The best that I can think of would be Dr. Ara Keshisian out here in California, as he is one of the handful of docs that can do the complex RNY to DS revision, and he seems to be able to do other complicated procedures that others can't or won't do. Closer to you, Drs. David Greenbaum in NJ and Mitchell Roslin in NYC are also well regarded for complex procedures and worth consulting. Good Luck....

    Revision of RNY

    Thanks for this info. I never thought of a home health agency but its worth the cost to get this moving along. I even told the doctors office about my dilemma and they had no suggested that but maybe I'm the first patient who had this issue. I have some friends I could ask but they are not reliable and I fear I would be stuck the day of the procedure without someone able to be there. In my case since its quite a distance from home I would probably stay local in a hotel near the hospital if I go with the first surgeon I met with. I figured I would stay a few days in the event there are any issues afterwards, i would be close by. I am attending a WLS seminar at a local hospital to where i live next week, mostly to get more info on the support after and such. Then if i feel they fit my needs in that regard I will make an appointment with a surgeon there and see my options. I want to be successful this 2nd time around and I want to make sure I get all the info I need especially in areas where I failed 20 years ago. Here in New Jersey in the northern part of the state there are not as many surgeons doing revisions and if they mention it on their sites they explains how risky it is and they dont take many candidates which worries me if they don't have as much experience with revisions. Southern part of NJ I find sooo many options and I have people who respond to my posts with their experience.

    I am so happy you have made a decision for you! I know in my case I feel like I am going at this alone as there are not too many people who have had a revision to their RNY before. I am ok with this as I know it is what I need. More than anything I am happy and will be happier when I am finished with the surgery (2 weeks away).

    I haven't made a decision yet. I have only been able to find 1 person who had their original surgery with the surgeon I met with. She ended up going for revision in Mexico I believe. She didn't have bad things to say about the surgeon but she said his attitude is he did the surgery the rest is up to you so she suggested I find support groups and a nutritionist to see after since he does not provide that type of support. Those are two things I know I have to do to be successful regardless of the surgeon. I had asked his office if there were any revision patients who would speak with me about their experience but they have no gotten back to me in about a month now. I am going to meet with some surgeons in my area where I live and see what they recommend in regards to revision. The original surgeon I met with is 75 miles from where I live and I have found difficulty finding someone to come with me just for the endoscopy to even determine if the procedure is right for me. Its tough not having support through this process. I am hoping if I can find a surgeon closer to home it will be easier to find a friend who can drive me home after the endoscopy and ultimately the surgery.

    Revision of RNY

    Update- I just had an endoscopy performed. We found out the pouch is quite large and there is a fistula connecting the pouch to the remnant stomach. This info is being presented to the insurance, and hopefully the revision is approved. Prior to the procedure I decided to pursue the revision despite the predicted results. I felt like I owed myself the opportunity to try again. In the interim, I've been meeting with a nutritionist to improve my eating habits. Im happy.

    Why do so many sleevers get GERD?

    This was a concern of mine too. Especially on the Facebook bypass support groups I was on. They were saying that 25% of sleevers end up getting converted. Yikes! But you can’t believe everything you read on the internet and so I went looking for more reliable sources of information - namely medical studies. Sorry I didn’t save the links so you can read them for yourself but I have screenshots of the information I wanted: As you can see, the numbers are actually quite small! So don't feel you have to worry about getting a revision - statistics are in your favor!

    Why do so many sleevers get GERD?

    I didn’t have GERD/ acid reflux until after the sleeve. I ended up with 4 ulcers in my esophagus. I just had revision surgery to the gastric bypass on Monday.
  10. Trying to compare like that is very hard. I am sure there are an equal amount of side effects and benefits to both. This is why you have a surgeon to discuss it with that should be able to advise the best option for you. For the record most of the extra risk with the bypass is at the time of the op, it is a more complicated surgery than the sleeve so requires a more skilled, experienced surgeon. I had a revision from band to bypass in Jan and am very happy. I didn’t want a sleeve as i know so many people that have one and the majority seem to have either not had good loss or have major weight gain, I know this is also possible with bypass but I don’t know many people that have one. My surgeon also told me that with my history of reflux he wouldn’t consider sleeving me. At the end of the day the most important thing is the commitment of the patient. Realise the surgery is only a tool and that success requires hard work.

    June 2019 challenge

    6/28: I'm in control of what I can control... So very very true. I need to be revised to RNY for GERD. At first I was was shocked, then all for it, then felt like it was totally my fault but testing confirmed it was nothing i could have done to prevent it. So far im totally just rolling with it until its a reality. It feels surreal for sure. Especially doing a 2 week preop diet while being at goal 😭 but ... C'est la vie 6/29: My past doesn't define me, but helps me remember the future I'm fighting for 6/30: June has been a whirlwind for me, not necessarily bad but definitely filled with some... moments I'll never forget I truly appreciate you @Cheeseburgh for throwing an amazing party in here... the Skalender tried to kill us in the end 😭😂 🙌🙌🙌🙌🙌🙌🙌🙌🙌🙌🙌🙌🙌🙌🙌🙌🙌🙌🙌

    Unfortunately, i did not lose much weight as anticipated. The day of surgery i was 335 and my lowest was 260. That's with adhering to the diet given (low fat, low carb) and moderate exercise. My weight fluctuated for years and i met Dr. Roslin at a seminar where we talked revision options.   14. Reflux feels like fire. BUT Only testing from your surgeon will confirm the need for revisions. Are you still taking medication? Talking to your Team about your concerns? Asked for testing? Are you currently experiencing symptoms or wondering about the future? Good Luck!
  15. For CIGNA, it is a 1 bariatric surgery in a lifetime thing. They will never pay for a revision, period. My BMI when i met with 1st dr was 56 (down from over 60 when I started doing keto 15+ months ago). 1st Dr actually said he would do a bypass as well due to my very high BMI and the need to get weight off faster (both knees have severe arthritis and I have to get injections to be able to walk and that is even lately...even with 52.3 lbs lost..getting progressively more painful). Knee orthos (got 2nd opinion) have said both knees are so bad they require full replacement and as soon as my BMI is under 40, they will consider doing the operation. 1st dr though, felt uncomfortable with my clotting history so he said he would only do sleeve at this time and to save up my money (which I don't have) to pay for my own revision to bypass with duodenal switch in a yr or 2. I still have mixed feelings about the 2 different surgeries..things I like and don't like about the both. Do you think I should still reconsider? I'm asking this as I'm wanting to make the best decision..not just what either dr's have said...and I'm torn although both said I needed the bypass originally.
  16. @Qbaby08 We're twins!! I had my revision with Dr Illan on 6/15 also. I was actually his first patient of the day. I only know of one other person from that day which was Clara, from Cali. We were driven across the border and back together. Not sure where you were hiding. 😉

  21. I just had a bypass as a revision from a sleeve. It was done about 6 weeks ago, and the sleeve about 10 years ago. I had terrible reflux basically since the sleeve was done, as well as a hiatus hernia. I have had zero reflux since the bypass was done! It definitely worked for me.
  22. Some of you may know that in the 11th hour, finally listening to my gut about my original surgeon and his anesthesia team, that I made a switch in doctors. After having gone so far down this road to where I was scheduled for surgery this coming Monday (7/1) with a doctor that had good reviews and a recommendation from someone I respect, I no longer trust myself as much as I did. Dr. Chad Carlton has great reviews as well and they go back for a much longer period of time which is 1 thing I like. However, I am so....well, to be honest I feel quite shaken up about the whole thing even though I feel I did the right thing. Does that even make sense?? While I found that I was relieved initially and my whole family is glad that I changed....my emotions are all over the board since last night. Maybe it's the change of 'I was going to have surgery on 7/1 and now the date is back up in the air..but I'm into week 2 of the liver shrink diet and I was to start bridging to Lovenox (blood thinner) today'...and I know Dr. Carlton was a bit taken aback at the amount I was prescribed. Oh, and switching from sleeve to bypass. Original Dr felt I was too high a risk to do bypass due to clotting history & said we'd do a bypass instead 1-2 yrs down the road (however with Cigna, you only get 1 type of operation for entire LIFE)...original dr said to 'save my pennies'. Dr. Carlton said while yes, my clotting history is a risk (I have a permanent filter) but I come with no other co-morbidities like high blood pressure, high cholesterol, diabetes, etc. that also introduce 'risk'. He said he has done over 6,000 procedures and does mostly revisions to bypass or straight up bypass (he also is 1 of the few in area that has a lot of experience with gastric bypass with duodenal switch?). I have 150-175 lbs to lose and he said that he did have some sleeve patients that were super diligent and were able to lose that kind of weight with just a sleeve...but I know that for many with this much to lose, a bypass is..what...still the "gold std"?? If anyone has any experience with this doctor, could you please let me know. While his reviews are really good, so were the reviews on my other doctor. I don't want to leave ANYTHING to chance. I'm at that point where it's "trust, but verify".

  24. Everyone responds to surgery and pain differently. I had a revision from band to rny and didn’t have any real issues. As far as I recall my pain was well controlled, I had a pump that I could press when I needed to. Even when I had a problem a couple of hours later where my BP went through the floor and I had to be taken back into surgery I wasn’t in pain. Cold, light headed, felt like I was fading but no pain. Waking up after the second op in ICU I still don’t recall being in pain. I just remember thinking I’m alive and being very happy about that. I had the pump things on my legs, a catheter so I didn’t have to get up and a lovely nurse . I did however feel as though I had been hit by a bus in terms of energy etc and felt like that for a good couple of weeks after the op. As for it not being pleasant well that is to be expected, you just got anaesthetised and your body cut up and rearranged, you didn’t spend the day at a spa! Let your nurses know when you are in pain so that they can give you painkillers. All the best , hope you feel heaps better soon
  25. This is another of those "it's anybody's guess" things. There is a different Blue Cross organization for each state, and they will all vary on their coverage details depending upon local market conditions and state laws. Then there is a difference between individual plans, ObamaCare plans and employer provided plans - all of which can vary depending upon what options the employer chooses to buy. Some plans don't cover WLS at all (even if they did in the past) while others limit the benefit to only one WLS per lifetime. Muddying the waters further, if the revision you seek is due to complications of a previously covered surgery, then it is not a WLS procedure but is treating that complication (it sounds like it to me, but it isn't my decision.) There should be policy bulletin for your BC policy online that spells out what applies to your policy - what they cover or exclude as relates to WLS. You can call their number, but don't take their answer as the last word, as the customer service reps have been known to be wrong. When you find a surgeon to handle your case, their insurance coordinator should be able to tell you exactly what your policy covers, as it is their job to know the lingo and understand all of that fine print. I understand that some surgeons are averse to working on other surgeon's patients (fixing others' mistakes...) but that is not an exclusive thing. One of the docs that I associate with (he runs out support group now) does a fair number of RNY to DS conversions, which as he doesn't ordinarily do RNYs, means that he is fixing other's mistakes (as well as fixing wonky sleeves that others have left behind) so they are out there - you just have to look. Thinking about that a bit more, if a surgeon doesn't want to revise other's patients, then he is either making a lot of mistakes himself that need revising, or he isn't very experienced in doing revisions, so you are better off searching for someone else.
