What to take to surgical consult?

I have been cleared through my insurance company for my first surgical consult for gastric bypass. I went through 6 months of testing with Kaiser in advance of this to get the referral. Should I that that stuff with me? -

I too am about three months into the process. I've seen my surgeon once. Have about two more months of nutritionist visits etc. Does anyone have any tips for me?
