider foods because they were easiest to eat due to the acid issues. And now that the band is gone and the acid issues are better, I can eat healthier foods like fruits and veggies that my filled band wouldn't allow. And I have had no issues with insurance. Mine has covered all of my surgeries and will also pay for revision. I'm sorry about your insurance issues. Bariatric surgery should be covered by all insurance just like a heart surgery or knee replacement surgery. Best wishes!
  23. Hello all, I am having trouble with my band and want to get it removed. I have acid reflux constantly and I'm worried it's ruining my esophagus. I always feel like I have a lump in my throat. I was advised to remove the band and get a gastric bypass in its place so I don't gain all the weight back that I've lost and will continue to lose weight. My presurgery weight was 315 lbs. I got down to 195 over a five-year period but I had some issues. I had a slippage a couple of years after I got it and had to get it repaired. I had an incident where I couldn't get anything down (even water) so they removed all of the fluid. I was afraid to get it filled again. I've gained back quite a lot of weight. I weigh 243 lbs now. I'm 5-6" so I'd like to get down to around 150-160 lbs. My primary insurance, Florida Blue, excludes all bariatric surgery. 😡 I have great secondary insurance, BCBS Michigan, that covered my original band surgery (at almost 100%). I had the office coordinator at one of the doctor's offices that I called about a consultation tell me that my secondary won't cover anything if my primary doesn't cover it because they only pick up costs after a claim is filed. This "coordination of benefits" is pretty crappy for the consumer. I'd love feedback, especially from anyone who had health insurance issues like this or had a revision surgery to a gastric bypass. Was the band removal surgery and gastric bypass done at the same time or did you have two surgeries? I'd like to get it all done at the same time rather than have two but I've read that some doctors will only do the two procedures separately. Lastly, I've also read that the band's "shelf-life" is around ten years due to possible erosion issues. When I originally got it, 20 years ago, I was told that it was for life but could be removed if there were any problems down the road. Sorry this is so long. 🙂 Take care, Holly
  24. Like others have said, just don't make a habit out of it. Part of the reason I had this surgery is because I don't want to continue the lifetime of diets and guilt with eating. I want to lose the weight and just have a healthy relationship with food, that includes the occasional candy bar. Beating yourself up or being overly restrictive may lead to more binges. Just accept it happened and move on. If you find yourself desperate for candy try to find an alternative option or go for a mini size. But this first year is your fast lane for weight loss and you should try to maximize that as much as possible. I was on Protonix for 2 months post op and still have it for as needed. I was never prescribed the other med you mentioned.

    February 2022 Surgery Buddies

Hello! I had surgery on 2/22 and had major complications afterwards. I was in the hospital for 13 days out of the last month. I, too, had a hard time getting protein and fluids down and my weight loss has also stalled at 16 lbs. My doctor ordered thrice-weekly fluid and vitamin infusions for me. Perhaps you can ask your doctor about that.
