For My Own Info, and yours....
04/11/17-Went to PCP for first consideration, she agreed and put in referral for surgery to insurance.
04/25/17-PCP called, they got the letter back from insurance needing more info. I went in that same day to do the paperwork and answer questions for them, like diagnoses and what type of diets tried before. Blood work ordered and she wanted X-Ray of my knees and hips of arthritis to have as well.
Blood work done at PCP, A1C done, negative.
04/26/17-X-Rays done at Holy Family Inland Imaging.
