    How did you do on your pre-op diet?

    @Spinoza. Do you realize you are now about half the weight from when you started! You must be doing something right and I envy you. I'm a guy and don't ever wish to weigh what you weigh. 160-170# and I'd be thrilled. I won't be having my surgery until later this year - probably duodenal switch. Absolutely fantastic for you. I know it was not "easy" in any way.

    Hi Mark, welcome. I noticed that you’re willing to treat other side effects of your weight with medications & tools - CPAP machine, meds for the high blood pressure, and knee surgery for the worn out knees… But you hesitate with GERD and constipation, both of which can be corrected with medications alone (over the counter options as well as prescription). Having the surgery could even correct your high blood pressure and sleep apnea, which would remove those issues from the list. Not to mention, weight loss in the long term is desirable for knee replacement. This leads me to ask, what are you actually scared of? Weight loss surgery is an extremely effective procedure for long term results, and the RNY gastric bypass is highly effective in treating or preventing GERD. I’m so glad you’re here, because there is a lot to learn and reconcile with when it comes to weight in our society. At the end of the day, getting to a healthy weight and keeping it off is what everyone here is working toward. You will never be alone in this if you choose to move ahead. Best of luck to you!
  3. Hi all, new here. Well, of course I've lurked here at times over the years but ran away every time.. A little bit about me, been heavy most of my adult life, managed to live fine keeping most side effects at bay. Sleep apnea, use a machine, HBP under control with meds. No diabetes or other issues. Three years ago I hit my highest at 450 and as covid got rolling I started to exercise a bit more and watch diet a bit better. Then all the wheels started to fall off lol. Knees giving me some problems which then led me to find out I was bone on bone for both knees. So I continued to get better on the diet and switched to biking and rowing. Phentermine/Topiromate helped along the way and ultimately lost about a hundred pounds. Had to stop those though when I had an Afib incident. Kept myself in the 350 range but man has this plateau been tough. Doc has me on ozempic, A1C was also starting to creep up, to see if that will help but it really hasn't done much. I'm ok with diet but still not where i need to be to lose another 75 pounds, which is my target to get knee surgery. I truly believe that once I get my knees fixed I can be active enough to aid the weight loss. It's tough now with the knee. So why am I here.... well my Primary and my cardiologist have both said it's time to really think about surgery. They both feel there are too many metabolic/hormonal challenges to continue my same approach. I met with the surgeon yesterday and he's recommended a sleeve. I'm at my whits end and feel like I have no other choice, but at the same time I read these stories about GERD, constipation and all these other side effects I turn around and say eff it, it's just not worth it. At least I'll die happy... I know, not exactly healthy, but I'm being honest... I've started the process, have a meeting with Nutritionist next week and psych and Endoscopy are after that. Likely can't get this done until August so I have some time to reconcile in my head the future, but right now I'm a bit overwhelmed and a little defeated. I'm going to poke around here some more, thanks for reading. Mark

  6. I would suggest something about the infamous three-week stall. It seems like there's a post almost every day from someone who's about three weeks out, frantic because they haven't lost any weight in several days. I really wish more clinics would tell patients about this early stall, because it happens to the majority of us, and most people don't know about it ahead of time so they freak out when it happens. also, something about how the rate of weight loss is dependent on many factors, some of which you have little to no control over, like genetics, how muscular you are, starting BMI, age, gender. And also not to expect to lose 30+ lbs the first month, because that's not common with people unless they're the size of the people on "My 600 lb Life". Just let them know that if they stick to their clinic's plan, the weight WILL come off. Their ultimate success is dependent on how compliant they are, not their rate of weight loss.

    Starting YouTube Channel

    I think a sort of "debunking" themed episode would be good....answer questions like "Is this the easy way out?" "Will I gain all the weight back?" (I mean honestly this forum is a gold-mine for myths and misconceptions if you go looking for them). Something along the lines of "Why am I not allowed to eat xyz yet but my friend who used a different surgeon is?" That seems to come up a lot; the question of can I eat this yet and why/why not? My biggest piece of advice for YouTube is watch your analytics to see what your audience responds to, try to take a little extra time to make your videos look polished and produced -- especially for a medical topic, you don't want them to appear amateurish. Don't use copyrighted music if you're planning to monetize later. Tutorials are your friend (YouTube taught me everything I know about video editing and cameras!)

    Agreed, your starting weight is actually fairly slim in comparison with a lot of people you see on here; less to lose means it may come off more slowly.

    So sorry you are experiencing this. I think you need to find another bariatric surgeon & discuss your options. A revision to bypass will help you lose more weight (maybe not a lot but some) so yiu can look into knee replacement surgery in the future & it certainly will help with your gerd. You can not survive on shakes & protein bars. Besides not getting the nutrients your body needs to function effectively, the low calorie diet will be slowing your metabolism down more. You did lose 100lbs. That’s almost a third of yourself. That’s something to celebrate. Remember the average weight loss is about 65% of the weight you have to lose to put you in a healthier weight range. So, not knowing your height or what would be your healthier weight, I’d expect you may have been pretty close to that statistic. So your stall, which are very common & can occur many times, may have been you reaching your new weight set point & stabilising. I find it ironic that your surgeon complained your sleeve was too big. I mean they did the surgery so if the sleeve is too big that’s on them not you.

    Yep, your fertility often increases whike you are losing weight. Estrogen is stored in your fat & as you lose weight it is released into your bloodstream causing an estrogen surge. Many surgeons recommend trying not to fall pregnant while losing so you can gain optimal benefits if the surgery hence the advice to double down on your contraceptives. Of course it is your choice whether you try to fall pregnant & sometimes things are meant to be.

    That’s probably a conversation with your surgeon. And it depends upon what hormones you are talking about. There are many hormones produced through out your body that have roles to play that are not related to your weight directly or indirectly. For most of us it is our weight that causes our hormones to be unbalanced, unless there is a medical condition like diabetes, over/under active thyroid, etc, so losing weight will help to balance out your hormones. Most hormone diets will advise you to lose weight by adopting a low processed whole foods diet, high in protein & be more active. This is what yiur post surgical plan will look like anyway. They often say manage stress & sleep more - both will be helped by losing weight. The surgery will reduce the amount of the hunger hormone ghrelin that is produced (it’s removed with your tummy) but your brain does eventually compensate by stimulating other areas of the body that produces hunger hormones (motilin in your intestines) & your hunger does return. The surgery will also give your metabolism a boost (but you have to keep it burning otherwise you’ll lose some of it’s effectiveness). Dietary changes will also help to better regulate your hunger & satiety hormones. There is evidence to show a diet of high processed food will increase the production of your hunger hormone & reduce production of the hormone that tells you you’ve had enough. Estrogen is stored in fat so as you lose weight you’ll first get a oestrogen surge & you may notice changes with your menstrual cycle & it can make you more fertile. It’s temporary. Your cycle stabilises once you get close to your goal as your weight stabilises. Though you may notice you don’t have as many issues or symptoms with your cycle. And may help stabilise your weight a little too. Losing weight will also help reduce any diabetes issues you may have & better regulate your body uses insulin.

    Hey everyone, So I've finally come to terms that I may need help. I've tried to lose weight since I was a teen and I'm now 29 with no success. I've hit my highest weight of 250lbs at 5"3 and it's extremely depressing. I know I'd technically qualify based on bmi but every clinic I've called has said I need a referral even though I have a PPO through Covered California. Does anyone have any experience with covered california blue shield of California? I am meeting with my doctor tomorrow and in the off chance that she doesn't want to provide a referral, I want to know my options. Thanks in advance

    Pre-op food tracking

    I've tried apps in the past, but figuring out the calories, carbs & protein and all for things I made at home I found very frustrating. During my pre-op clearance process (surgery is scheduled for 3 weeks away) I was placed in a weekly group session with psych. One of the things we discussed was keeping a food diary. We were told in the class that food diaries are the #1 best predictor of weight loss. What helped me was when I was advised not to focus on the macros, or calorie counts but to focus on what I ate, when I ate it and how I felt. I whipped up a spreadsheet on Google Sheets and just go to it on my phone and list it down, it was frustrating at first but I found it has really helped me these last two months that I have been doing it. I'm hopeful that it has helped me be on the right footing in my post-op "new life." I hope that helps.
  14. I was sleeved 08/2017. It's been a constant struggle. I was told by the surgeon that I wasn't progressing as well compared to others. That my sleeve is bigger than he wanted & my metabolism is slow & I needed to work harder. I do have severe GERD but I have learned to work around that. I found out I have tachycardia. Also then & even worse now mobility issues. I couldn't vigorously exercise due to having grade 4 Osteoarthritis in my left knee. I stuck to the program & went from 323 to 223. I stalled & went into a deep depression. I wasn't losing unless I ate 800 calories or less. I gained all of my weight back due to decreased mobility & depression. I fell off the wagon per say. I now need a hip & knee replacement plus my right knee is almost just as bad as my left. I'm in chronic pain. For me to have surgery I need to lose weight. Nobody would do the surgery a few years ago when I was thinner stating I'm too young blah, blah. My quality of life has dwindled the last 2 years and I gained it all back plus 20lbs. For the last 8 weeks I have been using my fitness Pal to login my food diary. I'm eating every 3-4 hrs mini meals. Substituting 2-3 meals with protein shakes/bars to obtain my protein goal. I would lose weight 1 week than the next gain back & forth. I don't understand. In 8 weeks I've lost 12lbs. I went from 343 to 328 to yo-yo 331. I have experimented & it doesn't seem to matter if I eat 800 calories or 1000 or under 1200. I gain/lose weight. I always eat no more than 1200 and no less than 800. Does anyone have any tips.   15. Surgon told us (my daughter & I) that if that happens go back to a full liquid diet for 2 weeks & then start eating the way he told us at the beginning. No more then 3.5 oz at a time, 3 meals a day with small healthy snacks in between. Or check out the Fast metabolism diet by Haylie Pomroy. Even though there is no way I can eat as much food as she wants a person to eat. Following each of her phases in ordered helped me get down to my goal weight!

    Starting my ESG journey!

    I went well. I was back at work on monday after my sx on a Thursday. I have not had any issues. I lost about half the weight in 3 months and I lost the other half in 3 more months but I did take Mounjaro for 2 months to help me get the rest of the weight off. My surgeon prescribed it for me. It made me sick and constipated so I quit taking it. I was close enough to goal anyway and finished the rest by myself. I have maintained now for a month.

    Please send me that information. Can I do you here with my email and all? I am ready to get back to having a life. Doing music again. Taking trips. Not having to worry about if I can go somewhere as I might not be able to get into someone’s car. Part of that is being so tall, but the weight definitely doesn’t help. I think what I’m looking forward to most is just doing things that seem normal to people that aren’t obese. Things like sitting in a booth at a restaurant. Flying on an airplane. Fitting in a normal sized car instead of only a dang suv. Shopping for clothes at regular places and being able to go up stairs and downstairs without gasping and panting. Im so ready to do this. I want to be active. I want to stop turning into my desk chair.

    You started at a relatively low BMI. Many low BMI folks, including me, lose all or almost all of their excess weight. You're going to do great. Just stay focused and follow the plan.

    I have no idea what my actual initial weight was because I refused to buy a higher capacity scale, so my heaviest measured weight was 396. But I’m sure I was more than that. All those things you mentioned struck a chord with me. I remember having to ask for tables, not booths, and then having to size up the chair to see if I could actually sit. I didn’t have an aneurysm but I had uncontrollable hypertension and was getting precursors to heart attacks. So I’m here to tell you that it all changes, and very quickly. I’m 18 months post-op. I didn’t hit the goal I wanted but I did get below 20% body fat and am within 10 pounds of my lowest weight of 262. (I’m six feet tall and built like a brick sh!thouse even without the fat). I eat intuitively now (after therapy to help get me to have a better relationship with food) and I exercise a lot. I’m running a 5K on Sunday and it’s… just not a big deal. I fit in booths and I can run up and down stairs and jump to a 30” box. I am not on any blood pressure or blood sugar medicine, and I no longer need a CPAP. I wish I would’ve done this a decade earlier. Here’s the thing, though: the program where I had my surgery done was not that useful. I ended up with a nutrition coach I hired, who got me eating correctly while still losing weight hand over fist. I had already been doing CrossFit (yes, at 400 lbs—the coaches simply scaled the workouts so I was getting the same stimulus as the jacked beasts in the gym but at a level I could do) and building muscle helped with fat loss and body comp. I needed therapy and psychiatric help to resolve my anxiety and depression. You’re gonna do great. Write down all the stuff you can’t do now and use it as a checklist for later. You’ll be stunned how quickly it happens. Just make sure you have the support team with you, don’t rely on the bariatric program; they’re a hammer and everything they see is a nail. I’m happy to pass along contacts for the nutrition program I started at 3 months post-op, or give details about my exercise program, I just don’t want to look like a spammer.

  24. Yes, you most definitely can! But it is all about mindset and what you feel you are deserving of. You must feel "worthy" and trust that your body loves you and is always working in your favor, not against you. Even when our bodies gain weight it is because it is protecting us in some way.
