    Sleeve vs bypass

    So I had the sleeve first in 2013 and it worked well for me. I lost over 100 pounds but about 4-5 years post op I gained most of my weight back and started having significant GERD which I never had before. I recently had a revision to Gastric Bypass and my reflux is gone and I am losing weight again. From personal experience if you have any issues with reflux I would go with the By pass

    Hi All, I am new to this thread and was glad to read everyone’s stories and victories. Congrats All. As for me, I got my bypass done on 12/23/20. It was a revision from sleeve which was done in 2017. Had to get bypass surgery for gerd as well as weight loss. In past 8 weeks now, my gerd is improved, but I have lost only 14lbs of weight. I wanted to ask if this is normal. Is there anything that I should be doing to increase my weight loss. Please advice. I am really disheartened at this stage.

    Is duodenal switch too drastic?

    I just had the SADI-s (single anastomosis duodeno-ileal bypass with sleeve gastrectomy) done a week ago. This is the revised version of the traditional DS. I did so much research and chose this over the RNY (my surgeon didn't even suggest this for me) and the VSG. I am starting at a very high weight (420 at my pre-op appointmnent with the surgeon and 412 at day of surgery which was 2 weeks later). In addition to my high weight and BMI (I hate using BMI because it's not a good indicator of healthy weight AT ALL) I have PCOS, sleep apnea, and hypertension. Knowing that this procedure will basically reverse all of my comorbidities AND help me lose a higher percentage of excess weight AND keep it off is why I chose the SADI-s. I could have just chosen the VSG first and see how I did with that surgery, but I don't see the point in that. There's a higher chance I could just regain the weight I lost and need the revision surgery anyways. I'd much rather just get the surgery that I know will provide me better results the first go round. I'm hoping to see some serious transformation in the next 18 months. I'm really looking forward to getting more mobility back and having more energy. It's the little things I'm excited for....being able to take a bath because I fit comfortably in the tub and can get out easily...taking my dogs for longer walks...being able to bend down and tie my shoes easily...going to a concert and being able to stand and dance for hours without pain or needing to sit down...not having to deal with the anxiety of flying and needing an extra seat....not using a seatbelt extender...sitting in booths at restaurants...having more clothing options... Long story short...do the research, listen to your surgeon's advice, and make the best decision for yourself when considering the long term.

    Hypothyroidism and gastric bypass surgery

    Well, mine’s been complicated. My revision (8/2020) was mostly for the hiatal hernia of almost my entire sleeve that gave me constant hiccups. Then I had an internal hernia repair 11/2020 and then a small bowel resection 12/26/2020. But since my revision I’ve gone from 212 to my goal of 155. Mostly because hernia repair and bowel resection recovery follow the same diet as post op WLS- liquids to soft then solids as tolerated. I just had an EGD today to make sure everything looked good and I’m cleared for whatever I can tolerate as long as I chew well.
  7. So it been almost 9 months since my revision to RNY from VSG. I had erosive esophagitis D and as of this past Dec it has gone down to an A. I still take Dexilant 60 mg and as I sit here typing I am on fire !!! Just wondering if anyone else is still going through this after being revised. I take the strongest PPI available that I am aware of but I shouldnt have to be on this PPI after the revision but here I am !

    Hypothyroidism and gastric bypass surgery

    It’s not just calcium and iron you need to watch for with the 4hr window. You also need to watch out for acid reducing medicine as well or at least that’s what my prescription bottle of levo says. I was sleeved 12/17/19 and then revised 8/11/20 for gerd. In the time between I did stall for almost 6 weeks which was frustrating but I eventually started losing again. Also, the closer I got to my goal weight the slower I lost.
  10. I just had be revision this morning ing from Sleeve to RYNbypass. He also fixed a hiatal hernia. I am walking amd sitting in chair, no drain but omg the headache I would when I got out of Anesthesia wasddeadle, started in neck and took over full head. They have only given me Tylenol and to radial for ( like super inprofen) pain since I woke up from surgery. It has been 4.5 hours now amd I am worried that pain will hit me and I will not be prepared, they said they will give me morphine pill if pain gets bad. What should I do?? Headache is killing me. They did give me zophrane for naseau so I am goood there. They said I will be discharge at 11am unless something occurs. Oh and they just took my blood sugars and it is 179 so they have to give me insulin but I am not a diabetic???? So confused???? HW 324 Lowest 159 Current 229 Goal 154

    Regain 2 years out

    I am almost 7 years out. I lost 148lbs now I’ve gained about 40 back. So I’m desperately trying not to sink.... I have also been thinking about a revision however I would like to try fitness and back to calorie counting.

  14. Please bear with me while I ramble and get something off my chest. I started this journey with a gastric sleeve surgery July 2019 things went bad quickly I was vommitting uncontrollably and very rapid weight loss I started losing my mind and had little to no muscle control by September. After a severe fall I was put into a rehab hospital and they decided to change the sleeve to a bypass. The revision was done 2/2020. After the revision I was discharged to my home. I’ve started regaining memories and muscle control. By June 2020 I was able to walk independently (with a walker for balance) by September I was able to forgo the walker unless it was long distances. Even with this crazy pandemic I’ve been pretty upbeat and happy I am literally half the size I was when I started this weight loss journey. Yesterday I went to my obgyn appointment they were complimentary and saying how I don’t even look like the same person. Then I got my after office report. I always read over them in case there is something I forgot or missed. The first line goes this is an obese female.... damn I’ve seen that word with ever dr appointment for more than 30 years but it has never bothered me like it is this time. I mean I’m literally half the person I was before and almost died to get to where I am now what else do I have to do to get rid of that word?

It is always based on YOUR anatomical structure, now, after your first surgery, but I actually HAVE had a TRUE RNY bypass, in 2003, that was "reversed/revised" 20 months later, due to a non healing ulcer, and major malnutrition. I ended gaining about 100 lbs back, and LOTS of gastric problems. I finally found a surgeon (actually in same practice as my original surgeon), that agreed to take a chance on me. He "bypassed my bypass" 8/17/2020. Said it was first one like that he had ever done. That was 50 lbs ago. So, YES, IT CAN BE DONE- IN SOME CASES. SEEK OUT PROFESSIONAL HELP. GOOD LUCK.

    Sleeve Narrowing

    Sadly, it happens - more frequently a few years ago (6-10) when most bariatric surgeons were still learning how to do sleeves than more recently, but I guess that there are always some who are still learning! The sleeve tends to want to bend in the middle, or form an hourglass shape, if it isn't done quite right, it may not yield a total blockage type of stricture, but it can leave the narrowing that can impede the flow and/or exacerbate reflux problems. While most surgeons in the US are now far enough up the learning curve to usually avoid this problem, knowing how to fix it can be beyond their experience, hence many prefer to go with a bypass instead. It may be possible to correct your sleeve, but you may need to find a surgeon who is very well experienced with the care and feeding of the sleeve construction. My suggestion, if you want to go for a second opinion (which I think anyone should do when considering a revision,) is to book a virtual consult with Dr. Ara Keshishian, who happens to be on the wrong coast for you, out in Pasadena, CA, but he has been doing virtual initial consults for years before Covid as he has patients all over the country. This will at least give you a reading as to whether this is a viable option in your case, or give you confidence that the RNY approach is the best. If a resleeve is an option, then you can decide whether to travel across the country, or seek out another surgeon closer to you who can do it. I would suggest looking for one who routinely does the duodenal switch (DS) procedure, as they tend to have the longest and most extensive experience with sleeves. I believe that there are at least a couple in FL, and several further north along the East Coast. If you do choose to proceed with the RNY route, do discuss things carefully with your surgeon, as there are tradeoffs in how he proceeds. Limb lengths, as suggested above, are a compromise as if they are too short to minimize malabsorption, you can be more prone to bile reflux. There are several Facebook groups that cater to total and partial gastrectomy patients (primarily for cancer or gastroparesis) and bile reflux is one of their common complaints. When I was considering such a thing a few years ago, the surgeon I was dealing with said that as long as he kept the limb over a certain dimension (80cm, IIRC) then they saw no problems with it. Hopefully, the surgeon that you are dealing with has enough experience on both the WLS and non-WLS side of it to know those tradeoffs. Bariatric programs that are associated with major cancer center hospitals readily "swing both ways" on that, but one that only specializes in bariatrics may not. I wouldn't worry too much about the malnutrition issue, as the RNY is very well understood; it is somewhat fussier than your sleeve in supplement needs but things are pretty straightforward on it if you keep up with labs and change things up as those dictate; it can be problematic for those who get overly casual about such things and let it slide - then you can get into trouble. If you fall into that camp, then I would try to do everything to preserve your sleeve and its greater flexibility; otherwise, the RNY is a good alternative. My personal preference, as I was faced with some similar decisions, is/was to stick with the sleeve if it is viable, as the RNY (or something different) is always an option for the future, but once one has an RNY, changing things gets more difficult, so options are fewer. Also on the option front, with the bypass, there remains a "blind" remnant stomach along with the duodenum and upper intestine which are unavailable for endoscopic evaluation or treatment (things much be done surgically.) As there are an increasing number of procedures that can be done endoscopically these days, and into the future, and I have already had one lifesaving endoscopy this is an option that I am keen to preserve, if at all possible. Short term, you may lose too much as you go through the high level of restriction that comes in those first few months after surgery. In that case, there are ways to "eat around" your pouch by basically doing all of the "wrong" things for your WLS - drinking calories, eating slider foods, higher calorie options particularly fats as tolerated. The tricky thing is to avoid making too much of a habit of it as the restriction does diminish over time and you can naturally eat more of conventional foods to maintain your nutrition

    Sleeve Narrowing

    I was converted to RNY from VSG for structural (and other) reasons too. My surgeon just said we were doing bypass so I’m not sure if there any specific differences between that and what you will have done. It’s completely possible to maintain (and even gain) weight after your revision. The malnutrition aspect comes with how long of a bypassed Roux (or alimentary) limb will be. Your surgeon may choose to make that length shorter to prevent excess weight loss, but that’s a plan you will come up with together.     Sleeve to Bypass & hungry?

    Well it did. Took a couple months for the hunger to go away. I was 216 on revision day. And today I’m 186. I do not regret it. It boy it’s a lot harder than the sleeve. You really gotta chew your food or it will get stuck and if I eat greasy food m tummy hurts and I dump. It helps tho cuz then I make sure not to make bad choices. Lol I’m very happy. I burning and on my reflux med anymore. Soooo nice shy a relief! I do not regret this! :)

    Sleeve to Bypass & hungry?

    I'm seeing my surgeon next week and expecting him to recommend revision to bypass for my weight gain and Gerd. I'm concerned about the hunger some of you are describing, but I'm really concerned about not feeling the restriction. Does that feeling go away after the bypass revision?
