I agree, this is a whole bucket of emotions. I suggest journaling, get a new psychiatrist that has experience in bariatrics. First of all - you are still recovering from surgery, anesthetic and from lack of nutrition - the first few weeks I passed out a few times, try to keep fluids way up there, it takes work but it is very important, take a nap - it's okay. Soon you will be much better, and it won't seen so daunting. Good luck. -

I am 9 days post-op and I was not prepared for the emotional roller coaster that comes along with this surgery. Yesterday, I woke up with 0 energy and just feeling like an emotional wreck! Spent most of the day walking through a fog and crying. Luckily the nurse from the hospital had called to check up on me when I was feeling my lowest and reassured me that the way I was feeling was normal. That we were Psychologically starving and would naturally go through a lot of ups and downs as our hormones are shifting all over the place but would eventually settle down. She said we will go through spells of were we are burning up and then at times will be cold and that this is the body adjusting to our new way of eating. She said the gastric sleeve only gives you the capacity to eat 30% of what you ate before. She said before you know it your body will finally adjust and start burning fat and then you will have tons of energy and will be running circles around everyone else and not looking back. She said that is why you go through so many classes before surgery. She was very helpful to me and gave me information I had not been given. I did not tell her but In my clinic I had only been through 2 classes pre-surgery and that was the initial seminar that talked about surgery and why it was good for you and then only one other with the dietitian for the pre-op and post-op diet. I had only met the doctor twice which was the initial visit and then follow-up to labs and testing. So I don't think I went through what is usually the norm for this surgery and was much less prepared then most. I am so grateful for this forum as it has helped me with so many issues I was not aware of that I would face in this journey. the nurse on the phone had told me I should find a bariatric seminar near me and a pal to call when I am really low as they can talk you though it? My psychiatrist is so busy that it is hard to get an appointment with him and so I only see him about once a month and he hasn't been much help in this journey.

That is exactly where I feel my jejunal ulcerations pain. I haven't had an gallbladder since February 1984,when I had this pain flare up, I was scared. I had spent the entire month of August on a liver-shrink diet,andcall I could think of was " I've damaged my liver" and since I am 1 part worrier and 1 part overactive imagination, my mind marched right into " I had bariatric surgery and I'm gonna die anyway? WTF happened to me?" But Dr Noria, my surgeon Dr Needleman's partner and the person who has done all my endoscopies there, said the right subcostal pain, that one that made me want to dig my fingers under my ribs and rip something out? Purely the ulcers in my jejunem romping and stomping, "keep the Carafate going on every six hours, and if we keep you on TPN so your beleagured GI system can rest but give you adequate protein for healing, it's my professional plan that you'll be better soon" Hasn't happened yet, want it to--! but it seems to be taking forever! Then I remember there has only been 4 months since SURGERY and maybe I had better grow some "patience" flowers in my garden of Health. I have the rest of my life to enjoy my RnY, so this a " bump" in my journey, after all! -

I also don't know much about SIPS SADI-S Have you talked with your surgical team to rule out a medical issues? Can they give you diet instructions with calorie goal? I hope other people here that have GERD can jump in and give advice. 3000-4000 calories? I'm a vet and it does not make me an expert on anything bariatric. No one here can say what calorie goal to hit. We can give a best guess. You would benefit from a dietician who knows your surgery and medical background. Reality is you can't out exercise extra calories. I am not going to preach what you should do. Just some options and things to think about. Strategies for hunger: I agree with dumping protein shakes and bars. Real food is more satiating. You can easily get your protein goal in this far out from surgery. Dense protein. The bulk and fiber from veggie will help. Cut carbs and sugar they cause hunger and cravings. Eat small meals every two/ three hours. Log food in an app - Stay within your calories and macros (dont go over) After your protein gaol - Eat as much veggies as you want to fill your hunger. Snacking Cravings Head Hunger: Keep healthy sweet and salty options on hand. Keep a veggie tray in the fridge. If you have reached you calories and macros for the day and still feel hungry allow yourself as much veggies as you want. If you try to bargain with yourself for other options, you know its head hunger. Surgery did not remove my sensation of hunger. Especially with working out. I distance run and weight lift. I can eat in my weight loss deficit calories without being malnourished. I eat before/after workouts and stay hydrated. ** Running half marathons or further I eat differently. ** Stay healthy. I hope you stick around and post. Your experience with this type of surgery is going to help many people along the way. -

It really is amazing how many of us deal with mental health. And even more amazing that society often treats us like our weight is the reason. I'm working on my PhD in Health Psychology and I plan on doing my dissertation on how therapy effects the longterm success of bariatric patients. People forget it's a chemical imbalance. And when we have part of our stomach removed, not only are we losing access to serotonin (90%of it is produced in our gut, not our brain), which controls our mood, we're losing our main coping tool, and we're releasing hormones from our fat stores ! I made my own ground turkey (Italian sausage) meatballs without breading but with asiago and parm cheese and eggs. I brown them, then simmer them in tomato sauce (with garlic and Italian spices) for a few hours and YUM! Some recipes I found online mention adding almond flour. I'm not sure how to figure out the nutrition, but with no breadcrumbs I'm thinking they're at least close to the bariatric recommendations. I will have to look into it further once I have my surgery.

Proudest 2018 moment: worked really hard to lose weight over 2018. In Feb I had a bariatric appointment to discuss possible revision surgery and was 143 kilos / 315 lbs. Worked really hard even with dodgy lapband that either had no effect when too lose or terrible reflux whenever it was tightened and by October got down to 127.5 / 281 lbs for revision surgery. Every pound was hard won I tell ya. So proudest moment was when I got on plane to go to Belgium for my surgery and the seat belt fit without an extender. I was so delighted and happy! I'm four years out. I am a distance runner. I lift and train like a beast. Did you ever think you would have to work at eating more calories in your life? *smile* Transitioning in to maintenance mode take some adjusting. I used a TDEE calculator similar to yours. It says maintaining for me is 2166 calories. Honestly, I gain weight at the macros it suggests. I can burn 700 to 1000 calories on long run days. My diet changes to support my activity. https://tdeecalculator.net TDEE did not work for me. It may work for you. Your calories and macros will look different than mine. (age sex height weight activity is a factor) I had to change up the numbers from the TDEE to find what works for my body. Hate to say this but it has been a lot of trial and error. All instructions from dietician are different Below is my instructions Add low glycemic carbs Sweet potato, whole wheat bread/pasta and brown rice. If you are looking to get more calories in, eat small meals often. Eat as often as you need you need to get the calories in and stay within your calories/macros. Maintaining is a new mindset. It's no longer 3 meals two small snacks. Grazing has always been mentioned as a bariatric no no. Think of this as a maintaining health graze - several meals to hit your maintaining calories and macros. Wishing you the best, Jenn -
Has anyone tried IDEAL PROTEIN products? Good, bad, or blech? Worth the $$ or not? Any specific favorite products? Do they follow bariatric patient guidelines (high protein low carbs, low or zero sugar)? I'm still pre-surgery, getting my "homework" done; I don't have sleep apnea (yay), still pending are the two nutrition classes, cardiologist visit, and psych evaluation, then I'll be able to meet with a surgeon and get a surgery date

I also got a book by bariatric doctor matthew Weiner called a pound of cure . He suggests eating a pound of vegetables a day . Both of these doctors such green smoothies for nutrition and weight loss

Clothing while you're losing weight

If you had your surgery at an accredited bariatric center of excellence, try getting in touch with the hospital's bariatric surgery program coordinator (they are required to have one if they're accredited). They often manage a clothing exchange where people who have lost weight can bring their bigger clothes to trade for smaller sizes brought in by others further along in the program (or starting at a smaller size!). It's a great way to get free clothes that fit! If your coordinator doesn't have a clothing exchange, suggest that they start one! -

Yes, I am in the very beginning of pre-op. It sounds like you've definitely gone through it all! I have tracked my food in an app on my phone for the past 1 1/2 years fairly consistently to make sure I'm staying within my macros, so that data is definitely there for the dietician. I don't eat sugar, don't drink soda/carbonated stuff aside an occasional La Croix and use stevia sparingly in coffee. For drinks, I switch between Propel and Powerade Zero and plain water. Don't eat bread, no pasta, no grains, no beans/legumes, nothing inflammatory like certain oils and other ingredients. For the most part, I eat fatty meats, nuts, leafy greens, high fat dairy (sparingly) and I consume a lot of MCT oil - about 4tbsp + a day. My protein macros are set at 60g a day (which I sometimes exceed accidentally), because I'm not a powerlifter and I don't work out. During protein metabolism, unused protein amino acids can convert to glucose which causes weight loss stalls so that macro is important to not blow. Keto is very strict and I've done well, but not well enough. My job changed insurance plans unexpectedly which happened to include bariatric for the first time ever so I'm jumping on to take advantage of it. In 2017 I had a bad car wreck and nearly lost my foot and broke a ton of other bones so I haven't dared to exercise. Things still hurts from time to time, but I can walk. Even before the wreck I wasn't actively working out so pretty much all I can do is walk. I expect dietary changes post-op, that's a given. I just don't want to sabotage everything I've done on my own so far just because the dietician doesn't believe in Keto/LCHF. I might just be overthinking and assuming too much which is why I was curious about other people's experiences with their dieticians. -

Some dieticians can be quite stick by the book of what they were taught, others not so. Initially I guess its a case of one post op diet fits all, and then tweak as progress is made. I agree with Fluff be upfront, what your preferences are and why. There is scope with the macros that are provided to those who undertake bariatric procedures, and I think there is plenty of scope for increased healthy fat intake. Work out the percentages but do allow for the minimum of protein with the reduced calorie intake to make sure that lean body mass is maintained as much as possible while losing body fat. -

Honestly, my doc/NUT gave me the option of taking 4 Bariatric Fusion Multivitamins per day for all of my vitamins/calcium, so that's what I do. I found trying to plan individual multi/calcium too cumbersome. -

I used to take the Celebrate Chewable Multis, Calcium Chews, Iron Chewables, B12 sublingual, Biotin sublingual, D3 drops. But I got tired of the extra calories and carbs and switched to capsules where I can. I still take the B12 and Biotin as sublinguals though. I take these daily and get as many vitamins as I can from the Bariatric Pal store. They are pretty awesome and will get the order out mostly on the same day. I get my packages like within 2 days of ordering and they are very helpful with the order. I have a legitimate question, please? Speaking as a therapist and a RNY vet. What is it about taking away the supervised program that is attractive to those of you that are seeking surgery? Is it that it takes away one of the multiple hoops we already have to jump through or is it that it shortens the length of time before scheduling surgery? The biggest reason I ask is because it seems kind of scary for me because so many folks I work with (and folks here, too) are already saying that they felt so unprepared for life after surgery, I'm afraid that shortening the timeframes even more will make it even worse. So much of the medically supervised time should be for the "pre-work." I just really worry that this is a secret way for insurance to come back later and say "see, this bariatric surgery thing isn't working. All these people are gaining their weight back. We should stop covering it." I know, I know - it sounds like a conspiracy theory, but I deal with insurance companies not wanting to pay for services so often (3 sessions after a suicide attempt? I just saw my bariatric surgeon for my 5 week post op visit today. I asked him when I'd be able to eat a larger serving size, and he said: Plan on eating 2 oz of protein indefinitely, and up to 1 oz of veggies with it. Now this surprised me - in my literature from the hospital group (which he's the medical director of), it says to be eating 4-6 oz at this point. He recommends smaller portions but eating more frequently. Now I don't mind this too much, I'm just surprised. I get down 2 oz easily, and at 2 oz, I never feel that restriction so I never quite feel satisfied. He said if I never eat more than 2 oz, I won't stretch my stomach. "Stretching" the stomach slightly doesn't sound too bad to me if it means there's more room for veggies/nutrients. What do you all think? I'm just curious!

And I think you need some serious help. If you are doing coke and chasing it with alcohol, you have way bigger problems than can be resolved in a bariatric support group. Maybe check out some rehab centers.

Ah Darlin', this is not a normal and logical situation sweet Frustr8 is in. I am the rarer , watch out for your surgeon might have skimmed over talking about. I have a stomal stricture in Precious Pouch, and she has 2 ulcers also, on the back wall of my jejunem- the portion of small intestine the pouch attaches to, I have 3 more angry ulcers and the potential to ulcerate on down toward the ileum. I have had 4 endoscopy scopings, on every EGJ is still was not healing up, solider foods can't pass through, so I was on a liquid diet and still am, in theory. After my last scope I was hospitalized, that was November 28th, on the 29th a PICC LINE was established in my upper left arm, snakes through the venous tree, 48 cm, roughly 19 inches long, the tip is located in my superior Vena Cava, and rests only a couple inches from the right atrium , the upper chamber of my ❤. The line rested for a day then on the 30th TPN, which stands for Total Parenterel Nutrition, I get a feeding solution pumped in, I was on 14 hours on, 10 off daily, I just graduated? to a 12-12 cycle on January 1sr, I can try 1 meal of liquid something daily if I desire but the solution supplies my nutritional needs as it is. 1430 calories of goodness, Wowser that's about twice what I was taking when I was feeding by mouth. I simply count not ingest the 90-120 grams of protein Dr Noria says I needed to heal the verdompt ulcers, so this is an easier way for me. Oh I wasn't TOO THRILLED to have this happen to me, my son and I learned the steps so we do it all ourself, no stupid home health nurses having to do it. Twice a week I have blood draws, luckily they can remove blood from the PICC line, better than having someone go up and down my arms looking for an non- rolling surface vein. I used to look like someone threw rocks at me. I have lost over 100 pounds from my highest weight, lost 15 pounds on my liver shrinking diet in August , and close to 50 since my surgery September 5th, for a while I was protein poor and nearly malnourished , I was losing weight like a sea-sick anorexic sailor, nice losing but it was pushing me me into a depletion corner. Weighed myself last week at my Primary Care while my son was doing his appointment, looked like I was stalled by came home , did a giant #2, had to flush 3 times so I have to weigh less, tomorrow is blood draw and then appointment at Primary care for me, why they couldn't have booked us same day, is beyond my comprehension. But anyhoo, I shall weigh and see.And PICC stands for Peripherally Inserted Central Cathetern see you're getting a mini-tutorial today. How much longer will I have it? I have a surgeon appointment the 15th, another endoscopy the 25th with his Partner Dr Noria who love love loves doing them, she is also is a super competent Bariatric, Gastrointestinal and does do General Surgery but Endoscopy is her " Bang Zone" I really like her and Dr,Needleman, and have to believe they have a Plan of Care and have my best interests at ❤ but this sure is not the scenario I had planned for myself.👈😡👉💉💊

Mr bariatric surgery people took me from Wellbutrin 150 ER twice daily to 100 regular release three times daily but I am a RnY bypass, that may make a difference?At any rate I have a PCO appointment tomorrow, let's see if he thinks that is holding me or if he wants to add additional. A week ago was my birthday, an not-good day, really needed my psychologist or a psychiatrist, not suicidal, never have been, I lean more toward apathy and no one seemed to have empathy for me. But the New Year seems brighter thus far and may it please stay that way!

Hi, I am 22 months out and my cravings and bad habits came back at around 15 months out. I slowly started trying different foods and adding in bad food. I stopped logging my food, drinking my water, got lazy taking my vitamins and medication. By November first I realized how far off track I had gotten and I stepped on the scale for the first time in months. I had a 30lb regain, which crushed me. I changed some things and got rid of the bad food in the house and went back to basics. I struggle a bit through Thanksgiving and vacation after that but I managed to drop 14lbs. While I was happy about this, I knew I needed to really deal with my emotional eating habits. The first year was easy because I had no cravings. I really believed that I had it under control but I did not. I now realized that this is where the hard work starts. I started reading a book called Never Binge again which has helped me immensely...enough that I now know that I can hit my goal weight this year and that I have a plan and processes in place to help me when I am tempted. I purchased an inspirational food journal that helps me in the sense that it not just focused on food intake and macros. It's more about having a healthy outlook, mind and body. I had joined several bariatric groups on FB but I did not participate much. Now I spend a part of my morning in those groups communicating with various people. I also started a blog, which is terrifying, to be as transparent to myself as possible and also hold myself accountable for my food decisions, good or bad. We can do this...we just never give up and keep going when we have a set back. You can never fail unless you quit.

3mo post op labs showing high vitamin levels and above normal liver enzymes

I should start this out by saying I am not looking for any medical advice, this is mostly just to vent because I am frustrated and scared. I was sleeved on 10/15/18 and have not had any complications except for a kidney stone 1 month post op. For the last 2 weeks however I have been feeling extremely run down, muscle aches all over and daily headaches. I definitely felt like something was wrong with me, but in my head I thought it could be low vitamin levels or just my body adjusting to the weight loss. I am scheduled for my 3 month post op appointment on 1/22 and had my 3 month labs drawn last Friday, mostly expecting them to show low vitamin levels (although I have been compliant with my vitamins) and maybe that my thyroid medication would need to be adjusted (I have hypothyroidism). Well, the lab results that have come back so far have me really worried (lab results get auto released to me on mychart). My labs show that my B12 level and Ferritin level are higher than the normal range. My TSH was low, which i did expect. The most alarming result however is that my ALT, AST and Alkaline phosphate labs are significantly high. I know that those labs have to do with liver function so I am pretty worried. Also, the Vitamin A and zinc labs have not resulted, which is unusual for them to have not been released by now unless they are "critical" results (at least this is what I understand from the way things result to mychart). The only thing I can think that may have caused this is that I was taking additional Vitamin A on top of my bariatric multivitamin as instructed by my NUT, because my Vitamin A labs were under the normal range before surgery. I stopped taking the additional Vitamin A mid Dec. as instructed. I do know that Vitamin A can cause liver problems in high doses so maybe that is what happened... I have only taken a couple of Tylenol and I have not drank post op. Most frustrating is my PCP looked at my labs and said the surgeon "definitely need to review them". The problem with that is my surgeons office has terrible follow through. I called first thing Monday morning and conveyed how worried I was about the labs and I was told I would need to send them the results to review (even though they ordered them and should have gotten the results faxed from the lab). I sent them over and confirmed that they were received but did not hear anything back, and now here we are with them closed on the holiday. I do understand I am not their only patient and they are understaffed currently. I will call again tomorrow afternoon if I haven't heard anything as my surgeon only works M-Wed. Sorry for the long rant, I am just really worried about the potential for any liver damage.
