My Name is Sheryl Hazuka
I have been waiting for 3 years for my insurance to accept that I do need help... I finally made the call to Blue cross and blue shield and asked them if I had benefits for the sugical treatment of morbid obsesity.
The answere was YES!!! I was so happy that I burst into tears. I then contacted Alegent Health with the good news.. My first appointment was Wednesday July 11, 2012. Now granted this was a Weight loss surgery Seminar, a sminar that was about to change my life forever.
My second appointment was scheduled for Weight Management, with the dietitian.. that information was exciting and interesting news to learn. My third appointment was scheduled Sept 11, 2012 for a psychiartic consultation. That went well and fast. The doctor told me that I was a percect canidate for the surgery.

I am now awaiting for my first letter from blue cross and blue sheild.. to see if I am accepted. More details to follow..
