It could well be the low carb count - such lethargy is a hallmark of low carb dieting (one of the reasons that I never went that direction - I couldn't afford the side effects.) The keto enthusiasts insist that you are supposed to be able to burn fat to get that energy, unfortunately, our bodies don't read those diet books so don't always know what they are "supposed" to do. Talk to your surgeon about your needs and what can be changed if his program isn't working out for you - you may be doing more work than your surgeon was anticipating in providing those instructions. Timing can be an important consideration if you have specific workout times when you do your walking and run into your problem. A small snack an hour or so ahead of time, composed of something relatively high in complex carbohydrates, moderate in protein and low to moderate in fat can provide the longer enduring energy needed to extend things some - this is what I worked out with my RD when I was running into a wall about an hour into my swimming at about 4 months out and hit the spot just right. I did not increase my calories for the energy, but just reallocated what I was already consuming. If your walking is more spread out during the day, it may take a more generalized increase in complex carbohydrates or fats, or a bit more of both, during the day to meet those needs. Another thought is that it could be a bit of anemia - have you had labs checked lately. Iron is the traditional problem but B12 is sometimes a problem - some programs recommend B12 supplements as a matter of course and some don't, and some people are more sensitive to it than others as well. Also, double check your hydration, as you may think that you have been well hydrated, but if you just increased your exercise levels a month ago and didn't increase your hydration to compensate, you may not be doing as well as you think. -
The premise of the keto diet aligns quite well with "keeping carbs in check". I'm not much into the "high fat" aspect of keto, but I am okay with moderate healthy fats, high protein, and 0-4g of carbs a serving. Yes, it rules out a lot of the crap I used to eat, but it also depends on the purpose.. for me, it's merely to stay alive and be healthy. One needs very small quantities of carbs for energy, that's all the liver can store and make use of anyways before converting to fatty acids and distributing it. Given my extensive liver damage from garbage foods I'm almost excessively aware of the implications of reversing this course.

Did you have to exercise up to 2hrs and 15mins or equivalent it to 5000 steps everyday to be cleared by your psych?

I had an issue with my NUT who did not believe that I was walking 5-7 miles per day pre surgery. It would go something like this? NUT: What are you doing for exercise? Me: Walking 5-7 miles per day. Nut: ***Side Eye, look me up and down,*** "mmmmmhmmmm" Me: I mean not like at one time, but I walk a mile at least 3 times per day at work then 1-2 miles when I get home, not to mention my in between walking just to get from one place to the next. Nut: ***Side Eye, look me up and down,*** "mmmmmhmmmm" Me: I can let you look at my fitbit stats, if you like. Nut: ***Side Eye, look me up and down,*** "that's not necessary" Me: are you sure, I don't mind. Nut: What's your diet like? Me: I eat 1200 cal per day, modified keto, except on Saturdays when I eat what I want, but still keep it under 1200 cal per day. Nut: ***Side Eye, look me up and down,*** "mmmmmhmmmm" It was exhausting. My psych eval was stupid, pointless and literally was a check yes or no list. -
OutsideMatchInside I think pinned it down with talking about people's body composition. I rarely find people suggest that everyone benefits equally or everyone should do this diet, because everyones tolerance, discipline, comorbidities, genetics, etc. are not the same. But that isn't the issue, the issue is suggesting the keto diet has no scientific basis, which is 100% wrong.

Most people that long term Keto are calories aware, even if they aren't strict even with non restrictive stomach. Long term successful people fast also, which I do. A high fat diet makes it easy to fast (the original purpose of bulletproof coffee, to extend a fast). Fasting has its own benefits and I'm not even going to go too far into that First it is really important to know your body composition you can calculate your macros correctly. Most people that really Keto track (I notice anyone that is successful at any kind of way of eating or diet tracks). I use a Keto calculator and peg my protein to my lean mass 1g per 1 pound. This one is good https://ketogains.com/ketogains-calculator/ The math on that one is supposed to be better but I use this one because I like it better because of the loss projection chart. https://keto-calculator.ankerl.com/ As a sleever if you are meeting your protein goals, it is going to be almost impossible to get your calories too high just because of the low calorie count of protein, the density of protein and restriction. If I didn't use heavy cream in my coffee, I would never hit my fat goals. 120 grams of protein a day doesn't leave much room for anything else.

Easy with the high horse. Some of us currently hide behind said academic walls with dubiously obtained doctorates in Epidemiology. I suppose that in 'T.A-ing' 2 nutritional Epi courses, I must have passed along tons of 'crappy science' to students right? That's irrelevant to this topic anyway. I don't know why you're going on about Keto not being a fad, as if I said so somewhere. I just repeated what some studies said. On another post when I mentioned contrary study findings, you deemed them to be wrong, because they didn't agree with your "undoubtedly lengthy, and peer reviewed" research. oh well, I have said my piece.. moving on....

If you actually ever did any research or read any studies about high fat diets, you would have never even posted the first 2 sentences, because there are numerous studies not to mention several cultures through out the world that have been eating this way for hundreds if not thousands of years. Which is why I suggest readying a book by scientists and not reading bro science on the internet. Most of the studies I could link are behind educational walls. I did years of research on this before deciding on it. It isn't just a fad diet or pseudo science which is why I tell people not to even bother if they aren't will to make a life long lifestyle change or it flat out won't work for you. You can't just Keto for a while stop pounds and go back to pizza. The same way you have to make life long changes for WLS to be effective long term. I know talking about science here is kind of a non starter since 99.9% of people that post here don't even understand the difference between weight and volume. Science is beyond most people, don't even get me on understanding stats. I know everyone doesn't have a STEM degree but damn.

The data used in the link I provided does provide as much data as is available. It's why I cited it rather than one of the journals where you see the results but no data, or where other people would have to pay for access to see the data. Nothing is perfect, but as pointed out earlier, whatever diet that someone can comply with that is healthy is likely the best for them. For some this is the keto diet. In my case, my pre-operative weight loss was exclusively because of this diet after the misery of the first few weeks attempting a liquid diet drinking carb-laden "Glucerna" which is supposed to be for diabetics, yet was demanded by my RD. What I find more about people on the keto diet is they tend to be more informed about the effects of it. Vastly different than the similar Atkins fad where people had no idea what they were doing, just jumping on a bandwagon because commercials.

Nope. Although Atkins was close to Keto. The original Atkins diet by Dr Atkins, the diet has been really bastardized since his death. All those trash products they sell in the store are pretty against his teaching. Keto has a lot of science and studies to back it up. I feel like if people can't process or understand the science of running the body on fat they shouldn't even bother. I see so many people making bad choices on Facebook etc and they don't understand the science. If you understand the science it is so easy to make the right choices.

That's the funny thing about Keto diet research. It has been demonstrated to work in the short term.. long term studies show non significant to non clinical differences. The health concerns about long term keto diets have however, been allayed in the same studies. It was feared that it may alter lipid homeostasis in obese individuals and some studies have shown that it isn't the case. Cursory search is one thing, meta analyses is another thing. I have zero interest in bashing or praising. Just saying there are no set rules to this thing.. Even Bariatric Surgery has had its issues going forwards. It was a suggested 'solution'. Now it has been downgraded to a 'tool' you use to work towards a solution.. because research.

Any cursory search of med journals reveals results synonymous to this paper. While there is long term data lacking the same can be said of the VSG itself yet its clear efficacy makes it justified, just like this diet. And reminder: this thread is about the keto diet. I haven't read all posts here but I don't usually see diet fanaticism. My issue is bashing the diet and equating it to pseudo science when there is already real science out there. While there are lazy people who give credence to fad diets of all variety, it makes no difference to the science behind the efficacy of this one when done properly.

I did modified keto for almost a year before I had surgery. I lost 15 lbs initially then went off it when my father died. I utilized modified keto in the 6 months prior to surgery and lost 30lbs on my own.

I read no trashing.. He's just stating a simple truth about research. Nutritional research keeps advancing so nothing is hard and fast. Research that would have nailed the keto diet (7 countries study) was thrown out years later by further research. Just like that, we had to stop blaming eggs for heart attacks after blaming them for years. Basically, if it works for you, keep doing it. That's cool. I've also seen videos of people being sleeved via scope as well. To @RickM my RD put me on a 3 month liquid preop diet consisting of almost entirely Glucerna and water. Each bottle of that crap not only tastes like s**t making me regurgitate the stuff but has 26g of carbs. My RD wanted me on 6 of those a day - over 150g of carbs a day on a liver shrinking diet. It costs 8 dollars a six pack for this so over the span of 3 months that's 700 to 800 dollars in food for us, not counting food my wife and daughter need as well. This is why I researched (not Google as its full of the fad diet garbage sites) the keto diet and the fact that it's premise aligns directly with liver shrinking and thus weight loss. The liquid diet is, I suspect, more a general safety net measure by RDs to gauge "compliance" by the patient. It's very clear with my RDs comments in the Canadian NetCare system that she's been pissed with my lack of compliance despite all my positive results. Which is why the surgery is moving forward without her being happy.

That's a nice wall of text there but you didn't refute a single thing in this paper, nor did you substantiate your erroneous claim of "scientific gloss" when it's clear the keto diet is getting quite positive response from the scientific community, especially concerning weight loss and WLS. All you did was move the goalposts with a projecting statement of: "one diet works better than the other". Recall that this is your argument in trashing a diet without any scientific basis.

Looking for actual valid scientific studies that show the correlation between high saturated fat and low saturated fat in long term heart health if anyone comes across any. The information I am finding so far is that high saturated fat has a small or no impact on heart disease (or at least none found yet), and higher unsaturated fat is proven to decrease heart disease. There is so much inaccurate information out there that I am trying to stick to proven studies. Been thinking about Mediterranean Keto, but that basically takes a proven heart healthy diet and removes a large portion (carbs), so how do we know if it would still yield the health benefits?

I am 6 months out and i get that feeling. Its maddening because it feels like i cant eat enough but at the same time i know im stuffed. I have never had a problem with restriction. In fact sometimes i feel like i may have more restriction that a lot of other people. Reading how much you can eat at 8wks out makes me wonder if my sleeve is extra small. I can barely get in 2oz of dense protein. Forget adding veggies after. I would literally have to have veggies on the fork with my protein to get both in. But i do have days that i eat things that are unfavorable. Cookies and candy have been a weak spot for me lately. So im starting back on keto or just low carb. I noticed the less carbs and sweet beverages i have the less i want them. If i drink just water and my shake i wont want juice. But soon as i start sipping juice (even just crystal light) i want more and more then it goes to soda and thats when i have to pull back before it gets too hairy. Its a slippery slope. My nutritionist doesnt think i should be so strict but giving myself an inch is what got me to and kept me over 260lbs for years. I cant afford to let myself slip up during the best time for me to be changing my habbits and mindset. Maybe you can try strict keto for a bit and see if that helps. Having dense protein is the ONLY thing that will sufficiently fill me for a signficant amount of time. I dont need the other stuff. Fruit is really a filler/slider because of the water. It will give the illusion of fullness but for me it passes relatively quickly and im left upset that i have to now eat again. Same with breads and similar carbs. I know its just mental because physically its not that comfortable for me to eat it so it makes no sense that i even bother. Definitely if nothing else try to make dense protein the bulk of your meal. If you can, go for the veggies but if you have enough protein in then the fruit and other things wont be an issue. I know this went on for a while. I hope i gave you some ideas and a little help. -
With Keto and sleeve do you monitor more of your calorie count or focus more on fat & protein/g consumption? Because in reading the Reddit thread, people don't have restrictive stomachs like we do

Yeah I notice in a lot of the keto groups on facebook people go nuts on the fat. like I eat bacon but in moderation. Most of my daily fat just comes in the form of cream in my coffee, other than that I am pretty lean.

The oils I buy are olive, coconut, peanut, and real butter. Milk, yogurt, cottage cheese are all full fat. I plan on settling into a low carb lifestyle... not really full keto, but will aim for 65g/day. I've tried before, but feel my sleeve will make it possible.

I was stalling for about 6 weeks and started Keto 2 weeks ago. It helped me with satiety and I broke the stall and lost 3 lbs. I feel very satisfied eating keto. But I am still reading and weighing the types of fats I want to include. Keto emphasises a lot of saturated fats. And new evidence has come to light that saturated fat is not as bad as we were led to believe, although many reputable sources encourage a higher percentage of poly and mono unsaturated fats. Still researching, but I think there is definitely something good about adding fat to our diet. I look forward to hearing others' thoughts and experiences. There is a great site called RULE ME and it helps tyou figure out what you need to be successful. They have plenty of recipes too. My hubby tried the KETO diet thru a person who sets up everything and sends you recipes for your prep work. But he found he didn't care for most of the things she had sent him recipes for. Also we found it to be really expensive to follow. Because we had WLS we thought it was going to help us. But the truth is you can figure out what you need to eat and get tons of recipes on PINTREST. Plus the recipes that were sent to us I had to go online to look up every one of them. They sent the sites email for you to look up. I really think there are many many programs online to help us with that. Hope everything goes ok for you.
