I technically still have BCBS but we're in the middle of switching to Aetna. What I can tell you that I found out was with my plan the surgery was covered at 90% after a $250 deductible and then I had an out of pocket expense of $1,000. They had requirements that I had to meet before the surgery which were a 6 month (consecutive) physician supervised diet plan, psychological evaluation and meeting with the dietician.
