  1. Thanks for the advice. I do have a question though, did you feel like you had to get a revision because, with 50 pounds to go, you had just stopped losing?

    Beginning BMI 35 or under

I had a BMI of 32 with no comorbidities. I had to self pay. First spot was in September and I had surgery in November. This was 2017. I advise addressing any food addiction or other mental health concerns adequately prior to surgery. Or you may end up like me 3 years later and qualifying for surgery yet again and pursuing revision. Dealt with my trauma with food and I had to do therapy before I felt ready to seek revision now.
  3. You have been through the wringer on this, so sorry you have had to go through it. I don't have any experience specific to this, but some maybe distantly related. I have seen that when they have an intransigent ulcer problem, classically it's around the anastomosis between the pouch and intestine, they usually just reverse the bypass, though that isn't really an option for you as that would put you back to your sleeve, which implicitly may not have been that well done in the first place. By reputation, I would try to seek a second opinion with Dr. Michel Gagner, who I believe has a presence in Toronto. He was one of the early adopters of the duodenal switch back in New York and has done some fairly complex revisions (such as RNY to DS) so is familiar with more complex problems. As to "done as much as we can" there is more that can be done. The most extreme thing would be a total gastrectomy, which isn't all that far from where you are now with an RNY, and is done in some cases of gastric cancer or gastroparesis. Basically they remove the remainder of the stomach and hook up the esophagus directly to the intestine, where your pouch is now. or close to it. Between surgery and adaptation, they form a new pouch in the intestine. Surgically, it is a bigger deal than it sounds as they don't like attaching the esophagus to anything other than itself (you would likely be on a feeding tube while it heals, but you've already been through that,) but it is a not uncommon configuration that people live well with (similar to your typical RNY lifestyle once everything heals.) Your average bariatric practice may or may not have the experience to do this, and you might have to go to a hospital that has a specialized gastric surgery department and/or cancer center. Simpler than the above, I have seen reference to a few people who were having significant RNY ulcer problems, again in the typical anastomosis region, where they went in and moved the anastomosis down the intestine a bit farther to a fresh spot, so that might be an option depending upon circumstances. Those are a couple of ideas that you can look into and question your docs about, and maybe seek out ones with a somewhat different specialty that may be able to help. Good luck in working through this,
  4. I’m looking to have a sleeve to bypass revision but I’m not sure what my insurance will cover. Is anyone on BCBSIL PPO 204 plan?
  5. Yes! I am having my sleeve to bypass revision surgery on the 4th of December. :)
  6. For a regain problem I would not generally go from a sleeve to a bypass, as metabolically the two are too similar and one usually winds up in the same place after a couple of years. Indeed, most revisions don't yield a lot of additional weight loss; those that do mostly seem to be those who really crack the whip and get their dietary and psych acts together for the second try. One will usually lose a few pounds, typically twenty, give or take - mostly what one would expect from having to go through all the diet restrictions associated with going through the surgery again. The simple minded way I like to think of the reasoning for the general poor results is that our stomachs started out being able to hold 32-64 oz before our original WLS, and now, a few years post op, our stomachs, even after some adaptation and stretching, hold 4-6 oz (maybe 8 in some cases), we have gotten used to living with that, so there isn't nearly as much of a difference going through the surgery again. The DS will usually provide better results, particularly in being more regain resistant. As with the others, it is hard to expect as good results as with virgin DS, but it should still work better than the others. When I discussed this matter with our surgeon when I had my VSG - the prospect of using a DS revision as a "plan B" backup to the VSG, he cautioned about the problem of losing that regain; he said that the revision works best if you catch it early, before substantial regain has occurred, as the biggest strength of the DS is its ability to resist regain. You are fortunate in starting with the sleeve, as that has maintained more options for you - revising an RNY to anything else that may work better (like the DS) is a very complex undertaking that few surgeons can do; as the sleeve is the basis of the DS, changing to the DS is more of a "completion" of the DS rather than an outright revision and is a pretty straightforward procedure for any DS qualified surgeon. You seem to be doing the right things, in working out the problems that led to your regain, as that will be a key to your future success. Good luck!

My original plan was to get a sleeve, but after my endoscopy, my doctor told me that my esophagus had some irritation from gastric reflux and that I have a hiatal hernia. He said that if I got the sleeve, there was a pretty good chance I'd be wanting a revision down the line, so he suggested the bypass and I agreed, in light of my GERD and hernia issues. Good luck to you too! I'll also be checking in often.
  8. Hey I revised to DS 4 weeks ago tomorrow. I was 6 years out from sleeve. Insurance paid as it was treated as a new surgery and so I just had to do a regular qualification. No diets or anything, just whatever the insurance required. I think it took about 6 or 7 weeks to get scheduled from my initial consult. Do you know what procedure you are leaning toward?

    Sleeve to DS

I am 4 weeks out from revision to DS. For me things in that department got more normal at the 2ish week mark after starting mushy food/ purée.

@GottaBeBlair and everyone Hello everyone, I am new to this forum. It seems that GottaBeBlair and I had the same surgery about the same time, so I wanted to see if you are having the same symptoms as I am having. I am 1 week post op sleeve revision to sadi/sips/mini ds. I have just started full liquids, protien shake, yogurt +water. I take multi vit from bariatric advantage 2x day. I have been experiencing strong intestinal gurgling with explosive mini diarrhea with little black stool that smells terrible. in addition to the very smelly flatuence the only way I have found to calm everything down is to take liquid pain medicine. have you been experiencing similar side effects? and did your Dr. tell you how long this will last?
  11. Hi all, so glad they added this forum! I was sleeved in 2017 and lost 90lbs. Unfortunately I experienced great loss and trauma in 2019 to recent and find myself meeting the criteria for weight loss surgery again. I’ve done a lot of work on myself since and learned how to cope without using food. I’m starting the approval process for a revision on 11/19/20. Anyone on here get a second surgery due to weight gain? I know they do revisions for GERD and other issues but I want to hear from anyone who had a revision for gain. Did you sleeve to bypass? Resleeve? Sleeve to DS? Bypass to...? Did insurance cover it? Make you do a diet program first? Did it take months? tell me all the things!

    HELP!

What I found was that my insurance didn’t care one way or the other about the previous surgery. The reason was that it was longer than a year since, so it was not considered to be a revision in their view. I suggest you call them yourself to be sure. Also highly suggest you go for bulky clothing and wear your heavy boots 😂
  13. I’m glad someone has addressed this issue as I am 3 weeks post op RNY revision and having the worst smelling poo and farts of my life. The smell drifts through out the whole house and is quite embarrassing! Poopori and sprays don’t cover it. This with a liquid diet and now moving into soft purée only foods. Could it be intolerance to the bariatric protein powder I am using? But then it wouldn’t make sense as I didn’t have this problem during pre-op diet and drinking this powder with no fat milk. Gosh, I pray it’s something that passes but now that I read comments I see others suffering 5 & 6 months post op. Going to try digestive enzymes if I can find chewables? I already take probiotics and eat yogurt. Anybody out there with any other ideas? Please help!

sure ! SIPS= Stomach Intestinal Pylorus Sparing It's a modification and similar to the duodenal switch. I will be a revision surgery from a gastric plication
  15. The tummy tuck part was probably the easiest part, after I got my drains out at 3 1/2 weeks. I didn't need muscle tightening otherwise my dr would have done it. The back part of the "360" was hard to get used to. It took at least a month to be able to bend over and not feel like my butt incision was going to split open. The arms were a hard recovery and the most painful. My incisions opened on both arms. The right one has healed, the left has a little more to go. It will need a revision with my next surgery. One thing I didn't realize...I'll probably never have to shave my armpits again! The skin that is there now doesn't have hair! I measured 73 inches of incision between my 360 lower body lift and extended brachioplasty! I'm 9 weeks post op and my body is still spitting out some stitches that didn't absorb. I'm ready for round 2...breast lift and back lift. Hopefully that happens early next year. It depends on when I can get time off from work and the sale of a house to have the money to pay for it!

  18. I just had a tummy tuck, butt lift and brachioplasty Aug 25. My next step is breast lift and back lift early next year. It probably won't be Jan but hopefully soon. I still have an open wound on one arm and I need that to heal so I can get that scar revised. The arms were the hardest recovery I've had so far. I'm so happy I chose to have this surgery. I can say that now that I'm 9 weeks post op!

    HELP!

Welcome to the forums. Don't think of whatever you do until you get your revision as a diet. You and I know that diets don't work. Think of it as your new eating plan that you will do for the rest of your life. Your band restricted portion size. Presumably you also had an eating plan to go with the smaller portion size. Return to the plan and return to portion size as best you can. Your plan was supposed to be for the rest of your life so get back on board. Your insurance is as likely to pay for your revision as it would if you never had surgery in the first place. Do the best you can. If you give up on your plan until you have surgery, well, then you very well may gain your weight back. Good luck, Tek

    HELP!

I had my Lap Band removed three years ago. Now, my weight is starting to come back. I couldn't have a revision done at the time of the removal because I was not overweight. I weighed 143 lbs. Now, I'm up to 162. I have an appointment with my surgeon about, hopefully, a revision surgery. My BMI is now 33. Has anyone had a revision years later? Did you insurance pay for it. I'm scared. I can't go back to 230 lbs. Thank you.

    Anyone for October 2020?

Good Morning, I had my revision to bypass surgery yesterday morning. When I woke up I had no surgery pain. I was thinking it wasn't done until the GAS damn near killed me and still is. That gas pain ain't no joke, for those of you who haven't gotten surgery yet, make sure you get enough gas relief and walk. I walked around the unit 4 laps yesterday, the nurse told me not to overdo it.... over do it?! I'm walking this damn gas out and preferably out my butt 😁 which by the way isn't happening and the gas is stubborn. I'm belching tho but it's not moving. I did finally pass one little gas out this morning. I saw the doc this morning and he said my stomach was in my esophagus. Now when I get up gravity is taken place with my right incisions I'm using a towel to walk when I get home I'll use my binder
  22. Hi all. Long time lurker but first time posting 😆 I was ‘sleeved’ a few years back, was successful in losing some weight, though due to different circumstances regained a large portion of that weight and also developed qUite severe GERD. My surgeon has recommended the bypass for me which is now all booked in for December 2020. Feeling excited, nervous, fear of failure again etc. hoping for a smooth journey ahead and will definitely looking forward to being a part of this community. Thanks!

    Sleeve to DS

Hi Kozyjozie! Of course you’d have to consult with a doc to see if you are a good candidate for revision but I was the same as you. I regained almost all of my weight after having a child. I chose a revision to DS versus RNY since VSG is the first step of a DS surgery. So when I was revised, they didn’t touch my tummy, just completed the intestinal procedure. Please note that far less data is available on revisions, so the outcome may not be as significant as you’d like it to be. I am losing less than I did with my original procedure but I am losing much more than I would have on my own. I suggest before you even jump down the rabbit hole to really figure out why you regained. And come to terms with the excuses, so you won’t repeat. I got therapy as well. Hope that helps!

    Sleeve to DS

Hello everyone. I had the sleeve done august 17 2015. I lost 70 pounds in the first year. Little by little my weight started to come back. Now I’m the same weight I was before doing the surgery. Would this da revision be an option for me? I really would appreciate any advice you could give. I need to get healthy again.
