    Please help - Gastric Bypass vs SADI

    Your surgeon is right it is your decision, but, your surgeon also knows based on past patients experience and your health background, stats etc., what she feels based on her experience what would be the best for you. The surgery itself, the aftercare and long term outcome. I've never had to throw up, get the runs or any issues with my bypass. I feel most ppl don't have issues, but you hear about these that do, because why post everything is great I have no issues or questions? I chose the bypass because I have the worst luck known to mankind and I just knew I'd start getting acid refux/GERD if I didn't and I'd be back for a revision later. I've had no issues - none! I'm not sure why you think getting a SADI wouldn't have a chance to effect you the same way a standard bypass would. There is still the possibility of nausea/vomiting etc. I think if you only have 100 lbs to lose ....I agree with your surgeon, it might be a bit much unless you have diabetes or a BMI of 50% or higher. Just my two cents..although with todays economy..maybe its only worth a half cent! Finally, you won't spend your entire life only eating a spoonful of food at each meal, just smaller portions.
  2. Hi there! Been on my surgery journey for a few years now and I'm finally coming up on my RYGB surgery date in a couple weeks. It might just be nerves but over the last couple of months I've started to rethink whether I'm making the wrong decision with the going with the bypass. I spoke to my surgeon about a week ago & expressed I might want to switch to SADI. She said it's ultimately my choice, but she feels like RYGB is a better fit for me personally as I have a history of GERD (currently mild) and that she's concerned I might lose too much weight with SADI - I need to lose 100lbs. I've been reading & watching a lot of posts from people that have had each procedure & I'm really concerned with the idea of throwing up & having to eat a couple of bites per meal the rest of my life. The thing is, I rarely see anyone post complications with SADI. They seem to eat small meals but not as small as restrictive as RYGB patients & while they experience diarrhea - I don't see posts about vomiting. I mean, does everyone experience vomiting & dumping syndrome with RYGB? Does anyone know people that have had poor experiences with SADI? Ultimately, am I making the wrong decision by not going with SADI?

    I'm so sorry this happened to you. I originally had a sleeve and had several very rare complications that led to needing 7 surgeries in a year to correct everything. I had a revision to the bypass, which made all the difference and was life changing. I hate that the bypass wasn't the same for you. I'm glad you're home and starting on the road to getting better.
  5. I just had a revision from the sleeve to a bypass a week ago today. I'm already 16 pounds down. I had horrible stomach acid... the thing is, lifelong taking medicines for stomach acid is bad for you. Increases cancer risks, dementia risks, and accelerates bone loss. I did not want to be taking these meds for decades to come when science has proven how dangerous that is. I've had no vomiting or nausea while healing. This time around, the vitamins don't even make me sick which is a huge relief. They used to.

  7. Hi Sleeve to Bypass. good to know I never got the bypass. I was so worried cause of you saying the weight loss and all. I ended up in Emergency NYE as I couldn’t intake anything. I was in so much pain and extremely dehydrated. They went in and found I had to perforations. One just above the fundoplication on my oesophagus and the other in the stomach. A week in hospital on intravenous meds and now 1 week home and I am feeling so much better.
  8. Anyone experience a worst case with your bypass? After my bypass, my small intestine decided to close off completely . I had to have an energy surgery to reduce my small intestines I aspirated during the second surgery and ended up on a vent . I ended up with sepsis, ARDS, and fighting to stay alive. 10 days on the vent and 23 days in the hospital. I’m home now trying to recover and figure out all this stuff. I’ve had to learn how to walk and how to use my arms and hands again due to prolonged time on the vent. I knew this was a life style change but I didn’t expect to be in this shape trying to figure it all out.

    I think you might need to talk with your surgeon and be really detailed about when you can’t hold down food. All the time? Only in morning? Before bile movement after BM, certain food triggers, etc. I got the Revision from the sleeve to the SASI bypass. I had the sleeve in 2017. There was a balloon at the top of my sleeve stomach and for the past 2years I was wondering why after I took a BM, I felt bloated and I always had gerd with a little regurgitation. It took my new surgeon to tell me that I had a balloon/bubble. So, basically I didn’t know I had that until my 1 day post surgery follow up on my SASI procedure on January 10, 2024. Long story short, I think you can do a revision surgery. I haven’t heard of a reversal. There’s the SASI and the SADI bypass.

    Due to complication I am only just now starting a regular diet. I’m doing everything sugar free, but wondering does no sugar mean no sugar ever? Can I eat an occasional slice of pie at an event?

    I had both and got the bypass. I was post menopausal so I can’t say it helped with most PCOS issues. I swung the other way 2 years post op and got hyperthyroidism! So my team figured it’s a vitamin imbalance and I’m monitored more closely. (Lower vitamin D can increase thyroid levels). PPI can interfere with vitamin absorption, so does caffeine, so planning your vitamin times counts too. Talk with your team about best practices for your vitamins. I have found that I need extra iron with vitamin C at lunchtime and I can be consistent with it. That extra iron is just because my iron levels tend to be low, most people don’t have that issue. I take calcium, vitamin D and magnesium at night, B vitamins and Flintstones with iron in the morning with decaf coffee in my pea protein shake. If I stay really consistent with my water intake I can avoid PPI s for now. For me, it’s really a balance and worth the shock of being a little hyperthyroid for a month! Great luck to you both!     This is normal. It is almost like an initiation phase that will pass. I had a lapband in 2012. On Jan 8 I revised to bypass and had my gall bladder removed. I am still not past the misery stage but getting there. Gas pain is the worst. Keep the faith it will get better.

    My revision surgery (sleeve to SADI) went well on Thursday. My pain is almost non-existent, and my energy levels are good. Pacing myself getting liquids and protein in but doing fairly well with that as well. I was discharged the day after surgery and I'm home now.

    Thank you for the response @learn2cook it’s a little scary even though it seems to be really common. I don’t want a revision and hoping I can just get a hernia repair. But such is life. The doctor said I also had some curves to my sleeve so a revision maybe in my future. But he wasn’t a Bariatric doctor but a gastro. Thank you

    Hi Lark, I'm sorry you're feeling lousy. I'm 9 days post op, gastric bypass with Hiatal Hernia repair. It was rough the first couple days, it hurt and I couldn't stay awake for more than a few minutes at a time. And I couldn't pee. It was a rough go for the first couple days, but I got better by halves it seems starting 4 or 5 days out. I went home on Friday, took one pain pill friday evening and then didn't need anymore, yesterday and today I haven't needed any tylenol either. It will get better, sending you healing thoughts and hugs.

    That's really good to know! I heard the difference between sleeve and bypass described as a nudge vs a shove, and I knew I needed a shove. I'm glad to hear the hormonal issues are indeed likely to get some help!

    So my surgeon said that because of the conditions it would be better for me to have the 'harder hitting' surgery - by that he means the one that has the slightly higher % of weight loss results afterwards! He also said that the bypass is more effective in balancing out hormonal issues compared to the sleeve, and that what I need is an entire metabolic reset!
  20. Hello all I was wondering if anyone else has had this rare complication. I got my sleeve done in Mexico Oct 2021. About a year ago, January 2023, I noticed I started to have excessive saliva. It started randomly but picked up more. Well fast forward to January 2024 I got an endoscopy and was told my sleeve is “tortuous” and I have a hernia. This means that my sleeve has a lot of turns. I don’t feel any pain but the acid reflux which was manageable completely prior with one 20mg Prilosec, some days didn’t have to take it at all, has now been more annoying. I’m getting a referral to a Bariatric surgeon here in the states even tho the gastro told me I’ll be fine and don’t need to do anything. I’ve read that a twisted sleeve happens to about less then 2% of sleeve patients. I’ve also read some great success stories of surgeons fixing the twist and hernia without revision to bypass etc. I’m hopeful and not in any pain. But I don’t want it to get worse and it’s just a little overwhelming. Thank you!
  21. I did not have sleeve I had bypass, I have had pernicious anemia my whole adult life. I have a severe case and take shots every two weeks no change since surgery nor am I willing to risk the side effects from not taking my shots I had severe memory loss before I was diagnosed and my memories were not recoverable. B-12 your body only absorbs what you need you are better off taking a shot than not.

    I don't have a reply for your question because I haven't had surgery yet, but I'm curious if your doctor said why they recommended the bypass for those conditions? I also have PCOS and underactive thyroid, and have chosen the bypass, but didn't know there was a connection.

    Thank you so much for your response ChunkCat. Truly you make me feel so much better anytime I read your responses. I truly am sorry that happened to you too, I'm so glad someone stood up for you. I should of said something, but my anxiety kicked in and so I kept quiet and tried not to bother the nurses. I guess the surgeon felt I was already open so better something than nothing haha. Least that's what I feel happened when they saw my liver. I'm not mad at them just emotional. The surgeon visited me the next day and expressed he was sorry that he couldn't do the bypass. He also said he was very happy with the results and that he feels I will have just as much success if not more as the bypass. He also said that once I lose weight my liver should shrink more so we could discuss bypass down the road if I still feel strongly about it. I been on a waitlist for months to see a psychologist actually. This is because I have a lot if underlying trauma, C-PTSD, severe Anxiety, major depression disorder, etc. They also believe I could have more mental health illnesses, so they want to investigate so they can help me more. I think searching for a therapist also would be wise, but I struggle with opening up to people. I am very guarded. I also don't remember chunks if my past because I believe my mind blocked some out as a defense mechanism. I know there is bad stuff there but I don't remember it all if that makes sense? Sorry if I'm being a downer. 🖤 Also thanks for the suggestion of trying protein water!! I heard about those but I was nervous that they may affect my IBS too, but knowing you battle IBS too makes me feel more comfortable giving them a go. My IBS is so painful when it happens. It almost seems more frequent and even more painful now. I hope as my body heals and adjusts that this will not be as much of an issue. Thanks again for your input. Truly appreciate you.

    Courtnay, I am so sorry you had such an awful experience... I had some nurses that really irritated me and treated me poorly but an orderly found me crying because I was in so much pain and stressed by the nurse and so she reported the nurse, which was uncomfortable in the moment because I had to talk to a bunch of people, but really made them treat me decently the rest of the time! You can definitely have the bypass later once the sleeve takes off some weight. This happens sometimes with DS surgeries where they end up doing the sleeve first and then the intestinal part later once some abdominal fat is lost. Livers can be really tricky. Mine was still "plump" during the surgery, I just happen to have an extremely skilled surgeon who was able to work around it, but he told me sometimes that isn't possible. And I really want to say this, what happened to you IS NOT YOUR FAULT!! I'd find a therapist that deals with trauma, specifically medical trauma if possible, to help process this experience. Later down the road if your weight loss isn't stable you can consider converting to bypass or DS, you have options now with the sleeve! I know it isn't what you wanted, and I don't understand why the doctor didn't just close you up and try again later if you felt so strongly about it, but hindsight is always 20/20 and the good thing is that the sleeve DOES take off weight and that weight loss will help you feel a lot better. You should be on a good PPI with your heartburn, twice a day if needed. Omeprazole was always useless to me, so I use pantaprazole in the morning and Dexilant in the evening, both are prescription and they work wonderfully, no food triggers at all. You don't have to suffer with heartburn and reflux, they really should sort your meds out for that. It will make nausea and eating a lot easier. Oh and a few weeks out my digestive system was very testy. I had IBS before and it was grumpy post surgery. Fairlife shakes make me run to the bathroom for days for some reason! I like the Syntrax Nectar fruit flavors, they are like juice. Seeq is also good, it is like juice too, they are water based protein drinks, not creamy milk ones, and my digestive system handles them both better. You can get samples of Syntrax from Bariatric Pal and samples of Seeq from their website. Sending you lots of love... I hope things ease up soon and those hormones calm down, they are really rough after surgery! ❤️
