    Nausea and low mood

    That was me pre-op and at my heaviest weight which was 320. I was on anti-depression meds years ago for a short time but found they made me worse. So don't be quick to get on any unless you find you can't shake the blues. And even then, the right type and dosage is very important. For me... I was so sick and tired of being sick and tired, it didn't matter how shitty I felt after WLS because I knew better days were ahead. I powered through the nausea. And yes, I had a few down days too. Recouping from major surgery. Hormones shifting. I can't relate to the reflux because I had RNY and hernia repair so that resolved my issues with it. At 10 months out, I feel like a million bucks. I'm hoping the same for you. Congrats on losing the 35 pounds and GL going forward.

    NO TRACKING ?

    *raises hand* 5+ year tracker, here. been logging my food intake since day one. im not as anal/precise about it as i was in the beginning, when i weighed and measured EVERYTHING. even spices and items with single digit calorie counts. now i kinda eyeball my food to determine portion size and sometimes just log a guesstimated calorie amount for an entire sandwich instead of breaking down the ingredients. i don't log when im on vacay anymore. but i DO still log. (oh and i still weigh myself every single morning- if i have access to a scale) for what its worth, personally i believe my tracking habits in the earlier years contributed to my successful weight loss and maintenance...if my weight started going in a direction i didn't like, i would know (because of my daily weighing) and i would generally also know why (because of my daily food tracking and activity monitoring), and be able to take appropriate action before it got out of hand, you know? at this point though, for me its more of a habit/hobby. i sorta feel like i'm pot-committed...i have all this data from the past 5 years and i feel like i should just keep going cuz it would be kinda cool to have like 10, 20, 30 years of data someday? i dunno. maybe i am still anal. in any case, i reached goal in 7months, and maintained below that goal weight this entire time. I am 5+ yrs post op i realize this M.O. is not for everyone and for some (most?) this level of effort would be more detrimental than helpful. bottom line: you know yourself best. do what works, don't what doesn't. good luck! ❤️ p.s. sorry this was so long

    What does your day of eating look like?

    I am six months out and my typical day is: Breakfast - 20g protein yogurt, 10 almonds Lunch - Tuna salad - 15 g protein Dinner- 3.5 oz broiled chicken thigh - 24 g protein Snacks - Kodiak Bear Bites- chocolate graham crackers - 5 g protien, Protein 2o 20 g protein water, pistachios or other nuts, peanut butter on seed cracker - to hit goal of 85-95 g protein. I try to keep carbs under 60 g. Re: Quest protein chips -- liked them before surgery, stomach doesn't like them now (I get nauseous). Good luck!
  4. I don't know if this helps, but when I saw the nutritionist last week for my last pre-op appointment, she suggested choosing three regularly spaced meal times, 4 to 6 hours apart. I've decided on 8:30, 1:30, and 6:30 because it works best with my daily work schedule. She said to eat what I could of "real" food at the mealtimes, but not to let myself keep eating beyond 30-40 minutes. My goal is 20-25g protein at the meal, but let's say I managed to eat 10g for breakfast. In that case, I would want to supplement 10-15g of protein shake at 11am (halfway between breakfast and lunch). If I managed to eat 20g of protein at lunch, then I could skip the supplement between lunch and dinner. But if I only got 5g protein at dinner, I should supplement with 15-20g of protein shake a couple hours after dinner. That way you're trying real food first, but not letting yourself get behind as the day goes on. She said it would take quite some time to ween off protein supplements completely and that was fine. Eventually, she wants me to be at 3 meals and no snacks with 60-80g protein per day. If you don't want to rely on protein shakes but you find you can easily eat something like a Greek yogurt in between meals, you could do that instead. But basically it's just going to take time, and even after you are at a point that you can usually get all 20-25g of protein in a meal, there still may be some days where you can't and you need a supplement.
  5. I had the Duodenal Switch, which is different than the most common two surgeries being discussed here. I picked it because I had type 2 diabetes, high blood pressure, high cholesterol and triglycerides, sleep apnea, etc... I also picked it because I knew a purely restrictive surgery wouldn't work for me. Portion has never been my issue. Pre-op I was eating about 1000-1200 calories a day, for about 10 years, and did nothing but steadily gain weight. My surgeon said it was obvious I have a metabolic disorder, so I needed a metabolically strong surgery. I also didn't want to risk gaining weight back and the DS has the lowest rate of regain. But as a trade off it can cause more vitamin and protein deficiencies if not managed well and the diet is a bit different than the others post op because of our malabsorption. However, diabetes was my biggest concern and the DS puts about 95% of T2 cases in remission, and most stay that way. Post op my blood sugar dropped to 82 within 24 hours of surgery and my high blood pressure normalized, so they took me off of my diabetes meds and blood pressure meds. I'm 3 months out and my a1c is 4.8 without medication!! My blood pressure continues to be normal without meds. My cholesterol and triglycerides are also normal without medication, which hasn't happened in decades! All this to say---consult with a surgeon who offers ALL the surgeries if you can find one in your area. Talk to them about your reasons for wanting bariatric surgery and consider your options carefully. The sleeve surgery is amazing for a number of people, but it isn't right for everyone. The bypass is a fantastic surgery for certain patients, but again, it isn't for everyone. And the DS is a powerful surgery, but it definitely has a specific patient profile. All are great surgeries, but not all of them will be the best surgery for you. Only you and your surgeon can decide that. And don't be afraid to get a second opinion. I went to two surgeons before deciding on one (though both recommended the same surgery). I needed that extra reassurance I was picking the right surgeon and the right surgery. I'm so glad I did this surgery, I just wish it had been an option available to me when I was younger.❤️

    Eating With The Duodenal Switch

    I had the SADi DS not quite 10 years ago. I eat throughout the day because I like to, and I can easily eat a footlong Subways Stake & Cheese or anything else. The stomach restriction wore off after about 2 years. The weight stays off due to malabsorption & daily exercise. The downside of eating so much is gas and more frequent #2's. Generally, 2 to 3 times a day is normal but no issue and seldom an emergency. Also, when I have a morning bowel movement, I feel fantastic afterwards and happy to shed all toxic waste out of my body. I never get constipated but I do eat a lot of fat everyday in the form of cheese and Ice Cream.

    4 yrs post VSG to RNY

    Just a little 10-15lbs mostly because of the stressor of surgery itself (but I regained it and lost it again) My surgeon was keen on me staying in the 26-28 BMI range so he suggested to not make the bypass length his standard 150 cm but 120-130 cm instead.

    Hi there, i’m 25 days post op and had a horrible bout of nausea yesterday. Completely out of the blue, I was out walking, stopped for a cup of weak black tea which I drank sitting outside in a bracing wind, when I stood up, it came over me like a wave. Horrid. At 10 days post op I’d had a couple bouts of nausea, mostly at night, when asleep, and they would waken me up. Treatments from my surgeon include a nightly cyclizine, stemitil wafers and a morning lanzoprazole. I was surprised to feel nauseated yesterday, cause it had been a couple weeks since the last bout. hopefully it will become even less frequent?? well done on going out yesterday. It’s good to get moving.

    10 days post op and I decided to go with my BF to do the grocery shopping. I figured the walking would be good. Half way through, I started to feel nauseous. I think it was all the smells from going up and down the isles. I went to sit in the car and just being outside with the breeze helped subside the feeling. Anyone else have issues like this?

    Feeling very emotional and regretting

    No scars from the reaction, nothing permanent. But at the time, I had itchy, red, raised rash all around the surgical sites. Warm to the touch. The red areas were a little swollen. After about 10 days of treatment, everything went back to normal and was fine.

    I'm so jealous. I also had surgery 1/24 and have lost 7 pounds. One of the reasons I did the VSG was because when I would try to lose weight, it would come off so slowly---like it would take maybe 6 weeks to lose 5 pounds. It took me 8 months to lose 30 lbs and that was with Qsymia and eating about 1000 calories a day. I would get so frustrated and just give up, I'd go back to my regular diet (which wasn't crazy--usually 1500 - 1800 calories a day) and everything I'd lost would come back. Now, given my usual speed of weight loss, 7 pounds in 9 days is great. But on this liquid diet where I'm getting 300 400 calories a day max, I thought I'd lose more quickly. I was hoping for at least 10-12 lbs by now. Still, I'm happy that I've done this; it's going well and I know that I will succeed. I was just hoping for it to be quicker!
  12. I just had bypass as a revision as well about 10 days ago. The last two morning I woke up with feeling something in my throat accompanied with sore throat feeling? But no acid taste. I really hope it’s not reflux!

    I have an appointment with the family Dr on the 22nd and will show her what is going on to have it documented.. I honestly did not know if I would do skin removal cause I did not think it would be an issue but I am only 10 weeks Post op and still have 95lbs to lose so I am sure I am going to need it.. I will try the baby powder/ cornstarch blend..

    High Copper levels

    At 10 months post-op, I am losing fat stores that haven't been touched for YEARS. Since our fat holds toxins along with hormones, I can't help but wonder what's circulating in my blood. I have days where I feel tired or have headaches and know I am detoxing stuff my body has held onto forever. High Copper levels can become dangerous if not addressed. I would want to know how high and possibly get a second opinion if your doctor has no answers. But if he's testing every few months, I'm guessing levels are elevated but not in the danger zone. And it could be as simple as dietary. Good luck and hopefully you will get a good report soon!
  17. Menopause is the worst. It’s when I gained my weight. Went from 60 to 75kg very quickly during perimenopause. This was my usual fluctuation so I didn’t worry that much. Then entered full menopause & bam another 15kgs. I swear I gained it overnight. Several of my friends were the same - gained 10-15kgs very quickly with menopause. Couldn't really shift a kilo of it. Drop a couple & put them straight back on. I had hoped that being on HRT might balance out my hormones & help me lose some weight but nope. Struggled for a couple of years. Wasn’t happy. That’s when I decided to have my sleeve. Why don’t you try getting in contact with a bariatric surgeon to discuss your revision options like to a DS. Does your surgeon who did your RNY still practice? Have you considered whether the new GLP - 1 medications might be an option? Certainly worth a conversation with a surgeon. PS Congratulations on your long term success with your original surgery. Such an achievement.
  18. A little back ground with me. I had gastric sleeve 10 years ago because of my over weight issue I wasn’t getting my period and I wasn’t able to get pregnant. After the sleeve I loss total of 60 lbs, and got pregnant. The first pregnancy ended in miscarriage. I was very depressed about it. So my next 2 pregnancies I made sure I eat and gave enough nutrients for my babies. Also I didn’t keep up with the weight loss after I gave birth. Now after 10 years of the surgery I regained almost all my weight. My issue with pcos such as no having period came back… didn’t have period for like 2 years. Also have other issue like cholesterol, foot pain and Gerd. I thought about having revision for almost 3 years and I took action and had gastric bypass about a week ago. I am feeling some emotional up and down…been crying for the last few days. I feel such a failure of having to have to get the revision when the doctor gave me a chance. My surgery also didn’t go perfect. I was under the anesthesia for over 4-5 hours, (I didn’t get an exact time from the doctor when I came out from surgery. Family was inform about the equipment issue and didn’t see me untill the afternoon) because one of the equipment broke and the medical team had to wait for the replacement. I went into surgery 7am and woke up 4pm. when I woke up they had me take a walk and tried drinking sips of water. I vomited right back with 2 big chunks of clogged blood. And I tried drinking again but kept vomiting dark brown color liquid out, like old blood. ( sorry if it’s too much description) it was quite a lot of it even I didn’t drink so much water. The nurse informed the doctor and they said it’s normal and they did just literally cut my stomach. So some amount of blood is normal, but because I was under the anesthesia for so long maybe I am feeling sick from it. So they told me not to drink anything for the first day and started me on liquid the next day. The next day I was fine and went home. I felt horrible the first day and the pain in my stomach is there everywhere probably gas. The feeling after the revision surgery is completely different than how I felt the first time. I am aware it’s a bigger operation, but I just didn’t expect it to be like that. When I return home my stomach pain was still there, but eventually it did get better, but after a few days my whole stomach was so itchy, I thought the incisions were healing and that why I am feeling this way. The next day all my incisions were swollen red. BTW I had 6 big incision and 2 small needle punctures. When it got swollen, red and itchy it was at night and I waited another day and called my doctor asap in the morning. They told me I am having an allergic reaction to the dermabond. I am just feeling itchy, my incisions are raised and lumpy, it’s been over a week and I lose only 5 lbs. ( comparing to other people who loss 10-20lbs, i think to myself why am I not loss it?) and when I look at scars from the mirror I just broke down and cried. Thinking what did I do to myself…. I know I should stop being so bitter, and stop complaining. But I just feel like such a failure because I failed the first surgery, and I put myself in this journey again and things are not going so great… there are the positive side that I should look at, but I just can’t help it. *note- goal weight 150lbs pre op weight 273lbs started liquid diet 258lbs I had a stall during liquid diet for 1 week. Most people loss 10-15 lbs during liquid diet. I loss 8lbs. I followed everything my nutritionist told me and had 1 lean protein 3oz and 3 oz vegetable for lunch for 2 weeks. i w so hungry that the morning of surgery the nurse told me my blood pressure was actually very low. Surgery day 250lbs 1week post op 245lbs I don’t know what type of response I am waiting to get here. Maybe I am just venting and hoping to feel better.

    Laying Flat

    Congrats on the surgery! I was sleeping in the recliner for 3 weeks, and was 5 weeks out before I was comfortable lying completely flat. I'm at 10 weeks now, but I'm still using some pillows to prop myself up in the bed, I guess I got used to being propped up, but I am able to sleep flat without discomfort.
  20. Congrats on the surgery! Expect the first month to be a roller coaster of emotions, hunger, anger, and a day or two of regret. That's a rite of passage for us, where I think everyone second guesses their decision. By week 5, I finally started getting a better handle on things and I'm still very happy with my decision. 10 weeks out and I'm in better shape than I've been in decades! As others have said, you feel hungry because you are hungry. Your body is used to the previous amount of eating, and panics when it realizes you're not eating what you used to. Since you were a candidate for surgery, then your body has enough fat cell reserves to live off of that for a few weeks, which is why they have you focus only on water the first two weeks, while working in more protein to prevent you from burning muscle. However, our bodies don't understand that logic and kicks into survival mode. It only understands "food" or "no food". The first six months is critical to the process because that's when we have the best opportunity to retrain our mind and bodies to adapt. In a sense it is traumatic to our bodies, and it will fight until it understands that this new normal is safe, because that's its job, to keep you alive! Once you're back to solid foods, that's when recognizing head hunger becomes more important. Around that time (somewhere around weeks 4-6), you'll have worked up to eating enough calories to be sustaining metabolism, while still running enough of a deficit to burn fat. At that point, your body will try to tell you "hey, we're close to the old normal again, go ahead and eat more!". As my therapist reminds me, remember to listen specifically to your stomach, and not your mouth. Practice eating enough to where your stomach feels full, and not pressured from too much, but at the same time, study your habits to see if you're eating to stay busy, to deflect stress (I'm guilty of this), or just because your body thinks you should keep your stomach topped off. During that period, if you feel like your stomach is craving more, double check what nutrients you are getting. If you're deficient in vitamins or minerals, you will develop insatiable cravings, but your body can't tell you exactly what its missing. It just yells "I'm Hungry!". Check with your doctor on which supplements to add or remove, and also branch out with different styles of food as long as they fit your calories/macros. I felt like I was starving for most of last week, then I got some Korean food (sort of a bibimbap inspired kale and cabbage salad with beef) on Friday, and the cravings stopped. I'm still trying to figure out what itch that scratched, but obviously I was missing something in that! Also, be very careful with sugar, starches, breads, rice, etc. That can send you into a craving spiral that lasts 2-3 days. Some sugar alcohols like Sorbitol and Xylitol also trigger that for me, while Monk Fruit, Stevia, and Splenda don't. Everyone is different, but pay attention to those ingredients, because that can make the hunger feel worse!

    When the honeymoon is over

    I agree with the others stating the importance of having a therapist or coach to walk through this with you. Dieticians and nutritionists are very helpful with making a tailored diet plan for your needs, but don't always address the issues in our head, since that's not in their job description. See if you can get your insurance to cover sessions in therapy or counseling, if nothing else, just to have someone to vent to that is obligated to not judge you . If insurance pushes back, check with local therapists that offer "coaching" sessions at a lower rate. I had to do that until my insurance rolled over at the first of the year. I'm only 10 weeks post op, but I'm dealing with constant head hunger and cravings. I'm learning now that I used the act of eating, and not the feeling of fullness, as a way to deflect stress. At least now, if I overdo it, my stomach will rebel, which is its own problem, because I don't want to put stress on the staples. My therapist is helping me practice ways to listen to my stomach, and not listen to my mouth, if that makes any sense. When those two are fighting, the stress and feelings of being a failure appear to me. I have to remind myself that despite hitting a stall and dealing with the cravings, I'm already much healthier for having decided to have surgery!

A few months later at a Dr appointment for reflux the topic of having endoscopic gastroplasty was mentioned and eventually the surgery was scheduled for September 2022. I reported for the surgery and was going thru the pre-op process when our son called to let us know that he tested positive for COVID. The doctor advised us to reschedule. The next opening was a couple of months off, so it was rescheduled. I reported for the rescheduled surgery but after at check-in we were greeted with the news that the ventilation system for the operating room was down and we would have to reschedule again. This pushed us into 2023 and required navigating a new health insurance provider. Finally On 1/24/2023 I had my surgery and by 5/1/2023 my weight had dropped to 224 then “stabilizing” at around 235. 2023 had lots of ups and downs: - 4/25/2022 – weight 281.8 – bmi 38.2 - 1/23/2023 my last coke have gone over 1 year without a coke. Was typically drinking 1 – 2 20 oz bottles per day of regular coke. - 1/24/2023 - surgery. - 3/31/23 got laid off from work. - 4/6/2023 – robotic hernia surgery - 4/26/2023 – kidney stones - 5/1/2023 – weight 224 – bmi 30.3 - 7/11/2023 – procedure to evaluate hiatal herniaI - 1/18/2024 - COVID. - 1/25/2024 - big disagreement with boss and resigned. - 1/29/2024 - received ontingent job offer but significant pay cut. Job won't start for a month so I have time to think abiut job or seek other opportunities. - 1/30/2024 - weight 234.4 – bmi 31.7 Things to work on: - Eat better and exercise long term. I’ll do ok eating and exercising for a week or two and then I start stress eating, not feeling like walking. - Eating – there are times when I don’t feel hungry but want something to eat. - Regularity – I’ve always been irregular and at times have IBS with uncontrollable BM and then other times go days ( 3 – 4 days) between BM. - I have a goal of going on a couple of long bike rides 10 -15 miles but need to start getting in shape for them. - Decide to retire or find new job. - I rarely feel full. I think that I’m eating less but feel hungry or feel like eating even if I’m not hungry. Looking for advice on healthy things to eat between meals and limiting appetite here is a list of things that I like to eat . - - I like Kind Dark Chocolate Cherry Cashew bars (170 cal, 22 g carbs and 6 g protein) - addicting - Inspire Square Protein Wafers by Bariatric Eating (200 cal, 13 g Carbs 15 g protein) - addicting - BariatricPal Protein Shake or Pudding – Chocolate (6 gm carbs 15 gm protein) - Lunches - Dole Sunflower Salad Kit - 350 calaries (40 gm carbs and 9 gm protein)

    I know how easy it is to avoid the scale when you know it's going to be bad news. I do it too. But I'm going to urge you to make yourself face it. If you face it, you can beat it. I think the good news is, no matter how much you've eaten, there is no way you could regain 130+ pounds in a month. Maybe you've gained 5lbs. Maybe 10. But you know what? You've still lost over 100lbs. So, go get on the scale, acknowledge the number, and then look at yourself in the mirror, and tell yourself, "Self, I love you, and you're f*ing awesome. You're doing a great job, and I know you're going to keep doing it." No negative talk, no guilt. After that, look into getting a therapist to help you with the emotional crap that's making this hard for you, because food addiction is real, and you deserve for it to be easier. The biggest thing I've learned about shame and guilt over the years is the way I talk to myself sometimes, I would punch someone in the face if they ever said those things to someone I love. Try to treat yourself the way you would treat the person you love the most.
