Ketosis - How safe is it?

I did a keto-based diet during my 7 months pre-op, and ended up losing 110 pounds before surgery. My diet was different from the typical "keto" diet since I'm a vegetarian, but I did try to stay below 25 carbs per day. I didn't count calories or fat at all. Just Protein (at least 100g) and carbs (no more than 25). I'm pretty sure I stayed in ketosis most of that time, though I didn't have any way to measure it. I'm almost 6 months out from surgery now and still eat very low carb. I am so glad I started before surgery because it got all the cravings out of the way when cheating wouldn't endanger my healing process. I'm sure it made the first weeks after surgery easier since I didn't have to deal with the cravings. As for safety, my surgeon and nutritionist both recommend a keto diet post-surgery. He says ketosis really helps the weight loss during those first months where most of the weight comes off. I'm just now starting to incorporate some whole vegetables into my diet. My program guidelines say protein only the first 5-6 months, and no simple carbohydrates (bread, Pasta, rice) for a minimum of 1 year (though I probably won't ever add them back in, since for me they trigger the cravings to come back). Just had my bloodwork done for my upcoming 6-month follow-up and all of my numbers are good. -

I don't have any kind of official hormone dx except "jacked". Several years ago started having cripplingly heavy periods. Found awesome hormone doctor, discovered I had no testosterone, very low progesterone, and was swimming in estrogen. Started natural hormones and have been mostly functional since. I get tested every three months and we make micro-adjustments. I went off all hormones a couple of months ago and it was *horrible*. So, back on the hormones and it calmed down again. And then I started the pre-op diet. My surgeon just wanted low carb, so I decided to go straight into ketosis - I even started a week early to try and get the hang of it. Diet went well, but period hit like a ton of bricks. I was so anemic from blood loss I could barely walk up the stairs. I was tanking Iron like a crazy person trying to be ready for surgery. The Keto bloggers claim this is your body basically doing a giant fat related estrogen dump. (Some also call it "Shark Week" and claim it's not unusual when you first go into low carb ketosis.) Their theory is that fat is full of estrogen; you're now burning tons of fat; hence your body is now swimming in excess estrogen and messing everything up. Is there any truth to this? I have no idea. In my case, my estrogen levels had been falling off (I'm 45) and spiked right back up again last month. So my hormone doc is going to keep testing and adjusting to try and keep things reasonable. I was only sleeved a few days ago, so we'll see how it goes. Consider going to see your hormone doctor, if you have one, or discuss it with your surgical team. Everyone says your hormones will be crazy after surgery, but it's good to have a plan in case things don't settle down on their own. My niece was like you - heavy periods eventually turning into ruptured cysts - and they must have had her on four or five types of birth control before they found one that helped. It's still there. I learned to eat around it and drink my calories, that's what ruined me. I barely got through 3 eggs and cheese today. Yesterday a double baconator was two meals. It's definitely not what it once was but it still works great. I can chug Water and bubbles don't bother me. Coke zero and La Croix water are how I satisfy sweet cravings. Studies show when you're in ketosis a multi day fast is beneficial. Between keto and the sleeve I can fast 3-4 days. Now I do have a ton of supplements and did a ton of research before starting. I really struggle to eat more that 900 meaningful/healthy calories per day. But I aim for 1200. I had my 3 eggs 5 hours ago and I am still stuffed to the point I can't consider Snacks or lunch at this point Sent from my SM-G920V using the BariatricPal App -

I, too, am a big fan of the Keto lifestyle and intermittent fasting. I took off almost 6 pounds of regain doing the Keto/Intermittent fasting for a couple of months. So @@Donny, I'm curious. How's your sleeve restriction at 4 years out? I'm 2+ years out and curious. -

I agree. I am very strict less than 20g of carbs. A cheat day for me is still under 50g (which is still keto) and I had blood work done two days ago with great results. The only bad marker I had was low HDL but since my LDL and Triglycerides are so low they didn't seem too bothered by it. My nurse was impressed with the numbers since they came from a guy with a 38 BMI. Last night I had a baconator lettuce wrap from Wendy's and today made full egg scrambled eggs with cheese and bacon. @@Donny That is an amazing story. I too love the Keto way of life. Before I decided to do WLS, I purchased some literature on the ketogentic diet. I started following it and had amazing results!! My co workers were like "how can you eat all that fat and loose weight, your going to get heart problems" blah blah blah. I showed them by dropping 30 pounds in 6 weeks! My blood work was perfect! Then I fell off the wagon... unfortunately... but didn't we all at some time. When doing this type of diet you cant sneak a cookie here or there. If you cheat... it messes you up for at least a week until that processed junk gets fully out of your system. I know that you know this but hopefully it will help some of the others who read this. This diet was developed to help children with ADHD and Seizures. It takes the place of meds for a majority of them!!!! How great is that. Just goes to show that our FDA had been wrong all along. I am a full supporter of the ketogenic diet and I hear a lot of people talking about ketosis on here but from what I've experienced you cant be fully in ketosis unless you are eating little to no carbs. I had purchased the keto strips and tested it. When your in ketosis its like a euphoria. You feel so good, mind and body. I'm happy for you, keep up the good work and positive attitude! -

I was the guy that came here studying the forum for stories of how people restarted successfully and never did anything about it. Or very rarely did you find people willing to discuss this because once they were back on track they didn't come back to the forum. So here is my success story so far. On September 1st I had had enough of regain and decided it was time to change my bad habits. I had VSG 4 years ago and dropped from 385 to 285 in 9 months. Then I slowly crept back to 335, a 50lb regain! So what did I do? I did 3 days of shakes then 4 days of shakes and small portions of chicken. Then I discovered reddit.com/r/keto and started reading. For the last 3 weeks I have been full blown keto with 5% carbs, 25% Protein, 70% fat on 900-1200 cal per day. I follow intermitant fasting on a 18:6 ratio. And I walk 3-4 miles per day. In one month of have lost 25lbs and only 25 more to my lowest but I won't stop there this time! Before I get yelled at about how horrible this is just do a bit of research. Many people have CURED their diabetes with a keto lifestyle and there is actual science backing all of it. -
Haven't read all the replies yet so pardon me if this is repetitive, but I think you need to add FAT, not more Protein or carbs to your diet. When you add more protein your body treats it just like carbs - it stimulates insulin to store it as fat in your liver, whereas fat is burned directly. Yes, they need to be healthy fats, but that includes saturated fat as well. Research is now confirming what our grandparents knew instinctively, that fat from healthy meat (grass-fed, not grain-fed) and saturated fatty oils are much better than the polyunsaturated garbage that has been the mainstay of our diet for the last 30 years. Coconut oil, MCT oil, butter from grass-fed cows, duck fat, the skin off truly free-range chickens. Of course as WLS patients we have to modify things a bit from typical keto/LCHF simply b/c we can't eat nearly the volumes others can. But I would go back to your previous level of protein, increase your Water, and just add 100 calories of pure fat - try Bulletproof coffee for a yummy way, or make coconut fat bombs as a treat. (Google, there are tons of recipes for both)

October 3 here and day 7 of my pre op diet. I have already lost 12lbs in this 7 days. Crazy. I have been feeling so ready up until this morning when I noticed the keto breath and realized I am full on in ketosis. I keep thinking there are people out there that lose their weight with just a keto diet and exercise not surgery. Hi. Keto is low carb. Low sugar high fat diet. So when I stalled I increased my fats. Started cooking my eggs in butter or bacon fat. Eating fattier meats. Avocado. cheese Etc. Worked like a charm. I don't know if ur eating carbs or sugar etc but this is what's worked for me. Now I'm at 137lbs so if I want to have rice here or there or potatoes I'll have it but it's pretty rare and normally makes me hungrier and less satisfied or sick so I don't do it much. -

I don't know the Keto diet but when you say increase fats are you talking about avocados? Are there other fats that helped as well? I'm at a stall now. Only losing a pound or two a week. Not sure which diet you're on, but super low carb can result in "Keto flu". I don't have a fat restriction on my pre-op diet and found that good fats + grey Celtic sea salt really helped me. Like, coconut oil and a pinch of salt in my shake. Or just a grass-fed burger with no bun. My headaches got better when I flipped into full ketosis around day 4. If you only have to be low carb (and don't have a fat restriction) look at some of the Keto blogs. I started a week early and was practically mainlining fat to get me off the carbs. I was right on the carb border most of the first week. But I discovered a Keto faux pancake recipe using eggs + reduced fat cream cheese and that a little melted coconut oil really helps that protein shake stick. And by week two I'd flipped into ketosis and found it to be easier. Also discovered that the Nectar protein packets are zero carb and awesome to keep in your purse. @@Hadenough2 I eat sausage, but I just started within the past 2 or 3 months. I had to find a way to increase my calories and I needed the fat and variety in my diet. I lost most of my weight so far just eating fish and seafood but I added pork in because it is economical and adds variety. I dont really watch my fat intake other than to keep it high enough, and I keep my carbs low. I eat more of a keto diet, which isn't that common among WLS patients. So you have to choose for yourself.

i just went to the mr. olympia convention and tried Quest for the first time. they have a new waffle flavored one that legit tastes like a waffle with maple syrup. altho sweet for me the taste is amaze. they also have a new snickerdoodle Protein cookie that's killer. they also now have protein chips, goldfish crackers and pasta! the chips and goldfish are amaze. have tried the Pasta yet but i'm excited. their ingredients are far superior to others so i'll keep trying their stuff. it really fits my keto lifestyle.

I did a lot of cardio in the beginning to take advantage of the honeymoon or rapid weight loss stage. At about 7-8 months out I added in more weights. I found that extreme low carb/keto zaps a lot my ability to lift heavy so if you are trying to lift, eat a balanced diet in addition to getting in your Protein.

@@Steph~L~16 i don't track calories. never have. i eat keto. no carb no sugar high fat. for me increasing my fat is what got me out of my big stall. its usually about eating more rather than less or more often. everyone is different but that's what worked for me. good luck! -
No drinking with meals?

i know everyone is different but i'll be a year out in October and i drink and eat whenever i want (yes, even at the same time with meals). ive had carbonation, ive had alcohol, ive had carbs. now as a general rule i don't do the carbs, carbonation nor sugar, i keep it pretty keto, but i CAN if i want without issue. if you aren't very far out definitely listen to your nutritionist just letting you know things get easier and your tolerance gets higher for certain things. just be patient. -

I know this is an older post but hoping someone can help i had RNY 4 mo ths a and have been using Keto diet for 6 weeks and feel good on it But I eat 1200 cal a day and exercise 5 days a week, yet stalled for 4 weeks well only lost 1 lb and clothes not getting any looser. i still have 50 lbs to loose and cannot seem to get weight to drop again 1200 cal 100 gr protein 15 gr carb 80 gr fat aerobics for 45 min per day thanks -
10m 100 down 100 to go still need help stalled nutrionist said impossible to loose 100 more :(

@@Munecagirl85 100 pounds in 10 months is really good. Your body might need to catch up. How long have you been at 265? You said you keto, what do you eat? How many calories a day? If you have really adopted a healthy WOE like keto, the weight will keeping coming off. Why did your nutritionist say it is impossible to lose more? Find a different nutritionist, or don't use one at all. -
So i am weighing in at 265-266 flip flops week to week iam still in a size 22 ive loat 2-4 sizes maaaybe since 100 pounds ago i meausre and no difference I do not eat carbs and i have adapted a keto life style ... I do not cheat and i exercise I refuse by pass so what could be the issue i need tl change skmething but what Has anyone lost 200???

So if you are a patient who LOST, then GAINED, then LOST it again, please share with us all your method of losing. Did you go back to pre-op/post-op phases? A 2 ,5,8 day pouch reset? A specific low carb type plan? Keto? Paleo? Please feel free to share as this is helpful to each of us in our journey.

@@Cassie111 I also have a book, with all kinds of nutritional information and outlines, but besides limits on the amount of sugar in Protein shakes that is allowed, there are no carb recommendations. I am below 25 net carbs most days, and always below 50. You have to find a way of eating that works for you. I eat low carb leaning towards keto. I eat this way because I enjoy it and it is sustainable for a lifetime for me. -

Do you feel like the sleeve has finally allowed you to keep it off? Do you think you could have sustained weightlessness from pre op if you didnt end up getting the sugrey? Was it because you had something to look forward too? My biggest concern is that i could maybe try the "diet" for a while. But i've even done Keto and when i get hungry i just want to eat. I'm concerned as a few people still said they felt hungr post surgery . Maybe it was head hunger but how do i even over come this? Not sure about what your post op diet looks like. My surgeon was very clear and laid out a very simple eating plan. Specific meal sizes that were initially all chicken, steak, fish.....then at 6 months begin adding crunchy vegetables. So....by definition....it's a keto diet to begin with so there is no other strategy for me but to follow the normal post op diet. Stalls are what occur when I don't.
