#16 Listened to the Keto Diet Podcast (Leanne Vogel), episode w/ Ashley Salvatori. I play with keto but I am not all in. Still can't believe that so much fat can ultimately be good for you. Especially since many of the people in my IF groups that do keto have terrible bloodwork. But i am open minded---and desperate!! #17 Did this last weekend, but I can't seem to dump my old clothes. they are piled in bags in the bottom of my closet. Day 3 of my no junk food challenge. I want to murder someone.

If you're only eating 6 or 700 calories a day and almost all of that is coming from protein, it sounds like you're not eating enough calories to support a healthy nutritional balance. You're also eating so much protein that you're excluding important micronutrients. I am a big believer in going ultra-low carb (love Keto), but to cut out carbs AND fat is definitely going catch up with you. You can live without carbs (except for fiber, which we really need a lot of), but fats and protein are both essential. Protein is very important--particularly in the month or two after surgery when you're recovering since amino acids are required for tissue growth, neurological repair and growth, hair growth etc. However, you can't really live long term on protein alone. First off, your body will convert protein to glycogen to compensate for the reduction in available glucose that you might ordinarily get from carbs. Protein molecules can be converted to glucose by your liver in a process called glyconeogenesis. It's an inefficient way to create glucose--which is a good thing in most instances since it means that your body consumes about three time the amount of energy to create glucose from protein than it does from carbohydrates. However, this process also creates a lot of unpleasant by-products and waste that needs to be processed by your liver and kidneys. If this is your primary/only source of energy for a long enough period of time, your going to risk liver and kindney damage and possibly failure. This is one of the things your Surgeon is checking for when they do your blood work every three months (it shows up in your creatine, thiamine and protein level reports). This will tell him or her if you're eating to little or too much protein. Second, you need to have fats in your diet. I am not talking about cheese burgers and bacon. We can all live without that crap. However, healthy fats are very important for neurological function, heart health, liver function and hormone production etc. You need to also be eating the right kinds of fats from things like avocado, fatty fish oil (wild caught sockeye salmon), extra virgin olive oil, ghee etc, nuts (pecans, macadamia, brazil nuts--all great). These are all great sources of omega-6 fatty acids, which are also very important for your immune function and will help you suppress free radicals and mutations that can cause cancer and other health issues. Third, even though I am a Keto evangalist, you do need to some carbs in your diet. Those carbs need to come mostly in the form of fiber along with some nutrient rich foods that will help improve your micronutrient profile. Sure, we can take supplements but most of those pills only mimic the vitamins we would otherwise get through a healthy diet. There not one in the same. That said, 20 net grams of carbs is more than enough to live on---but your total carbs (including fiber and sugar alcohols does not require any limitation). Depending your choices though, you can certainly have more than 20 net grams of carbs and still be very healthy and lose lots of weight. You just have to monitor your calories a little more carefully. Finally, even with your tiny tummy, you can easily get your calorie (nutrient) consumption up to 1000 to 1200 a day if you add in fats. A small handful of brazil nuts, macadamia nuts, pumpkin seeds or pecans will get you plenty of healthy fat, protein and fiber to fuel you through the day. Sure, these are calorie dense foods, but if you can't eat food in quantity, you should still try to get your calorie consumption up near 1000k a day. It's not going to really change your rate of weight loss significantly (it might slow things a tiny bit), but it will help you prevent regain to get yourself to a point where you can eat a sustainable amount of calories daily. You don't want your body to adapt to 600 calories a day and base your new set point on maintenance at that level. That's not a recipe for long term success.

So actually, "Nahmy" (Normy the cheers guy as said by Cliff Claven), aside from using ketones after WLS--it's a little known fact that some of the cutting edge research being done on cancer and dietary intervention today is in the area of the application of a ketogenic diet and supplementation with external sources of ketones. Turns out it may not be "full of gump" or hocus pocus. This researcher, Dr. Dominic D'Agostino is the one of the shiznits in the keto/low carb community.

Hello, I was wondering if you have done a little research on eating keto and lowering blood sugars. I really like the Dr. that started Dietdoctor.com. there is great information and videos. To see the videos, you do need to purchase a subscription at 9.00 dollars a month. However, you can do a one month free trial, just put in your phone the date you started, so you can cancel before you are charged. I kept the subscription because I find the videos informative. BTW, you can get the recipes free of charge. He doesn't charge for those. Good luck on your journey. PS. I had to find a Dr. that agreed to this lifestyle . You are eating whole fresh non packaged foods but still some Drs. do not agree. I do drink the bariatric hot chocolate however. It's my sinful pleasure, but low carb. You need some electrolyte powders to mix in your water. Look for one with a good amount of potassium and magnesium. It stops the "keto flu" symptoms like fatigue and headaches! A lot of people underestimate them. I did 3 weeks of a pre-op diet with no headaches or muscle cramps after the first day because I started taking those handy little powders! They help post-op too. -
Did your surgeon give you a plan to follow? I had a really hard time losing my pre-op weight at first. Then I went on Pinterest and found a bunch of low carb recipes. Following the keto method helped me drop a lot of weight easily, and I was always satisfied. Its so hard figuring out what is going to work for you and your body. I decided to stop eating simple carbs and focus on carbs from veggies. Using a tracking app like MyFitnessPal really helped me with portion control. And reading the book Never Binge Again helped me deal with my food addiction. It's an ongoing struggle though! I hope you can find a plan that works well for you! You can do it!

I develop a rash on my body Every Time my carb intake drops below 20 for longer than a few weeks. I have logged in MFP for years, and it correlates. Always on the trunk of my body. ( I hadn't even heard of 'keto rash' but assumed it was some sort of inflammatory body reaction). It is a rare phenomenon. If I upped my carb intake by 20 or 30 grams it would go away. I have had it multiple times. It is more annoying than anything else so I will just deal with it this year. It is now annoying than anything else and get a Costco size jar of hydrocortisone cream. I also am going to ask my PCP for a dermatology consult. Currently, I have it on my back. I have had it on my back, chest, stomach and neck. Every body is different and you should always listen to yours and be aware, note changes etc.

Ok, this is very important!!! Having the certainty that there is an "end" to the suffering is very critical. I'm pre-surg, but have been through really huge reconstruction surgeries on my boobs/back after a double mastectomy. And it hurt so badly, the only way I knew I could do it was because I knew that each day out would/should be better than the one before and that there would come a day when the suckage would be over. We can get through anything for a short time, especially if the reward is great. So I loved the idea someone said about thinking about all the good things coming in your future, as you endure each day and check that day off your list! Right?! Also, I have written this before, but my nephew is a surgeon and told me that any time you go in and mess with the gut (even just touching or moving it), you will deal with post-op transient depression. It releases hormones that jack with your mood cuz it's part of the gut/brain axis. Then too, you're not fully (most of us) keto adapted, so your mood is further affected by having little to no food, you're getting over anesthesia, you're bloated from IV fluids, not sleeping well, and in pain. It's the perfect storm for fatigue, crying jags, and daily naps. Treat yourself gently and with love. Nap when you feel like it. Move slowly. Take it one step at a time and don't forget to breath! ((hugs))

My nutritionist recommends 40-80g net carb per day at the nutrition class. I think that might be once you're on regular food again. I am very carb sensitive so this is fine with me. I spent months counting calories pre op and lost nothing until I started following the carb guidelines. I've only ever lost weight on keto or atkins so it's not a shock. My surgery is a month away and I've lost 7 pounds this last week from 297 to 290 staying below 50 g of carbs and 1200 calories. I got down from my highest weight of 315 to 265 on low carb but regained back to 297 after spending months on prednisone for rhuematoid arthritis last year

No you don't. I won't be doing low carbs either. Hate counting anything as well. And I have my reservations about Keto too. I agree with everything you've posted in regards to it, I choose not to say much about it cause then you get a some folks jumping on board, almost as if you're wrong. You're not. Do what works for you. If a Keto isn't it (definitely not me) than that's okay too. -

This. I avoid carbs like the plague, because I have a terrible carb addiction and the only solution for me was to purge them from my life almost completely (outside of cottage cheese and green vegetables). That being said, I agree with what @OutsideMatchInside has said. I can eat in a keto fashion long-term, so that is what I do. The difference between a diet and a way of life is sustainability. Find what works for you and stick to it. The important thing is that you are making progress toward your goal. -

Yes, me too- Id love to find Keto books and how they relate to a WLS Post-Op eating plan but apparently there are none. Considering the success of @blizair09 and the many others here that follow this type of eating philosophy, I will be following them and learning and soaking it all in. I am really enjoying this website and all of the knowledge here, it definitely makes me feel more secure that I wont totally blow it when I start eating solid foods again! -

Yeah I think in Europe, they use the term LCHF more than Keto, in the past to use an app that had settings for it, Lifesum. The yearly cost was too high last year (I just checked and they lowered it if you buy a year at a time) so I switched back to LoseIt. The Developers of Cronometer have settings for Keto also. I think I might spend the evening taking another look at Cronometer. I used to use it to make nutrition labels for my custom recipes. -

Yes, I am pre-op. The weight I've lost was a result of 2 years strict keto....you probably saw that in my other post. I don't believe there's a set expectation for weight loss. The lady at my surgeon's office said with the meds, they expect me to lose approx. 25lbs which should put my BMI around 38.5. I gained a good 5lbs during the holidays which they're unaware of. I can lose 13.5lbs at this point and be right at a BMI of 40. I just need to get to that appointment and find out what they're wanting me to take. I'm guessing it's phentermine which I highly doubt will effect me since I used to pop Adderall like Skittles. No matter what, I'm going to make sure this surgery happens... -
Slim Fast shakes are pretty high carb/calorie (even the "keto" ones). I looked at the "keto" ones the other weekend and was like WTF! Try 5 small meals a day. Or at least pick a better shake like Atkins High Protein or Premier Protein.

Hi Debbie. Slim fast shakes (even keto) are a 'no'. Get yourself some isopure Zero Carb (https://www.amazon.com/Isopure-Protein-Powder-Isolate-Flavor/dp/B0015R26V8/ref=sr_1_1_sspa?crid=2NYY6LR9OVRPW&keywords=isopure%2Bzero%2Bcarb&qid=1554828012&s=hpc&sprefix=iso%2Chpc%2C116&sr=1-1-spons&th=1) if you must have a shake--mix it with some fairlife, or even water. But at six months you should really try to be eating solid food at least two of your meals (3-4 oz of lean protein, 1/2 cup veggies). Snacks are ok --in my book at least--but something like 1.5 oz of cheese, edamame or cucumber slices. And no more than twice a day. Good luck!

Had my first consult October 13th. All my testing is complete as of last week. Was hoping to get a surgery date for late December but that doesn't look like it's going to happen. (Duodenal Switch) I've been reading about others having 3 day up to a week liquid pre-op diets up to surgery. My surgeon said no less than two weeks. I guess all surgeons have their preferences. I've read a few books and listened to as many podcasts about Bariatric surgery per-op and post-op. I feel like a have a good hold on it. I started a Keto/Carnivore diet back in June. Limiting carbs and Sugar wasn't a big deal starting a 100 Gram Protein & 100 Gram carb diet the dietician has recommended for me to be on now. I've lost 47 pounds since June and 27 since my October consult. -
5'2", βοΈ10m PO, maintaining 115.6 lbs Yesterday, Sun Aug 25: I swear, I felt like I was eating NON-STOP yesterday. Btw, another Spoiler Alert: I didn't even attempt the planned 10K run this morning, I woke up super-unnaturally tired and achy and meh despite the calorie and carb increase yesterday (Yeah the carbs were still on the low-ish side for a regular balanced diet, but not for me) This got me to thinking to another possible reason as to why I've been feeling so unenergetic/tired/un-10K-runnable: Keto Flu! I went from being super carbaholic for over a week and then back to 30-40g daily carbs. Sooooo, modifying my experiment: will keep carbs at a level I know I like (while still upping cals in form of fats) and see if this lethargy passes in a few more days and the 10K comes back to me... PROGRESS UPDATE I had my gastric sleeve surgery on December 18th, 2023. I was nervous going into the procedure as I had never had any surgical procedures before, but it went relatively smooth. I was in a lot of pain while in recovery, to the point where the team had to push oxy and fentanyl simultaneously to help with the pain. Once the team was able to help me manage the pain I was in pretty good shape. It has certainly been an adjustment but it wasn't as daunting as I was expecting it to be. Its been about 2.5 months since the surgery and I've lost almost 60 pounds; I weighed in on at the hospital at 366 and yesterday weighed in at 308. My highest weight was 475lbs so all-in-all I've lost 167lbs utilizing diet, exercise and the sleeve. I found it fairly easy to comply with the post-op diet and have moved on to solid foods. I'm not hungry often but I make sure to eat at mealtimes to continue meeting my goals. I consume, on average, between 80g and 100g of protein a day and consume less than 900 calories a day. I don't consume very many carbs as a) they tend to contain more sugars and b) they tend to make me not feel very good. I do still have a bit of a sweet tooth but that has been easy to manage with the occasional chocolate chip. I've also completely kicked sugar and have switched to monk fruit sweetener with erythritol. I also dabble with keto recipes, mainly deserts as I find that some "entree" recipes contain way too much fat for my liking. My family, friends, and coworkers notice the weight loss more than I do. I'll see it a little bit here and there but not to the extent that others do. I still see myself as that 475lb guy that struggles with his weight and his health. I know that it isn't true, but that is still how I view myself. My clothes are fitting me much looser and I think I see some weight loss in my face and neck. I still have a lot of excess fat and tissue around my midsection and especially the "love handle" region. I definitely suffer from body dysmorphia as I only ever see the overweight me. My overall goal weight is 250lbs, which is only 58 pounds away which sounds crazy. That said, I would throw a party if I reach 225lbs as that is my "best case scenario" goal. I genuinely believe I will achieve both goals but there's always that part of me that doubts it. This has been the best decision I've ever made for my health and I wish I had made this choice earlier. -

At one year post-op, I have cut out pasta, rice, and carbonation. I will probably never have any of those things. I still follow a keto way of eating; therefore, I keep my carbs below 25 grams per day. So, I have little to no sugar. That is my choice, not my body's demands or anything. As far as alcohol goes, I have some sort of alcohol (usually vodka, water and Crystal Light Lemonade powder that I take to the bar with me) just about every weekend, and when I travel for work (60%-70% of the time), I often have a glass or two of red wine with dinner. I have found that my tolerance is now back to about where it was before the surgery. I have absolutely no issue with the alcohol whatsoever. Like @OutsideMatchInsidesaid, I just allow for the calories on days that I am going to have drinks. (Though, to be honest, I need to get my calories up some to make the weight loss stop, so I haven't been as much of a stickler about it lately.) I haven't tried beer, mostly because of the carbonation issue. And as much as I miss it, and I do, I've adjusted to drinking other things. For the record, I first had alcohol at three months post-op, and there has never been an issue. I've lost 217 pounds (from almost 400 pounds), and am happily at goal (180 pounds). YMMV. -
Really??? LOL I actually think it's pretty darn different. The standard advice is to eat your protein (and the word "protein" in that context almost always means meat or dairy) first and then if you can fit in other healthy foods, do it. (As you mentioned in your post, fitting in 1-2 oz of broccoli around your protein, maybe, if you have room.) In contrast to this Dr. Weiner strongly emphasizes eating vegetables first and fitting in protein around that. He also recommends plant foods - veggies and legumes and fruit and raw nuts - ahead of meat and dairy and has fairly low protein requirements - about 50g compared to many bariatric plans encouraging 80-100g. (Of course he isn't vegan by any stretch and I've seen him recommend yogurt smoothies and small amounts of animal protein, etc. He even said in one video that he doesn't think that a fully plant-based diet is "necessary for the general population." So yeah, while we're claiming him he's not necessarily "ours," know what I mean? Just a good resource!) While Dr. Weiner's diet is "low carb" compared to our pre-op sugary diets, it's certainly not low carb in the bariatric sense. You can't eat many fruits and certainly not any beans and whole grains and still remain under 30-50g carbs. It's also low-fat. Difficult to be low-fat and low-carb at the same time! He also promotes eating in abundance instead of focusing on calories, a huge difference from most bariatric plans. You just need to eat your abundance from healthy plant foods (Obviously you still end up eating fewer calories because you're eating high-volume, low-calorie foods but it's the mindset that helps make a difference!) So on a spectrum with keto/Atkins on the left and veganism on the right I'd say he's definitely well across the center line to the right, while most bariatric plans are basically keto except with low-fat dairy and some wiggle room for beans. Sorry for the long post, Fluff - I am fully carbed again, having eaten an abundance of fruit, oats, and rice in the last couple of days that I've been free from my liquid diet. So I'm all peppy and ready to argue again!!! Heehee!

I was originally considering the sleeve, but my surgeon was concerned about the reflux so we opted for the bypass. I'm so sorry to hear you have to go in for a revision, but I sure hope it brings you some relief. I have a standard two week liquid pre-op diet, nothing special. But I'm modifying my Keto Chow protein to use almond milk instead of heavy cream during that time. Last night I made fluff for the very first time using the SF jello and greek yogurt - it was very good. So experimenting with recipes to see what I like/don't like cause I know I might too tired or need a family member to help -

Okay so you and I are having a similar experience and I have to say I have been feeling super frustrated because I see people losing 40 pounds in a month and I am closer to 20 3 months out....but, I recently saw my nutritionist and she said I am drinking too much water (which I didnt even know was a possibility?) But she told me to me very strict about not xri king before or after eating for an hour....the other thing she mentioned was super important...I had been doing a keto diet pre-surgery and it was really helpful and working! But, with my new tummy, I can't stay eating the same level of fat....so, I need to watch calories, cut out that cheese I had been ingesting....I have been working out, eating healthy....taking my vitamins....but, I am taking what she said into consideration and trying my best to lower my fat, increase my protein and not drink before or after meals...and I lost 5 pounds since seeing her....but, and I know this is just supposed to be a tool and not a miracle...I have worked really hard to lose each and every pound...oh and I am going bald, not very happy about that...sigh...
