I was supposed to have mine this past week, but had to reschedule it. I had to fill out about 19 pages of paperwork. Lots of questions about your behavior towards food. Your history of being obese. I was told it’s a one hour interview then another test/ questionnaire to do after. Good luck. Mine is rescheduled for April 3rd I think. -
