My family doctor referred me for surgery on November 2008.  I have an HMO insurance and before surgery was required to undergo a psychological evaluation.  The psychologist report suggested in the summary I attend outpatinet psychotherapy to rule out any underlying depression that he was unable to detect, and also attend nutritional counseling.  Referral was denied based on his recommendations to have psychotherapy and nutritional counseling.  My family doctor appealed the denial stating I have a good knowledge of nutritional needs after surgery and psychotherapy wsa not necessary because I am not depressed.  Health Plus required a release letter form the psychologist and unless I went to the therapy and nutritional counseling he would not provide an addendum.  So after attending psychotherapy and nutritional counseling for 6 months the psychologist added an addendum to my evaluation that stated I completed all his requirements and would be a good candidate for surgery.  Referral approved November 2009.  I had a meeting with the nurse and dietitian December 2009 and the next available appointment with the doctor was January 14, 2010.  I seen Dr. Ringley on January 14, 2010 and had my surgery on February 18, 2010.
