    September surgery buddies!!

    Today is my 1 year anniversary and I am happy and sad at the same time. I got so very close to goal but I have spent the last month in a stall so I really don't have that awesome feeling that I hoped to have at the 1 year mark. I know this stall will pass too and I'm happy with how far I've come. I made it to normal bmi which I didn't think I would ever see again. I still want to lose at least 10 more lbs so that I'm safely in the normal range but 10 lbs seems doable after the last year. I still have trouble accepting that I've changed but little things make me so happy, like crossing my legs and feeling bones that were hidden. I'm happy my hair has stopped falling out and is starting to slowly grow back. Slower than before but I'll take slow as long as it grows lol. I started a journal at the beginning and set goals for myself and guessed at sizes my arms, thighs, waist etc should be. I had no idea what I should set those to so I looked them up in google for averages for women my height and I actually hit or beat all those measurements except my waist I set to 30 inches and I got to 33. I have a ton of extra skin there though and hope to have that removed so I still might hit that goal I took pictures along the way and wore the exact same outfit and its amazing so see how far I've come. I'll see if I can figure out how to add it here. Its not a great picture of me but it for sure shows the weight loss . I hope everyone else is doing well and I hope to see more updates from others.     Revision from VSG to Bypass

    I would want to look closely at this, verifying the bile reflux and determining if there is any acid reflux component to this before getting into long term treatment options as the treatment can differ widely depending upon that diagnosis. If it is bile, then I wouldn't expect Pepcid or other anti acid meds to do much as the are treating acid and not bile, a base. I'm not sure what meds they do use but likely different ones. Bile is used to neutralize the acid coming out of the stomach along with the digested food into the intestines. Is your surgeon in the loop on these findings (I assume so, but check if you haven't heard from them yet,) as that may change his prescriptions. If it is strictly a bile problem, then a bypass will probably correct it, but not guaranteed as it moves the stomach/pouch outlet downstream into the natural path of bile secretions; the key, according to one surgeon I discussed this with, is the length of the roux limb, as that is the one that connects the pouch with the mainstream intestine and how far any bile would have to travel to reflux into the stomach. This doc noted that at 80cm or greater (IIRC) he didn't run into any bile reflux problems. The basic RNY procedure has been around for some 140 years for gastric cancer and gastroparesis (it is usually termed just a partial gastrectomy, or likely some other fancy latin names as well,) and it that use, bile reflux is a not uncommon complication. My non-MD take on it is that in those cases, they tend to keep the limbs short to minimize malabsorption and weight loss (last thing a cancer patient usually needs is more weight loss!) So, the longer limb makes sense here. Discuss this and make sure that your surgeon is up on this aspect of it. The other option if it is basically a bile problem is the DS, duodenal switch, which is pretty much a guaranteed cure for any bile problems owing to the very long path between the bile ducts and the stomach, but relatively few bariatric surgeons offer it owing to its greater complexity. Note this only applies to the "traditional" or Hess DS and not the newer SIPS/SADI/"loop" or simplified DS, which like its mini-bypass cousin has bile reflux as one of its common complications. The DS will not help any acid reflux problem as it uses the existing sleeve (though may resleeve it if it was malformed causing GERD rather than just overproduction of acid,) while adding the intestinal rerouting for malabsorption. The DS is a better choice over the RNY revision if slow or inadequate weightloss is an issue, too, as it is a stronger metabolic tool. Good luck on this - bile is surely a much less common problem with the sleeve than acid reflux, so the industry isn't quite as settled on solutions for it.

    May 2023 surgeries

    First, great job on the weight-loss. I've noticed I can eat more and I'm starting to get my appetite back. It's so important to follow the plan... get in your protein (and some extra if needed). I aim for 3 oz per meal or about 80+ grams per day. Keep it lean- chicken, fish, shell fish, and plant protein. Make sure you're getting your veggies in. Cut up a veggie tray for the week and enjoy with a tablespoon of hummus or guac. Low fat cheese and yogurt. Keep measuring your food. It's so easy to overeat. Fluids are really important to stay on top of. Another thing that helps is to stay busy and keep your mind off food when you're bored. Exercise is also helpful to boost your mood and metabolism. Just be careful of the after workout munchies. It's totally a thing for me. A once on awhile treat is important too. My nutritionist said that if we always deprive ourselves, we'll regress back to old eating habits. So, for example, I love onion rings. And we we go to dinner that my husband orders them, I'll have one... or whatever I may be craving. Small amounts of something special once a week is totally within the plan. Everything they taught you before surgery is for life. Focus on not deviating from that too much.

    No forum for SADI patients?

    There's no BMI requirement for any specific wls procedure. The type of surgery is a medical decision made between the Dr and patient, not the insurance company. Most insurance won't cover wls unless you have a 40 bmi or other medical issues like pre diabetes etc. I had a 39 to 40 BMI at 287 lbs. I choose the Sadi DS over the traditional Hess DS, RNY, VSG or Lapband after disucssing my goals with my doctor. My criteria was to lose all the weight, keep it off and be able to eat normally eventually. All of them are true for me today (9 years later) but I also do my part with at least trying to mostly eat well, and getting daily exercise. It's important to note that wls creates an imbalance in your body and you have to make adjustments including the vitamins, eating, accepting loose skin, bloating and gas issues with bad food choices, etc. FYI - It's not an easy journey and takes time. If you make the commitment, it's worth it. Below is my journey and my almost 63 year old body 9 years post Sadi.
  6. I had lymphoedema because of my weight and I also had major problems when walking. I would be so out of breath and my heart would hammer in my chest. I was classed as disabled. I relied on a stick. Housework was difficult, It would take a week to clean my home, a room at a time. I ironed a shirt and sat down, panting. Now since surgery I am super active, can clean the house in an afternoon. I can walk miles without stopping to rest. My lymphoedema was in my arms and legs, I was very embarrassed about the size of them. Although I do not have the most beautiful ankles now, I would say they pass. My arms have a great deal of loose empty skin. At my age, I am wondering if I should have corrective surgery or not. I have been on 3 overseas holidays in very hot climates since surgery. My ankles and calves have never swelled and they always did before, especially on the plane journey. Its good not to wear the ugly socks. Good luck with your decision making
  7. Today was the 3 month anniversary of my bypass/hiatal hernia repair. I've done ok on the calorie restricted diet--still can't eat certain meats (I've stayed away from steak which I love, haven't eaten lamb and even a moist pork chop is a little tough for me- instead, I'm eating. lot of fish). My portions are small-- -protein say three tablespoons, veggies, two tablespoons, mashed or sweet potato- 1/2 a small sized one. Today I weighed myself- I went from over 271 to 221 which is kind of crazy! (I am around 6 feet, used to be taller but I'm a geezer so I'm shrinking). Hooked up at a new gym that I can walk to from my house, so I will go more frequently and won't need to spend money on ride shares (I prefer not driving in town). I was shocked at how weak I was--It is going to take some time to strengthen and I'm being very cautious about torquing my gut given the surgery (even though the surgeon cleared me for weight lifting). I'm doing my supplements. I guess this is a good news/bad news thing- not that I live by the scale but I was amazed at how much I've lost so quickly. And a little disappointed in how weak I am. My wife said, "dude, you went through major surgery, take your time." She's probably right. There is no question that the surgery + pretty strict adherence to the post-op diet does work. Now I just have to rebuild my strength over time.
  8. So, I had a screening phone call from the nurse and surgeon weeks ago and got the go-ahead for sleeve surgery on sept 12th. I had my in-person pre-op appointments yesterday. I am self-paid, mind you (18 000$), and already gave my 3000$ deposit. So, I meet with this nice dietician that starts asking questions about my past, about how I got to the weight I am now, etc. So, being an honest and open person (and also being already very nervous about the surgery, so, yes, I'm crying profusely during this appointment, which makes me look even worst), I explain that I have been suffering from Binge Eating disorder for the past 25 years and that, yes, it's still an active issue. But I also mention that I have started therapy with a therapist specialized in eating disorders. That I understand that this is a tool, that it involves a lot of work, etc. She gives the ok, they take my 15 000$ check, give me the powders for the liquid diet and send me on my way. 2 hours after, the clinic calls me to say that the dietician spoke with the surgeon and that BED being a new info for him, he is requesting a clearance from my psychologist to go ahead. I've seen this psy only 3 times so far.... So, basically, he is waiting for a note from a therapist that has seen me 3 times to decide if he's doing the surgery or cancelling, or postponing. I'm very upset. I am very anxious about the surgery (being put under, the breathing tube) and have been trying to get mentally ready for the surgery for the past 2 months and now it's all up in the air. After taking my deposit. After having phone calls with me and giving the go-ahead (and never asking about any disordered eating history!). So I will know next week what is decided. I am not mad at the dietician, she's doing her job. I am not mad at the surgeon, he's being careful . But I am mad at myself for opening up, being honest when I'm sure a lot of bariatric patients would have said nothing for the same exact reason (fear of being refused). I should have known better............ I should have just shut my mouth!

  11. I know it has been a long time since my last post. I'm doing well, just had my third fill and my doctor and I are thinking that is it for awhile. Just started exercing again (changed jobs in September and fell off the wagon), and the weight is coming down nicely now. Good luck to all my fellow banders.   17. Post Op Day 6 ---> I've lost so much weight so far in Week 1 that my wife asked me..."Are you sure they didn't put a tapeworm in you?"

  18. Supervised weight loss visit # 5 is scheduled Monday, with 1 visit left. This makes me 1 step closer to crossing over to the other side.

  19. I need a form that my weight loss provider can fill out for each visit that shows that we discussed weight, exercise, and behavioral modifications. If I can get this from some one on here it would be gtreatly appreciated.

  20. Hit two major milestones today: 1. I'm down 50 pounds since my surgery!! (And down 70 from my heaviest weight!!) 2. I can now comfortably bend straight down and comfortably lace up my hiking boots!
