Hey there potential band peeps. I really thought it was going to be more of a struggle to get approved. I have United Healthcare Choice through my 1st employer, and I am glad we have the program "Optum Health" available for possible WLS(weight loss surgery) potentials. I have always been a pretty thick girl, not skinny but not overweight. I started struggling after 3rd son 9 years ago, and I have ballooned to this weight. I have been told numerous times I don't look like I am in the 200's more like 189. I am MORBID OBESE with BMI 41.I so anxious to start fresh and have a healthy lifestyle in the months/years to come. I have tried many diets and always failed. I lose a few pounds only to get stuck at 190 ALL THE TIME. I am scheduled for surgery 5/24/10!!I had to get cleared through a 45 min psychology eval (paid cash $165) I did bill insurance 1500 form and GOT REIMBURSED $140! Beware there is crazy psycologists who will tell you it would take a few visits or testing one word"CRAZY" it took me 45 min. I loved the Dr. he is in Del Mar, CA DR. Khan1 Diet/Nutrition class 1 Orientation to know what to expect2 year WT. history submitted to insurance. I sent my 24 gym membership from 6-8 years ago, Personal Trainer info, New Gym membership, ER paperwork stating my WT. and job mandatory physical's with weight. I was assigned a case worker who approved me in 1 WEEK!GOOD LUCK TO ALL
