Sleeve Veteran researching revision to SADI

I'm so glad you tried the muffins and liked them!! I really enjoyed mine, they kept well in the fridge and warmed up really well with a quick zap in the microwave. Regarding the nutritional needs of a SADI patient. We don't really know, honestly. The SADI is too new to have some of the longer term studies that have been done on the sleeve, bypass, and DS. But it is commonly thought by most surgeons that the dietary needs of a SADI are more than with the bypass because SADI patients are thought to malabsorb more than a bypass patient. And their needs are most likely a little less than a traditional DS. That said, I attended nutrition classes with all the different surgeries in one group, so we had to learn everybody's requirements!! Most good surgeons recommend that SADI patients follow the dietary requirements of a DS patient to be on the safe side. That means 80-120 grams of protein, less than 50 TOTAL carbs while in the active weight loss phase (the dietician said "net carbs" is not an official measurement, but if you want to follow net carbs you'd need to keep under 30 net carbs since they get counted differently, most DS patients introduce a bit more complex carbs into their diets once they reach maintenance weight) and the fat grams recommended by the ASMBS is 60 grams of fat by 1 year post op. However, SADI and DS patients malabsorb a good amount of fat, so veterans of the surgeries will tell you that you need more fat than that. I seem to feel best around 80-100 grams of fat a day, but there are veterans of the DS surgeries that eat closer to 150 grams, or whatever alleviates their constipation and eases their symptoms of low fat like dry eyes and dry skin. According to most studies, a DS patient only absorbs about 20-30% of the fat they consume, around 60% of the protein they consume, 60-80% of the complex carbs, and every single calorie of simple carbs!! That's why simple carbs are so important to restrict post surgery (besides the digestive drama), you can eat your way around any surgery with simple carbs... And this is why counting calories is pretty useless for DS and SADI patients. We don't yet know exactly what the percentage of absorptions is for SADI patients as they haven't done a breakdown study (as far as I know) but it is safe to assume SADI patients absorb a little more fat and protein than a traditional DS patient would. The best way to find your own perfect macros post surgery IMO is to aim for 80-120 grams of protein and then watch your protein labs. They will tell you if you need more protein or if you can back off a little. And for me at least, I find my skin, hair and eyes are highly sensitive to when I have too little fat in my diet. -

Hi there!! Thanks for sharing your experience! I will let you know that mounjaro does make my reflux worse!! because it slows down my digestion. It also causes constipation in some people. But if you don’t have these issues it’s good! I have not had it yet but I’m going to get vsg. However I do wish I could get the SADI but my insurance won’t cover that one. I have heard people do so well with the weight loss and reversing diabetes. It’s just expensive without insurance. But would if I had the financial freedom! Bypass is also very successful too! Check out some podcasts like “the skinny truth” or check out Dr. Roller’s instagram. I listen to “Core Bariatrics” and “The Bariatric Grind”! They’re good listens in the car and super informative. Of course following your trusted surgeon’s recs are #1! -
HI All! I had gastric sleeve surgery on 12/6/23. As of March 7th, I am 45lbs down. I stopped weighing myself after this date, though, bc I was becoming OBSESSED with the scale and would fall into a deep sadness each day I didn't lose at least a pound. So, I decided to put it away and focus on my feelings and habits. Anyway, lately, I've noticed I can eat much more than I was. I watched a YouTube video about a doctor explaining that after month 3, patients tend to notice they can eat more. He mentioned that it usually freaks them out at first because not being able to eat a lot at all was the major tool in helping the weight loss, but to not be alarmed because it's normal and to focus on healthy foods, working out, etc. Still, it's scaring me because I am scared I will fall into old bad habits and start overeating again. So I was just wondering, has anyone noticed this? I noticed I can eat more than I was 2 months ago. It's freaking me out a bit bc I'm scared I'll overeat, esp bc certain foods are easier to overeat than others. The other day, I got a bag of organic Doritos and ate the whole bag throughout the day. I know I shouldn't have, but I was weak. So, has anyone experienced this? Has it affected your progress at all? Please be kind. ❤️ Thanks!

I'm 5 yrs out from byoass/11 yrs out from sleeve and at goal. I've been adminning a small WLS board since I inherited it in 2015 - we have a close group of mostly ladies (a couple of gents) and most of our people are fairly far out from surgery but we do have a few newbies. We do 2 Weightloss Challenges a yr and they're pretty fun, We have 3 prizes and they're decent prizes. 6 wks long, % of lbs lost, weekly weigh ins, rules - the whole sha-bang. Lots of support. The last challenge, I was going thru some stuff and I let life get in the way and while it went off without a hitch, as my co-admins are amazing, it wasn't the same as usual. I promised everybody the summer challenge would be GREAT! I started working on it (yes, I have help from my co admins) and I have daily education, Fun Food Facts, I do something where I ask everyone to say something supportive to someone they don't know well, and I try to do small weekly challenges. It hard doing everything online. I do a hydration challenge, a moving challenge (try to park a little further, put in a few more steps in ur day), no fast food for 7 days , no snacking for 7 days, do (however many) crunches (however many) wall push ups, etc. You get the idea. The challenges are getting stale. Thinking about doing a "take ur pic in an outfit on day 1 and on last day and see how it looks" challenge. If you were in an online challenge, what would you appreciate doing? What would be fun? What kind of information would u like to see or know? What kind of support would you like to see or get? If you'd like to get involved, you r more than welcome to join us! It starts July 24 and goes 6 week. I will be posting info on it on the website in the coming weeks but u can certainly do a search on challenges and bring up a TON of them! We'd love to have you, however you DO need to be 6 weeks out from surgery (for fairness, as you lose a TON of weight early on, not quite fair). I worked with a therapist beginning about a month and a half before surgery, until I was over a year out. I just recently stopped seeing him. I'm still struggling with this. At my initial consultation, my surgeon told me that obesity likes to come back. That a lot of patients end up needing to go on medications, after the surgery, due to weight regain. For some of us, this is going to be a lifelong struggle, I'm afraid.

My bmi wa 37 but I only carried my weight in my belly. I was on 20 mg of protonix then after I was bleeding internally they bumped me up to 40 mg. I’m daily and now dr wants me on 40 mg of protonix and 40 mg if Pepcid which to me is stupid this is my bariatric surgeon. I’ve been arguing with her that none of this is working and my burping has increased back again. That’s a lot of weight loss in 6 months, I’m still at 180 and it’s been 4 months. Idk if I could drop that fast in 2 months……… I already lived through the dos and donts as a GERD patient. -
TL/DR: Worried about losing weight by not eating enough. | Looking for tips to reach protein/calorie goals when not hungry. | Looking for others' experiences with lack of hunger this far out in WL Journey. ------------------------- I meet with my nutritionist for check-ins about every 3 months. For this last one, my 9-month, she told me not to be upset if I hadn't lost, as a stall was expected. Yet, I had lost just as much as I had been. We went over what I had been eating & she wasn't concerned about content but suggested increasing variety. [I'm a meal repeater 🤷♀️] Though, she again warned me that it is now especially likely that I would stall since I hadn't yet. Since that appointment, I've still been losing weight. I know it might appear strange to worry about meeting the goals we are here to meet, but my nutritionist is big on trying to relearn the natural cues of the body. So, as they are absent, you can notice pressure in your stomach, sighing, or a runny nose as indications of fullness. As indications of hunger return, you can utilize them to increase portions within the parameters of slow eating and not exceeding controlled sizes. The thing is, I don't know that the initial increase after, you know, the literal wounds healed, I have really expanded much more. I don't really get hungry until 'early-bird dinner' time. So eating during the day is purely done out of obligation. [& if I'm being truthful, there are times when I forget entirely until after work.] So obviously, that leads me to try to have all of my calories in one 'meal' in the day, which I have to eat over a couple hours [even if it is small] if I don't want to get nauseous or actually regurgitate. [The regurgitation has improved slightly over the last couple of months. I had a very reactive stomach that required me to prolong my dependence on protein shakes. Pre-surgery, I don't think I came even close to needing to vomit since pre-school 🙁.] To be fully upfront, I have not counted a single macro or calorie or weighed a single food item during my entire journey. I've tried tracking before in my weight-loss journey, and it was not healthy mentally for me. I realize that I may be under goals rather than over them, but straightforward counting of calories is just not something that would be part of a successful journey for me. So, all of this is a longwinded way to say: - Is anyone else struggling to eat enough this far out? - What helps you to reach your protein/calorie goals when you don't really want to eat? - Any other tips/tricks/specific brand or meal suggestions?

Oh we had our surgeries about the same time and i had to stay for an extra day too due to complications. Congrats on 60 lbs! Well i'm asian and while asians are known to be pretty quiet, when it comes to weight- they don't hesitate to comment. Whether it's your family, friend of the family, distant relative, or someone they've seen a few times at the store. They call you over for dinner and when you've finished, they tell you you're fat and pack up all the leftovers for you to take home. (Say no all you want, it's gonna happen anyway). That being said it's kind of concerning that they haven't said anything at all. I don't go out much so it's mostly dr's visits and they've noticed and said something right away. So i have a weird sample size. But i have to see i dont really take any comments to heart, i just wanted to see if anyone could tell and it's kind of a mixed bag atm.. -
Doubts about plastic surgery

i had an arm lift (and breast lift, and tummy tuck...all in one shot in Dec 2019). i always disliked my upper arms. when i was thin (and thought i was fat) i was self conscious of what i thought was the size of them and rarely wore sleeveless tops. then when i actually got fat i NEVER wore them. if i had to go to an event the warranted (an arm-baring) dress, i always wore a shawl. fast forward to the weight loss and i was left with some saggy jiggly upper arms. while they looked okay when my arms were at my sides, i really did not like the look of them flapping in the wind when i raised them, lol. and i raise them alot, ha...i'm one of those raise-your-arms-in-the-air-and-wave-them-like-you-just-don't-care kinda gals, lolololzzz. so i had them done about a year post op (6-ish months after i reached goal). LOVE EM. while i did wear sleeveless again after the weight loss, i feel a lot less self-conscious about them after the arm lift. and really, it makes all the difference :).. now i do have very prominent scars on my arms (i scar very badly, and knew this going in), but for some reason the scars dont bother me as much as the flapping did. Go figure. recovery is no joke though, i'll be honest. had i not done too much too soon, i would probably have been good to go by 6 weeks...but i didn't, so my recovery was more like 3+ months. that was over 5 years ago now, and i've been wearing tank tops and other sleeveless tops all year round since, hahaha. it really all boils down to what YOU are comfortable with...i was told by countless of people that i didn't need to/shouldn't get it done. but i got it anyway, because the only person who can say i need it is ME. if you have the means and the desire, the option is yours. Off to a slow start after surgery. Anyone else?

That's good to know! I was shocked I lost so much in the pre-op phase. It took me 6 months to lose 15lbs from my highest weight this past summer, and then I lost the same amount in 2 weeks. So I imagine my body is reacting to that as well, like what the heck are you doing?! I just got to start soft proteins a few days ago (my doctor skips purees) and my calorie count is increasing as well. I seem to be able to get around 30g protein from food, and I'm still supposed to have 3 protein shakes, so I'm ending up with 90+ grams of protein and 600-650 calories. Pre-op I was at 600-800 calories per day and 80-100 grams protein, so maybe that's more of a sweet spot. Hopefully this will give me a nudge in the right direction! -
I really don't understand how my weight is distributed. I'm still in the "obese" category and definitely overweight. But I have visible ribs already when I move, including the ones below my collar bones. I didn't see these until I was very thin back in the before times. If I lose another 50-70 pounds, am I going to look like a walking skeleton wearing a leather cloak?

@Spinoza is correct. It is easy to do because you can’t tell you’ve had enough yet. Besides the staples & sutures holding your tummy & digestive system together, a lot of nerves were cut. So signals we used to rely on aren’t working or aren’t working properly yet so those messages just aren’t getting through. It takes a good 8 weeks to be fully healed. So it may not be until you’re into your third month before you start to feel signals again. Oh, and your signals may be different to what they were. And, no you won’t have stretched your tummy or impacted your weight loss by doing this this one time. And you would know if you had damaged your tummy. Just tread carefully & slowly & follow your plan to protect your healing tummy & support your recovery. -
I just want to say that this thread makes me smile. 5 months ago, I came out of a 7 year relationship. He loved my body and I’m grateful for that, but unfortunately has a lot of demons of his own he was not willing to work on, so it ended badly. Now that I’ve started this journey and am losing the weight, which is still so new as I’m only one month out, I see the change in my interactions with men and I find it very interesting. I am TERRIFIED of eventually getting back out there and dating. I’m afraid they won’t understand my story, my body, the inevitable loose skin or that I will be a different person entirely. It’s such a strange thing to think about. I’m just happy to hear that so many of you are in body positive, supportive relationships. It means those men are out there.

It doesn't sound like it. I know that the amount sounds like a lot compared to the can-only-eat-three-spoons-of-pureed-food-and-even-less-when-solid crowd but the amounts is not what strikes me odd. If the foods you're listing are fairly typical for a day of eating you simply might be lagging nutrients. No fruits at all, only a little bit of vegetables. I'm not surprised. The sodium content of the foods you listed seems to be fairly high. Quick weight loss or weight gain usually almost includes some water retention. My weight can swing up to almost 1 kg within one day because of how much water I hold. You say you want to start tracking. Are you a person who likes to track food, a person who abhors it (raises both hands simultaneously here) or a person who is not a fan but in the end doesn't mind it? Tracking foods/calories/macros can help people - however, it could be that you can benefit from a different food selection without even having to write everything down you eat. How far out of surgery are you? Are the stats you're listing in your profile still correct? -

Yes, pre existing gerd is a hard no for sleeve in almost all cases. Sleeve won’t fix it & will likely make it worse necessitating a 2nd surgery to convert to bypass. I had reflux/gerd before my sleeve. The only reason my surgeon decided to proceed with a sleeve was because my reflux was mild & I managed it for years with dietary choices. I only took meds a couple or so times a year. If it was more severe, & I needed regular medication, he wouldn’t have done it. My reflux is worse & I take meds every day which manages it but the symptoms are different. We have similar stats in regards height & I had a similar starting weight (200lbs). Having reflux didn’t hinder my weight loss. Plus I’m menopausal (perimenopause & menopause was when I put on the bulk of my weight & pushed me to obesity). I lost all my weight plus more & have kept it off. And I wasn’t active at all Lol! Avoid comparing your rate of weight loss with anyone else. We all lose at our own rate & what is best for us & our body. Comparing usually leads to frustration & worry. Have you met with your dietician recently & gone through your meal tracking? There may be something you’re missing or need or don’t need. A stall of more than a month isn’t common. Try to increase your fluid consumption. You should aim for 64oz or likely more as you say you are very active. And yes, while muscle weighs more than fat it’s not that much more. Say a container of fat weighs 1kg. The same container of muscle weighs 1.2kg so we only talking ounces of difference. Have you been taking body measurements to better understand where you might be in regards to muscle growth? -
i made a vsg four years ago, i started from a weight of 148 kg, going in three month to 122 kg. than arrived covid. italy was in lockdown, i regain weight to 138, than go to 126, back again to 134, in this period i see the 119 when i was sick, and now back to 128. i feel like i was a failure, and i don't know what to do. i work with a dietitian and an endocrinologist but nothing work on me. what am i suppose to do? i am the only that have that problem? in the first year i was alone, nobody follow me and now it's so difficult make it work

I love this topic, I'm recently thinking about how I will want to dress when I hit my stable weight. I've reinvented myself many times over the years and now I'm going to again. Right now I'm only wearing sweatpants and pajamas since it's so cold but come summer there will be lots of dresses. I'm 7 weeks post op and already my swimsuits are baggy and I'm hesitant to get new ones, I don't know what size I'll need now, and I don't think it will fit for very long, but maybe worth it anyways then donate it. I did splurge on a pair of jeans that I love but they're starting to sag on me. This is going to be fun and a bit scary I think. -
short answer: yes! upper/lower/mid back pain improved! as did foot pain, knee pain! long answer: it got better, then it got bad again, and then better. apparently rapid weight loss results in weird aches and pains as your body musculature re-adjusts to your smaller size. lower back pain returned and i got a new hip pain, but then both went away. aside: but one thing that was new and annoying and hasn't gone away is that get back pain now when i sleep on my stomach (i never had this problem before)...my guess is because now that i don't have a larger stomach to prop me up, my back is actually hyper-extending now when i'm on my stomach which results in pain in the morning. boo.

You had surgery almost 50 days ago, and your worst "transgression" is one instance of drinking tea with your food and consuming 2 fries and half a small onion ring? I have a feeling if past you could see this, she would not see much resemblance to herself. If eating those things felt triggering, like you're afraid it will be a slippery slope to undoing all the changes you're making, that's definitely something to work through, either on your own or with a therapist. I totally get that fear. But the hardest mindset shift, I think, is to realize we're not on a diet. This is life. Carb counts and protein goals and fat limits and daily calories are helpful guidelines to be mindful of, but they aren't laws that must be followed 100% of the time or else. Instead of beating yourself up, congratulate yourself on being more than halfway to your weight goal! And next time you have an event, decide ahead of time how to handle it if there's nothing there you can eat so you can go in feeling confident you won't have a slip.

I've stuck to buying underwired bras - two at a time and only when the current bras were clearly ridiculously large - as I lost weight. I tried to buy reasonably cheap versions - sometimes that worked out and sometimes it didn't! I'm at goal now (regaining a little if anything) and buying 'forever' underwear, LOL. I've never needed anything officially compression but I know others prefer that. I don't mind a bit of saggy skin underneath my underwear. Nobody sees it! -
OK - firstly, you are doing absolutely fine. Anyone who has lost 60lbs in 5.5 months is WINNING! We all stray from the path a little at times! You have already identified where you strayed - carbs. So from tomorrow, get back to your surgeon's plan. I suspect it will be protein first, veg second, carbs last (by a long way). Stick to that. I have a sleeve and I was still losing miniscule amounts of weight almost 2 years out. You have loads of time to get to where you want to be. I wish you luck!

I recently got diagnosed with stage 1 type 3 Lipedema 1.5 years after my VSG. The reason that pushed me to explore because I had lost weight all overall but my legs didn’t change much compared to the drastic changes I had in the rest of my body. The pain, heaviness and bruising I had before surgery continued. I had originally thought it was related to my weight. My surgeries are booked with a Specialist Lipedema Surgeon, they will take place later this year (hopefully all goes according to plan). I was wondering if anyone else is going through this too? I would love to hear anything you would like to share.

Hey! I'm SO glad your pain is better managed now than it was a few days ago! I totally hear you about the passing gas (and btw I need to go back to tagging multiple people in a reply, feel like I spammed this board >. ) I'm nervous about taking too much stool softener because I know with the SADI loose stools can be a thing but at the same time with all these danged protein shakes I'm just not sure the colace 2x day is doing enough. I hope bowel movements level out for me soon; I've gone from one extreme to the other once already and don't want to do it again lol -- We made it to the other side, though! I'm so happy for you and hope your recovery continues to go smoothly. I'm interested in the binder thing people keep talking about - I wonder if they didn't do that with me because I had the SADI/SIPS instead. (For those not familiar with the SADI/SIPS, it's basically just starting with the sleeve and the surgery they most commonly do to revise it by shortening the intestinal tract beneath in place of the Gastric Bypass right off the start - I chose it because I wanted to eventually be able to take NSAIDS and there's a thread here that discusses the SADI/SIPS more. My surgery was still laparoscopic and recovery has been about the same from what I see, just fewer points of connection internally to heal) I'm also excited for all the rest of you folks with upcoming surgeries! I don't want to spam anymore but did want to say to @Singingbarista and @AndreaJD that the insights you've offered were super helpful, especially in terms of 'telling others' about the surgery. That's GREAT advice - to say you've been working with a dietician and weight loss clinic. You're so right; it shouldn't matter what others think but women especially have long been taught to seek approval. It's just another thing to relearn and the amount of support on this forum has been absolutely fantastic. I finally got a delivery of Premier shakes this evening in flavors OTHER than chocolate, French vanilla, and Mocha and oh my goodness it's SO nice to taste something strawberry-ish! I didn't want to keep responding to each post independently but to those who had very little notice about the surgery date - that's for sure a mixed blessing! Less time to prepare, but less time to be nervous about it, I guess? Sometimes it's easier to just plunge in (as long as you feel educated and ready!) To all of you going through the liquid diets, especially if you feel hungry during it - I'd suggest you might really want to consider trying some of the 30g protein shakes. I was very worried that I would be hungry but those are surprisingly filling, especially if you drink a lot of sugar free flavored water / Gatorade / Powerade of some kind. And don't forget you can enjoy sugar free popsicles, flavored warm broths, fat free cottage cheese, sugar free pudding, sugar free jell-o, and sugar free yogurt (usually - I mean, obviously follow the guidelines you were given by your surgical team!) The pudding can be made with protein shakes to help with your protein intake instead of milk, and if you match the flavors it's actually quite tasty and filling. -
Anxiety is awful. I used to stress out majorly due to my career, but even having been retired for ten years, I still have problems sleeping. I work out like an animal, now that I've lost most of my excess weight, and keep myself mentally engaged in a lot of things, but I still suffer from insomnia. My one doc, who I've used since the mid-90's, still has me on diazapam (addictive, but at some point for me, it just doesn't work so I stop and clean out) and Mirtazapine, which definitely puts me down most of the time. Only problem with the latter is that I don't wake up til 11am. Not good if I'm attempting to live a normal life. But, he's against sleeping pills and I get it. The other things, like Melatonin, just make me woozy. I'm in way better health as a result of the surgery and working out constantly. I'm consuming lots of protein through drinks plus food. I feel good. The lack of sleep can definitely ruin your day. I don't know what the current thinking is in the medical community. I trust my guy but still, it kinda sucks to have to medicate to get sleep. I'm real big on integrative medicine. Maybe there's an answer there. Dunno. Nice to see you folks!!!!

Congrats on your surgery and welcome! I had a 5 week stall with losing and gaining the same 2-4 lbs. I'm 9 weeks out now and it just broke!! I started being really obsessive with getting as much protein as possible (I'm a DS patient and we need alllll the protein), and making sure I was getting at least 64 oz, more like 80 if I could help it. And then I added steps in, increasing by 500-1,000 steps a week. And then 2 weeks later the stall FINALLY broke! I don't know if it was a coincidence or if what I did helped, but I'm really thankful it broke. I lost 5 lbs! Hopefully it'll continue down from here, the stall weeks are stressful.
