    Question about fats.

    Okay so I am still so terrible with this stuff but my dietician makes it seem so much more confusing and I really need to understand this so I can get it right this time. I asked her what my requirements would be post SADI surgery and she said 1000-1200 calories 90g protein 90 carbs (45 fruit and veggies and 45 from starch), and 15g fat. I am trying to eat similar to that now to see what it’s like and I am logging my food. I figured my portions will be a little larger so I won’t be able to hit the exact goals but wanted to see where I am. This morning I did great with my protein iced coffee which is just a caramel shake with some coffee mixed in. Low calorie and carb and high protein. This afternoon however I thought I would have 2 scrambled eggs and one Turkey sausage Patty. The calories and carbs are fine but it seems like Either that fat goal is unrealistic or the protein goal is from normal food if there is really 15g of fat in two scrambled eggs and only like 13 grams of protein? Even if I forget the 7g of fat leaving out the Turkey sausage just two eggs is too much fat for one meal and reduces the protein. I hate egg whites but assumed two scrambled eggs would be okay. I know post surgery I will not eat two whole eggs which will reduce the fat some but also the protein. Could i add protein powder to the egg or something? Maybe an egg and a yogurt would be a better option? Or Do I have to just suck it up and eat the egg whites. She also said something about three teaspoons of “healthy fat” and I’m not sure if that’s in addition to the 15g of mono and polyunsaturated fat or if 15 is the total which may be where I’m getting confused. I just emailed her asking for clarification but last time it took her like two weeks to get back to me so I figured I’d see if anyone else can make sense of this.

    Would it make more sense if the fat was supposed to be 15g per meal? It seemed like she was calculating it from three meals when she was talking to me because she was adding the carbs out loud. That seems like a mistake that could be easily made and that would be closer to what the DS vets have said. Still not quite as high but it is a little different surgery than DS. Plus adding 30 more grams of fat brings the calories to 1,125 if I’m understanding and calculating that correctly which falls within that range that she gave me.

    No, she didn’t say anything beyond that. Honestly I’m not sure if it’s because she doesn’t care or more because she just doesn’t know. Which is extra frustrating since I called her office and specifically asked if she was prepared to help with this particular surgery and they had to get back to me so they asked her. I basically feel like I wasted $100 at this point. On top of that the vets on here that had DS which is the closest thing are telling me that she is way off on carbs and fat and that DS doesn’t really pay attention to calories at all. I have an appt with the surgeon tomorrow and I already have a list of questions about the surgery itself so I’m going to ask him if I can make another appointment with him to talk about nutrition or if he has another nutritionist he can reccomend because this is getting to be ridiculous.

    Vitamins

    I wonder if you would have better luck bringing both bottles or a printout of what’s in them with you to your appointment so they can “compare” the ingredients for you and tell you if it is “possible” for you to take the same ones or if what the surgeon said is a better plan. I have tried comparing vitamins before to find cheaper bariatric ones and its apples and oranges. can only imagine how differnt prenatal ones would be from bariatric. I feel like a doctor would know what’s really important and what’s not better than we would.

    Vsg 2017 and PREGNANT!

    I have seen others on here who have become pregnant, some within the first year, who were just fine. I suggest that you make appointments with your surgeon and nutritionist asap (along with OB of course). The surgeon will run labs to see if you are low on anything from not taking your bariatric vitamins and the nutritionist can tell you what you should be eating that’s best for you and baby.
  6. Personally, I would put a call into your surgeons office just to be certain. Ask to leave a message for the NP or PA. They usually return calls quicker and If they don’t know they will ask the Dr anyways.

    Suggestion

    Please add the option to list a second surgery to our profile if possible. I’m thinking it’s kinda confusing to people that I am asking questions about pre op when I had surgery three years ago. Also please add the SADI surgery to the list of surgeries as well.
  8. I am contemplating revision and have to choose between bypass and Sadi so I am curious to follow this as well. Not sure if your surgeon does the Sadi revision but I was told that the weight loss should be a little better than bypass. There are drawbacks too though with more absorption issues it can cause more malnutrition than the bypass and also bathroom issues seem to be a common complaint. I was told with proper nutrition that both of these are less of an issue.
  9. Even though I was cleared of any eating disorders by the psychiatrist prior to my sleeve surgery I am fairly confident that I meet the criteria for NES. According to my research the treatments are antidepressants which I am already on and Cognitive Behavioral Therapy. I asked my therapist who uses a different therapy approach what that means and she said CBT is about changing behaviors and she suggested that when I wake up wanting to eat that I crochet instead (replacing an undesired behavior with a healthier one). Well the other day I had to fast for blood work and I usually put a towel or something through the handles on the fridge to remind me not to eat or drink anything other than water and I thought to myself if this works for fasting why couldn’t I just do it every night. So I made sure to eat a high protein yogurt before bed so I know it’s not actual hunger then combined the visual reminder with the alternate behavior and I made it through the last two nights without eating any snacks but it wasn’t easy. I woke up and walked to the fridge several times and had a number of arguments with myself and even tried to rationalize that it was okay to just have one snack because it’s still less than normal. My husband said the good voice triumphed over evil for now. 😂 Anyways, I am curious to know if it’s that “simple” of a fix for lack of better word. Has anyone else had experience with NES? I am curious to know if this substitute behavior thing really works and if so how long it will take until the alternate behavior becomes my new habit. (How many more late night arguments am I going to have with myself)?

    Thanks. I did get a reply from someone with DS and she said that she thinks that my dietician is wrong about the fat and the carbs. For her those Carbs are too high and the fat is too low (which makes sense if the absorption is less of everything except for carbs with that surgery. I thought it was pretty crazy for two scrambled eggs to be all the fat I could have for the whole day with any surgery. I have an appointment with the surgeon on Monday and I’m going to ask him if I can maybe set an appt to talk to him about nutrition or if he has another suggestion since that dietician seems to be misinformed when it comes to this surgery as well. . If she doesn’t even know the general guidelines she certainly isn’t going to be able to answer questions about specific foods. I did see some information online about the post op diet from a bariatric center in Mexico that goes over specific foods that are more or less tolerated which I will try to find again ask him to review but that’s the most information I found and it doesn’t mention macros at all.

    I’m trying my best to mimic what I will be eating post surgery and today I logged my food and just mostly paid attention to carbs and protein. I logged but otherwise ignored the fat for now since I thought she was wrong. In the morning I had a protein shake with coffee. For lunch I had two scrambled eggs with Turkey sausage, morning Snack was a dannon light and fit yogurt, dinner was taco salad with pepper, onion, 1/3 cup cheese, 1/4 avacado and 4 oz lean taco meat. Then a chobani flip yogurt for snack which is the worst thing of the day because of that little bit of chocolate and peanut butter I guess. Anyways i added that to be 61g carbs (23 was that yogurt), 54.4g fat, and 105.5g protein. I am thinking I was close. Maybe adding a little more healthy fat such as more avocado or cheese or cooking with olive oil instead of pam spray and then switching out that evening yogurt with a lower carb one and I would’ve been pretty close to you. I realize as I’m going over this that I’m going to have to tweak this quite a bit for a smaller pouch if my surgeon re-sleeves me which I hope he does. This is going to take some time to figure out. Thank you so much for all of your help.

  13. Hummm. I was concerned about the bathroom issues too but was told that it’s mostly only when eating bad or too much. I did read on one site that you can take immodium at night to help prevent the early morning bathroom issues which I assumed based on that it was a more commom thing. I did read that it’s worse right after surgery and usually lessens with time. I have my fingers crossed for you it gets better.

    So I finally got an email back from My dietician and the macros she gave me for diet post Sadi is 1000-1200 calories 90g protein 90g carbs and 15 grams of fat. That doesn’t seem to be anything close to yours. I am so confused and wondering if she is wrong. I feel like if I don’t absorb a descent pirtion of that I would be malnourished.

    Counting Carbs or Net Carbs

    The thing is that they assume that you can only eat so much so if you prioritize protein then veggies then you should only be able to eat a tiny bit of healthy starches, if any, and based on that theory everyone will be okay. When I had Sleeve three years ago I swear mine was left larger than most though because I could drink and eat more from day one than they said I should. I always said that I know I had surgery because I could no longer get my moneys worth at an All you can eat buffet but I could still eat quite a bit. When I was back to “regular” portions I hit all my protein goals then ate my veggies like suggested but still had room for more carbs than I should be eating. I did great in the beginning when I was told how many ounces I should be eating and I wasn’t hungry so I just stopped myself there but later on I think I needed more guidance than just calories and what to prioritize. I know I was an exception to the rule though. When I went back to discuss revision with another dr he said not that I’m suggesting it but if you were eating a sub how many inches could you eat and when I said 5 or 6 easily he said that I should not be able to eat that much. I believe that is why things went wrong for me. I reached my first easy goal I set to encourage myself but never made it to my ideal body weight. Then a few month later when actual hunger came back and I ate until I was full I started to gain even with the healthy options because of my portion sizes. Admittedly if I had not been frustrated and basically gave up at some point I probably would not have gained ALL my weight back and I would still be healthier now than when I started, but I know where things began to go wrong so this time I asked repeatedly until I got even more guidance from the dietician.

    I wonder if it would be cheaper to just mail a box ahead to the hotel with your snacks and supplements than to have to check An extra bag if you are like me and won’t have the room for all that. 😂. I would’ve suggested Amazon if it was within the US but I don’t know the cost or shipping time for international orders. I know that I would prefer to just have it with me or there waiting for me instead of wasting vacation time shopping for necessities. Plus I assume the brands we are familiar with would be expensive to buy there.

    I had this same question. I talked to the dietician and forgot to ask her about that too. I am contemplating the Sadi which is low carb. She told me to get 45 from starch and 45 from fruits and veggies then says 1/4 to 1/2 cup of each per meal but didn’t specify net or total carbs. I’m thinking my requirement is total carbs though because if 1/2 cup of pineapple has 15g which is a higher carb fruit so even that three times a day is only 45 total grams of carbs. If it was net carbs it would mean I should be able to have more than 1/2 cup I would think. I think where it really matters is on processed stuff. Hopefully I remember to ask the surgeon on Monday.
  19. After googling I guess this may be a difficult questions since it’s so specific to the patient but Does anyone remember what labs they ordered for preop? Was it just the normal CBC? Or was it all the vitamins and extra stuff. I have to go back to my PCP again tomorrow morning because they couldn’t find a vein when they tried on Tuesday and as I sit here I’m thinking that probably in another couple weeks I am going to have to go through all this again. If I had thought of it earlier I would’ve called but I have to be there at 8 am and if possible I will ask them to add whatever the surgeon will likely require.

    I think you’re right after they try and miss it’s like they are nervous and they are less likely to get it after that. I don’t think I’d let them try 8 times unless it was life or death. A lady today that used to work at hospital said they have topical stuff and a shot they can use to numb the area but you have to ask for it. That doesn’t help with the bruising or the pain after it wears off though

    Yea they just had to do the wrist on me this morning. Wasn’t pleasant to say the least. I am sorta dreading surgery day when, like you said, I will actually be a little dehydrated because they had a hard time even after me forcing down three extra bottles of water this time. I don’t recall it being an issue for my sleeve. I guess I had someone good. I am starting to think that’s what it all boils down to. Most anyone can get it with good veins but anything a little harder takes someone with more experience. I am starting to develop an anxiety to needles that I never had since this vein issue. I used to be fine as long as I didn’t look at it but now they always ask me if I’m okay. Idk if it’s just because I look in distress or if they just know they are torturing me. 😂

    Thank you all for your replies. It sounds like I’m going to have to go back since it’s so different for everyone. I was hoping it was just a couple extra tests I could ask the NP to add on easily. It’s always such a big deal cause I had a lymph node biopsy under one arm which rules that out and my veins in the other apparently Aren’t great anymore. sorta wondered if it was just the girl at my primary dr at first because the issue started when I started doing it there but I can’t see any veins in that arm anymore either. She had someone else helping her today and between the two of them they found one (the third try) and told me to remember the location of it cause it was a good one they said. My friend told me that if I go to hospital they have actual phlebotomist that know stuff that the nurses do not though so if he does order more I will just ask him to send the order to the hospital this time. I am curious to know if this hard to find veins issue coorelates with weight. I am hoping maybe it will get better after surgery. Wishful thinking probably.

    Constipation

    Do you spread out the calcium into a few doses separate from the multivitamin? That’s the only thing I can think of to help. I believe calcium is known to cause constipation as well as the iron so if your getting both at the same time it could be too much.
  25. I am interested in hearing about this too. I’m curious to know if they do the same thing for sleeve??
