I would want to look closely at this, verifying the bile reflux and determining if there is any acid reflux component to this before getting into long term treatment options as the treatment can differ widely depending upon that diagnosis. If it is bile, then I wouldn't expect Pepcid or other anti acid meds to do much as the are treating acid and not bile, a base. I'm not sure what meds they do use but likely different ones. Bile is used to neutralize the acid coming out of the stomach along with the digested food into the intestines. Is your surgeon in the loop on these findings (I assume so, but check if you haven't heard from them yet,) as that may change his prescriptions. If it is strictly a bile problem, then a bypass will probably correct it, but not guaranteed as it moves the stomach/pouch outlet downstream into the natural path of bile secretions; the key, according to one surgeon I discussed this with, is the length of the roux limb, as that is the one that connects the pouch with the mainstream intestine and how far any bile would have to travel to reflux into the stomach. This doc noted that at 80cm or greater (IIRC) he didn't run into any bile reflux problems. The basic RNY procedure has been around for some 140 years for gastric cancer and gastroparesis (it is usually termed just a partial gastrectomy, or likely some other fancy latin names as well,) and it that use, bile reflux is a not uncommon complication. My non-MD take on it is that in those cases, they tend to keep the limbs short to minimize malabsorption and weight loss (last thing a cancer patient usually needs is more weight loss!) So, the longer limb makes sense here. Discuss this and make sure that your surgeon is up on this aspect of it. The other option if it is basically a bile problem is the DS, duodenal switch, which is pretty much a guaranteed cure for any bile problems owing to the very long path between the bile ducts and the stomach, but relatively few bariatric surgeons offer it owing to its greater complexity. Note this only applies to the "traditional" or Hess DS and not the newer SIPS/SADI/"loop" or simplified DS, which like its mini-bypass cousin has bile reflux as one of its common complications. The DS will not help any acid reflux problem as it uses the existing sleeve (though may resleeve it if it was malformed causing GERD rather than just overproduction of acid,) while adding the intestinal rerouting for malabsorption. The DS is a better choice over the RNY revision if slow or inadequate weightloss is an issue, too, as it is a stronger metabolic tool. Good luck on this - bile is surely a much less common problem with the sleeve than acid reflux, so the industry isn't quite as settled on solutions for it. -

Yes, very common, and here is one of the factors - In the middle of the video he goes over the typical progression of meal volume that we can expect, and it is consistent with my experience. I you simply let things happen and eat the same way we did the first year, but more, then there will certainly be the tendency to gain. While I'm not a big fan of everything this guy preaches (or any online guru, for that matter,) one of the things that I do like is his "eat your vegetables first" concept, which he applies after the first few months of protein first. My general evolution was to increase the veg content of my meals over time, rather than the protein, as the protein that we are advised to consume early on is usually our basic requirement, as we can't get that from pills like we can most micronutrients. So, I still have about 3 oz of meat in a meal today like I did the first year, but a lot more veg in there. And, it's still something of a struggle because the junk still wants to creep back in there stimulating old habits (we usually spent a lot more time getting fat, and getting used to it, than we have trying to be healthy now!) With a sleeve, or a bypass, at this point it is basic dieting/healthy balanced eating that we are all familiar with; it is somewhat easier than before as our volume rarely returns to where is was pre-op, but our old bad habits sure can come back. -
Hi all - I'm so sorry to hear of what you are facing guys; I'm sure you had not bargained for that! I wondered what procedure you all had. I had a gastric bypass and seem to have healed well thus far. I only feel nauseous very occasionally and the feeling is fleeting and thus I'm not actually put off of eating my food. I have not yet experienced 'dumping' and threw up only once much earlier on. I have not yet had a bad reaction to anything that I have eaten but I do try to be careful. I'm three and a half months post surgery and feeling ok. I swim daily and feel sufficiently healed so far. Have you all spoken to your health team? xx

Can I take liquid vitamins instead of meal replacement?

You can use whatever protein shakes you want. I used ready made the first 2 weeks, then I bought protein powder from Arbonne and made my own. You can take liquid vitamins as long as they have the same amount of vitamins as bariatric vitamins do. I wasn't told not to take capsules or any kind of pills with the sleeve when I had it, but I WAS told that when I had my revision to bypass. It took a little bit but I'm able to take all my normal pills and supplements like normal now. -
Waking up from surgery

With my sleeve, I had major pain for almost 2 weeks. With my revision to bypass, I had very little pain. Mostly, it was uncomfortable for about 4 days. Wish I had just done the bypass to begin with. -
Hello! I am new to this website and I would love some advice! I am hoping to be having surgery in November or December this year. I unfortunately am a picky eater (and drinker!) and am not a fan of the artificial sweeteners and stevia. I have tried a bunch of protein powders and drinks, and I am struggling to find one that I can drink! 😥 I LOVE Bocha Sweet and don't mind erythritol and Lakanto Monk Fruit seems to be just fine! I am REALLY struggling to find protein powders and drinks that I can use following surgery to get through the initial post op period. Does anyone have any recommendations, either for store bought ideas or recipes that I can use with the zero calorie sweeteners I do like? Thank you all so much in advance for your ideas and help! 😊

Unable to get enough fluids or protein 5 weeks post-op

Hello all! Newbie here! I had gastric bypass surgery on August 8. Since then, I have not been able to meet my fluid or protein requirements, or even get close honestly. My Dr and dietitian just say "well, you have to." I'm still feeling weak and tired and just want to feel better! Does anyone have any tips, tricks, recipes (I'm on stage 3) that might help? -
How common is chronic nausea?

Its rare but I got it with the sleeve. It was not bad enough to take me to the ER but was really debilitating all the same. If I am really careful in what I eat and take my PPI twice a day and use sodium alginate after foods it keeps it at bay. I would have got the gastric bypass if I knew beforehand. -

For those who just got it done how was it I'm scheduled for the bypass on Monday 9/18//23 and not going to lie I'm second guessing it right now I'm 35 years old is this what I want for my life Im only 320 pounds yes the weight sucks and my quality of life sucks but I don't know if I'm ready for all the medications vitamins etc idk I'm just thinking. Any input -
Tomorrow will be my 1 year anniversary since my gastric bypass surgery. I have lost 86 pounds. I’m not going to lie, I honestly thought I would have lost more by now. Although weight loss has slowed down tremendously, I have yet to see a gain on the scale so I take that as a win. I would like to lose at least another 50 pounds so I have to remember this surgery is not a cure-all, it’s simply a tool. I’ve been extremely lucky and have had no complications whatsoever. I don’t experience dumping syndrome at all (well I did once when I ate some ice cream and have steered clear ever since) but other than that there really isn’t anything I can’t eat, I just eat a whole lot less. I can definitely be better about my diet, like always, but one thing I’ve incorporated into my life that has stuck is exercise. I don’t feel good unless I exercise so it’s become addicting (in a good way). Anybody who is reading this and contemplating this surgery, or is in recent post op and struggling, I have absolutely no regrets about having this surgery. The first 2 months were rough, but I got through it and so will you!

Even Water is Nauseating (2 months post-op)

I'm 7+ weeks post-op from Gastric Bypass/RNY. I have zero appetite. I've spent over a month in the hospital because of infections, dehydration, etc. Since I wasn't getting results from the surgeon's facility post op, I decided to see my PCP for help. Best decision! Small changes (doubling my nausea meds, etc.). I may even receive IV fluids when necessary, maybe even from home, if possible. I realize the part of my stomach that communicates with the brain has been removed. I don't see an end in sight. I was told by nurses at the hospital that this aversion to food and drink is not unusual; for some it lasts months, or even years (I'm going on 8 weeks). It sounds like this improves with the sleeve, but has anyone else experienced this long term after a gastric bypass? -
I have fault for my health for 5 years. Several doctors and countless tests concluded that the same "bypass" is my best option. Therefore, I am heading for that operation table on 09/25/2023. Sent from my SM-G996U1 using BariatricPal mobile app

Today was my 1 month followup, 5 weeks post surgery. My dietician gave me a gold star review for my post op diet, and I graduated to Stage 4 of my program. Stage 5 should be approved in November, and that is the forever way of eating.
I had to go to a post op nutrition class as part of my appointment today. There was a young lady in there that had been eating WAY off diet, and she was not doing well... She had been eating fast food, but peeling the buns off, eating vegetables that were not well-cooked, and eating all kinds of foods that were not part of the detailed instruction that everyone in this program receives. We had SO many caveats from Day One to not try to jump ahead on the schedule for the foods that we eat. I was baffled that someone would decide to eat off program, especially so soon after surgery. She became tired of the liquids & soft foods, so she chose to eat ahead.
The result of her skipping ahead was that she had experienced a lot of vomiting & foamies, a lot of pain, and was unable to drink any fluids. She started feeling flushed and faint in class and had to be sent to the hospital to get some IV fluids.
The discipline that one needs for bariatric surgery cannot be overstated. We have to dig deep mentally and physically to get through surgery itself, learn new things, manage medications, and really lean into that crucial change of mindset and behavior. I hope that this young lady is able to get herself together to make the most of her surgery.

I am still waiting for my surgery...though classes were completed 9/2022. I don't like the waiting game. But I will say that my 'surgery buddy' mentioned this site and am happy I'm hear and hope to gain support and insight for my surgery.

Hi All, I new to the forum and this is my first message of any sorts on this site. I am getting ready to have Gastric Bypass procedure on Oct 2 and I am currently on 4 week liquid diet. I have completed 1 week and 3 days of the 4 week diet so far. My questions for you all :- 1. Is eating pickle slices or cucumber slices considered cheating? I am asking this as the protein shakes I am taking are super sweet because of the artificial sweeteners. 2. Can I have tomato soup or tomato based broth ? In my pre-op instruction it doesn't mention anything related to it. 3. Will my doctor know that If I cheated with a pickle slices or 4-5 mini pretzels ? I need all the advice and help... I am crossed over that stage of not having food is fine... but I just want to chew on something that is not sweet and not protein until I am in the mush stage. Thank you all in Advance.

I’m 10 weeks post op and after a very normal start to recovery other than very pale stools- I seemed to have developed very active bowel movements. im eating normally, small meals but nothing excluded and I am waking in the night to use the loo. just want to check if this is normal - I’m going at least 5-6 times a day where as before I was lucky if it was once a day. I have converted from baby to bypass so I’m used to the process - just can’t get my head around this side of it as never experienced this before - except when I was on orlistat years ago

I’m 25 days post gastric bypass and I’m still having abdominal pain every time I try to eat anything off the approved list. Every morning, like right now, I’m going to have 1/4-1/2 cup of scrambled eggs but like every other day I end up throwing it all up. I’m afraid to eat anything now because I throw up anything I try to ingest. Now that I’m reaching my one month mark I’m not going to try and eat what is suggested. I’m so weak and hungry but I hate that I know it Wil just all come up. Depression has set in along with detachment from things i loved before: taking my family to the lake, walking my dog, etc. i just lost any interest because mainly I’m so angry that I can’t eat even the simplest things. I’m asking if anyone here has gone through this or is this not supposed to happen and I should see my bariatric physician? The one food i can eat without retching is a small tub of ready made sugar free chocolate pudding cups from Jell-O. Looking to some help or answers to my problem. One more thing at times my chest feels so much pain it feels as if it wants to burst open so my doctor put me on hydrocodone for the pain but I rarely take them for fear of addiction and/or accidental overdose. I’m a mess and more than often I regret having this surgery even though I lost 54 lbs in 3 weeks and haven’t needed to take insulin for my diabetes-it’s been in very normal levels between 95-120 daily and my most painful neuropathy in hands and feel don’t hurt that much anymore. Thanks for reading my TedTALK and hopefully there’s a solution out there for me.

I have just arrived home from our 14 day holiday. We had it booked pre- covid and the company allowed us to continue postponing the booking until we could go. It was a All inclusive type holiday where you stay in a big resort and everything is at hand which suits us when travelling, my husband needs a wheelchair. We were a little nervous of the airports and flying as it is such a hassle with a wheelchair. It all went quite smoothly except for the times I was stranded with a hubby, wheelchair, two suitcases and two bags and a large handbag. At times it felt like I was the puzzle where you have a river to cross with a bag of grain, a hen and a fox. I wore the bags like a bandolero, pulled the suitcase and pushed the wheelchair with my stomach. Oh the joys of checking in at airports. I ate what I wanted, which being me, was always the best fish, seafood, cheeses and a vast amount of serrano/ parma ham. They made their own cookies and bread so these never got bypassed but in small amounts. I even had butter and ice cream. I drank strong coffee with liqueurs and some long cocktails. Usually one or two a day. A few times I felt really drunk when the bar tenders had made the cocktails stronger than the last. It was not a good feeling but I found something to eat and then the alcohol was absorbed quickly. I was a little worried to get on the scales this morning. Pre surgery, I could put almost a stone on in the two weeks [14lbs] a lot of it was water weight around my swollen ankles. I weighed in at exactly the same and TMI moment, had 8 days of poop inside me. So may even loose weight when the medicine kicks in [ or is it out ? ] So te he he another win win for my bariatric surgery

I went for my EGD last week due to all the GERD I have been experiencing for over 5 months which has got worse and no OTC or RX medication is touching it. Results of EGD: - Localized mild inflammation characterized by erythema and linear erosions was found in the prepyloric region of the stomach. Moderate bile reflux into sleeve. Chronic Gastritis. Doctor has me on RX Pepcid and I see him in 6 weeks to discuss revision to Bypass. Personally I wish I would have went with the bypass instead of the sleeve when I had my WLS. How was everyone's experience who had the revision to bypass that experience GERD. Was it gone after? -

With my sleeve, I took 2 weeks off and needed every bit of it. With my revision to bypass, I was up and doing things after 4 days. Only took a week off, but honestly didn't need it. Still took it, though lol -

Two days post- op from my gastric bypass. The abdominal pain is the worst part particularly in bed finding a good sleeping position. I don’t take pain meds at night but they probably would help. Sipping water and getting proteins in is a full time job but I’m getting the hang of it. My favorite drinks so far have been the unjury vanilla premade drink and the unjury chicken soup. I plan to try yogurt with unflavored protein powder in the next few days. Best wish to everyone wherever you are in the journey! -

Yesterday was my one year roux-en-y gastric bypass surgery anniversary and as of today I am officially in onederland! 199.8 lbs! highest: 410 lbs | surgery: 355 lbs | current: 199.8 lbs (1 year post-op) | goal: 185 lbs I had my one-year appointment with my surgeon today. We discussed my progress, and how my restriction is doing. I have some recurring nausea and vomiting that happens about every other week or so. My surgery a year ago had a rare-ish complication (occurs in less than 0.9% of rny patients). My roux limb, the piece of small intestine that now connects to my stomach pouch, became severely constricted by scar tissue, and I had a second laparoscopic surgery to fix that. I've since had three upper endoscopies to widen my stomach pouch because I still have semi-regular bouts of vomiting. My stomach pouch also has an ulcer which may be a cause for the vomiting. I have another endoscopy scheduled for next week, and my surgeon and I discussed my options: treat the ulcer with strong bouts of Prilosec, or if that doesn't work have another laparoscopic surgery to cut out the ulcer and reconnect the roux limb to the now even smaller stomach pouch My surgeon also put in a referral to a plastic surgeon so I can start seeing my skin removal surgery options. I doubt I will ever be able to afford the surgery in the U.S., but I would certainly feel better having surgery here than in Mexico or Thailand. My current stats are: Exercise: 20-30 mins cardio and 45 mins strength training each day, 6 days per week Hydration: 80-100 ounces of water/iced tea per day (my fave drink is Starbucks peach green iced tea) Food: 80-100 ounces of protein; 1000-1200 calories per day (I don't track my calories closely) I love strength training! I'm starting to see a ton of muscle definition in my legs, shoulders, back, and arms. I also recently attended a convention and walked more than 15,000 steps each day and it was TONS easier to walk around downtown Seattle than it's ever been in my life. I'm so excited to be below 200 pounds - I haven't been below 200 pounds since I was in middle school! I hope you all are doing well! Keep up all your hard work!
