Would there be a Co-Pay for Bariatric Surgery with Medicare and Medicaid?

I was just wondering, I am on disability I live in Michigan and I am interested in getting the Bariatric Surgery done. Only thing is though the place I am looking at going to says you have to have all co-pays or deductibles paid in full prior to the surgery. Does anyone know if there would be a co-pay or deductible from Medicare (Primary) and Medicaid (Secondary)? Gastric Surgeon New jersey
