    I totally get you! I am 4 months out from a revision from sleeve to roux-Y-gastric bypass to address GERD /Heartburn/Reflux and ever since I have had the surgery I have had no restrictions once so ever. I can eat foods that I was not able to with the sleeve, I have no dumping issues, no problems once so ever with foods and I don't get the sensations of fullness that I got with the sleeve and it scares the crud out of me! I lost most of my weight with the sleeve and then more when I did the pre-op diet and then lost another 10 after surgery. I am currently stalled for the past 3 months but am only 9 lbs from goal weight. I fluctuate 9 lbs up and down depending on whether I have moved bowels or not, LOL - you guys get that. I track my foods and keep thinking I need to go back to the beginning and back to only eating around 800 or 600 calories if I want to lose that additional 9 lbs but I am having a tough time doing that. Probably if I go back to basics and do the pre op for two more weeks, I could achieve this but not sure I could maintain. It is just so frustrating to not have the restrictions. I am pleased that I am 16 lbs smaller on average then I was on the sleeve but I worry for how long will this last? And when I do eat instead of getting a warning that I am about full, I get a very sharp painful feel when I eat too much and I never know when that will be. But I definitely can eat more not that I am trying to overeat. I do try and control my amounts but at times find that I have eaten more than I planned and then I get mad at myself. I get jealous of the ones that talk about dumping and how their bodies reject things and wish mine did this as it seems like a great control and that their tool works better than mine? It amazes me when they talk about only being able to eat a couple of bites and being full as this seems so foreign to my body? So you are definitely not alone in this!

    Revision Pain

    I haven't had a revision or rny but I assume if the pain has continued in the last six hours you have contacted your surgeon's or doctor's office to make sure everything is okay/
  4. Hi all, So I had the revision surgery to RNY on the 24th and I initially had no problem drinking. As of yesterday I am having major issues. I started protein shakes Friday and I'm pretty sure I dumped on the evening shake. I thought maybe I took my vitamin and shake too close together. But now I am also having severe pain when I drink water. It feels as though my stomach is being stabbed and twisted. I've tried both warm and cold and get the same results. Is this normal?
  5. I am looking into the sleeve revision surgery, yes weight gain in the last year. All time low 148 pounds. Im at 184 now.Im kicking my own ass,failure again. Who has had the "sloppy sleeve" revised or from sleeve to gastric bypass? I do medical abroad in Mexico . Lapband in 2008. VSG in 2016 .   6. in 2011 I was sleeved with no indication of having a hernia. I believe if the doctor saw that, he would have indicated to me that he would go ahead and remove it, but because it wasnt seen then, the sleeve was a stand alone surgery. Now had he done the gastric sleeve with the rny, I think my weight loss success would have been for a long time before gaining little to none weight gain. Nevertheless, the infamous GERD came to crash the party in 2019 and I was scheduled a bariatric doctor for a revision in September 2020. At that point I started the process which leads up to surgery and I was approved by my insurance company, thus my surgery date was 02/23/21. The doctor chose gastric bypass which he indicated that he tightened up my sleeve and rerouted my intestine where whenever I eat, it would bypass my stomach and go straight to my intestine, hence longevity of weight loss. He also mentioned to me that I had an hiatal hernia, which he repaired during the surgery. Because this is a revision, my weight loss will go moderately slow, however I choose to think that is on a "case by case" basis. Ive seen people with this revision go on to lose around 50-60 lbs in 4-5 months. I may very well fall in with these people, due to my rapid weight loss 9 years ago. my hw in 2011 was 303, sw was 289, fw 179, gw 160. I never reached my goal weight the first time, but I lost a total of 110lbs in 1 year, and then in 2013 I couldnt get the skin to tighten up, so I qualified for a panni, aka tummy tuck. i have big turkey wings and that is considered cosmetic, had Ive gotten that done, I probably would have gotten to my goal weight with ease because I went down to 173 after that surgery then the plateau started, but I maintained that weight for 7 years, then in 2019 I went through a depression state and going back to my bad habits, and smoking ciggs, then to make things worse, covid came to join the world so my weight skyrocketed to a shockingly total weight gain in 2 years of 76 lbs of it back. Thats 40 lbs away from going back to my surgery weight! That was a scary thought, but I think if I didnt have the gerd, my hiatal hernia wouldnt have been noticed. I know how to keep my weight down, because I had highs and lows throughout maintaining my weight without having a head game behind the fact that I was no longer 173. I was dating this guy from atlanta for 2 years and all I did was travel, splurge and dine out 3-4 days a week breakfast lunch and dinner. I didnt work and he took care of alll my bills and my childrens tuition for college and high school. This caused me to be settled and gained happy weight then my happy weight went flat as I got tired of being smothered and controlled by him so I left him right after valentines day. I then joined the gym at the end of February 2018 because I had got to 220 in my weight! I was making 50 that year so I wanted to at least get back into my size 9/10 before my birthday. Well I did. I lost 22 lbs in 3 months and was toned and force to be reckoned with. So I know that this is a long response, but I just wanted to share with you my journey as a 2 time bariatric patient. Im 5 days out from my revision, and I have lost a total 17 lbs which is to include losing 10lbs in 3 days from preop preparation. If I stick to the diet the way my nut and my dr has set forth for me, I could very well get past the goal weight they have for me, which is 180, and thats due to my frame, and my height as an African American female. At the rate that Im going, I will have lost a whopping 80 lbs in 2 months! But because the weight loss will be slow, and if I had to guess, I would say to get to MY gw of 160 would probably take me about 7-8 months to achieve that goal. Who knows with a sustained diet and exercise, I could very well get to my goal sooner. I hope this document of a response helps. lol, but best wishes to whatever surgery you decide to choose and God speed on your recovery.

  8. How long has the process been for you? I have BlueCrossMN and they have no protocol for revision surgery, I was hoping to have done within 3 months. I also looked up Alina clinic and there is one less than two miles away from me. Lot better than driving to Rochester. thank you

    Removing the pouch?

    The fundoplication is used in the case of a hiatal hernia (where part of the stomach is pulled up through the opening in the abdominal wall, through which the esophagus passes) which can be the cause of acid reflux, and you are right in that in may or may not be appropriate depending upon whether there is enough fundus left to plicate. If your problem is more with the LES than with a hernia, there are other things that are sometimes done for that - there are implantable devices that replicate the function of the LES (LINX is one of them, there are probably others as well) and likely some purely surgical approaches to it (I am far from any expert on this!). Bile reflux is rarely a problem these days with the RNY, mostly because the surgeons have learned how to set up the limb lengths to avoid it in the most part; it is not uncommon in non-WLS versions of the procedure, such as the total or partial gastrectomies for cancer and the like, where they shorten the limbs to minimize malabsorption and weight loss in patients who can't afford to lose more weight. What can be done if bile is your problem is to move the pouch farther downstream on the intestine to move it farther away from the bile ducts. Revision to the Duodenal Switch WLS is the best response to it as it is virtually impossible for the bile to work its way back to the stomach and esophagus, but thats a very complex procedure that few surgeons in the world can do. The distal (or long limb) RNY is next best, but has similar nutritional consequences as the DS and by your figures, it doesn't look like you need to lose more weight. But there can be in between compromises on limb lengths that can do the job, if that indeed is your problem.

    Removing the pouch?

    It can be done - it is called a total gastrectomy and is usually done for cancer, gastroparesis or other severe gastric problems - but it is something of a big deal to do. The long term functional difference to your RNY is small, but healing time is the big issue - the esophagus (eating tube) is only a two layer structure compared to your stomach pouch which is four or five layers (depending upon how you count things) so the direct attachment to the intestine is trickier than the stomach to intestine. This was an option I looked into some years ago on a cancer issue, and the partial gastrectomy (basically an RNY) had a normal RNY post op healing and eating progression. The total gastrectomy would have required several months on a feeding tube to let things heal before any food could be introduced. So, this is not something to go into lightly. (And, no, I never had either procedure done - second and third opinions indicated that they weren't necessary, at least for the time being.) Something else to look into - have they determined if the GERD is acid, or bile? That's a big difference in treatment, as if it is bile reflux, the basic RNY structure is already somewhat predisposed to that, and taking out the pouch will no help, and likely make it worse. A possible clue here is that the anti-acid meds aren't working, and they wouldn't if it was a bile problem. It sounds like your problem, or a good part of it, is a weak Lower Esophageal Sphincter (LES) which is the muscular valve between the stomach and the esophagus. That can be weakened by your obesity history (though that was a long time ago), by over eating - over stuffing your pouch can put pressure on it and weaken it, or by some other disease mechanism. There are procedures available specific to this problem that may solve things without having to go through the gastrectomy. Do some research on surgical treatment for GERD. You need to research and find a surgeon who is very experienced with bariatric problems and revisions to figure out what exactly is going on, and then what the proper solution would be. The RNY (or any procedure, for that matter,) does occasionally cause significant problems so you need someone experienced with that. Good luck in working this out! Note - there are some Facebook groups that cater specifically to people with total or partial gastrectomies, so that would be a good place to look to get a better idea of what living with that procedure is like.

    Agreed - best decision I made! I was sitting on the fence because of the lap band but so happy I did the revision. For the record, you helped me in that decision. So, thank you! I made it to 147 this morning. I am going to search for a personal trainer to help with a body transformation now that I am no longer in the "overweight" area. I will be spending some time this weekend going through clothes - its time to clean out closets;) Have a great weekend!
  12. A face lift was going to by my next and last plastics procedure, but based on my less-than-optimal scar healing tendencies, both me and my surgeon decided its probably not best for me. While the incisions for the face lift are supposed to be hidden in your hairline and around your ears, it would likely not be so well hidden on me. And we are talking my face, so I don't want to risk it. Sooo....I decided I'm going the fillers route. I've been meaning to go for a proper consult for months, and reading this post spurned me into action, LOL. I've spoken to her briefly twice before when I was in for followups with my PS surgeon for scar revision. I've JUST got off the phone and booked a proper meeting for next week! My areas of concern are my sunken under eyes, the nasolabial folds (i.e., marionette lines), and my neck. Also I am considering addressing my jaw as the last time I spoke to her she said an inject-able in my jaw will relax my muscles there (as I am a night time teeth grinder) and will result in less headaches and a less wider face. Interesting, huh? In terms of cost, injectables are probably a little more expensive in the long run vs plastics, but its a scar thing for me. Further, most people who get face lifts, still get fillers anyway as face lifts alone don't address volume, only skin tightness. I have seen endless amounts of before and after pictures of people with face/neck lifts with FABULOUS results. I think your best place to start would be to book a consult (lots of places are doing virtual consults), with a surgeon to get basic info. It wouldn't have to be the surgeon you ultimately decide on, but at least you can get some info and a baseline to compare with other surgeons. Keep us updated if you can! ❤️
  13. I’m thinking the GERD is from my regain, is there a bunch of hoops to jump through? My sleeve was done at north memorial hospital and when I went there about revision, the doctor stated that revision wasn’t an option and I needed to see dietitian and exercise more, I explained I was doing all that but I had some med changes and it caused a little weight gain. They said it would be at least a year of supervised diet and excersize then we could talk about a revision, uggg. what clinic did you go to and what hospital is surgery going to be done? mom sure it would be better than traveling to Rochester for all appointments. thank you

    Hi !! So sorry you are going through this and I know EXACTLY how you feel !!! I had so much anxiety and fear for months after this revision wondering how I was going to not gain without my restriction. I was pretty flipped out but here I am 9 months later and the smallest I have ever been. Its crazy not to feel anything and very stressful but there is this thing called dumping that will keep you in line and believe me you dont want to mess with it ! Also the malabsorption of calories is a real thing ! I lost 12lbs the first month and I was at my original goal weight of 180 day of revision. I only lost 14lbs after my original surgery and I weighed 296 that day !! I could and can eat more with this surgery then my VSG but if I over eat I get violently ill ! It really sucked after having the revision but I must say it is better then the sleeve and if someone told me 9 months ago or 6 months ago I would be saying this I would have said NEVER and NO WAY !! Here I sit with no problem staying under 160 where I struggled just to stay at 170s with VSG. Love to hear what you think later down the road. Good luck

    Thanks for sharing all your information. I am scheduled for revision March 16. My original bypass was 11 years ago and will have my pouch made a smidge smaller and further distallize the common channel a little bit for a little malabsorption. I'm so ready! My only concern is return to work after the revision surgery. I was off 3 weeks with the original. I teach high school and came back for finals week, so had relatively easy days and then another two weeks off for the holidays immediately following that week of work. This time I'm only planning for 2 weeks - pandemic and all has made this school year a whole disaster and don't want to be gone any longer than I must. How long were you home for recovery? I didn't read all 17 pages of this thread yet - so maybe you mentioned that... but any advice you can offer is most appreciated!
  16. I'm scheduled for a surgical revision on March 16. My docs (weight loss clinic) do not recommend the endoscopic procedure. As they put it - its like a bandaid on a gaping wound - it helps a little but not worth the effort to open the package and put it on. Good luck!

    You bet! Where are you from? Locally we have a Weight Loss and Wellness Center affiliated with one of our hospitals. They are phenomenal and do ALL of the dealing with insurance and appeals and all of that. I'm happy to share their information if you are near enough to Springfield Illinois and would like their help. My revision was initially denied as well, and within two weeks they had arranged a phone call with the insurance company's doctor and my PA. I didn't have to do a thing except worry!

    My recovery is going pretty good. I am still on liquid diet. I did however come back to work right away. I had my revision last Thursday, got released on Friday. Came back to work on Monday ! I am not so sour. I was Saturday and Sunday, but since i work from home its not all that bad. I am just really praying and hoping this REVISION puts me back on track to loose weight again. Crossing my fingers !!

    The switch will not help you with the GERD unfortunately. I am doing it because of regain. Allina’s program has been excellent. They commonly do revisions. It may be something to look into!

    Hi all! I am scheduled for revision of my bypass on March 16! Original stats : 260lbs pre-op RNY Bypass : November 2009 Lowest : 135lbs Plastics - Abdominoplasty; Panniculectomy; Thigh Lift : June 2011 Divorce - Remarried - Regained Sept 2020 : Re-entered Bariatric Program at 279lbs Current : 245lbs I will have my pouch resized and reconnected distally for a little bit of malabsorption. Hoping for ~ 70lb weightloss and then to maintain.
  24. I've been married for almost 42 years and my husband was not in favor of either of my surgeries. I had the sleeve in 2014 for obesity (I had a 36 BMI like your wife) and just had a revision to bypass in October 2020. But in both cases, his concern was directly related to his fear that I was doing something drastic and irreversible for weight loss (sleeve) and GERD (bypass). But I can also tell you that he is very happy with the results. Tells the neighbors how great I look etc. He is very supportive of my eating habits and just thankful that I didn't quit cooking meals just because I don't eat them!! The most important thing you can do for your wife is share your concerns but reinforce that it's her decision and you are there for her both before and after surgery.

    I see the surgery more as a strong tail wind that helped me to get to where wanted to go - but I had to do a lot of the work. I don't really eat until I feel restriction, because at that point, I've overeaten and there's a risk of it coming back up. "Full" cues are often different after RNY than they were before, and it takes awhile to figure out what they are. I usually just eat as much as I know I'm supposed to eat (which I pretty much know having been at it for almost six years), but when I do overdo it and get to the point where I'm feeling some pressure in my chest - or a bit uncomfortable - I know I need to stop RIGHT THEN or I'm going to be sorry. Granted, I had a virgin surgery - not a revision - so it may be different for revisionists.
