Sent by mistake before I was finished and can't figure out how to Delete! Just Google the requirements. Your medical center will have its own requirements too. I think the psychological visit was part of my hospital program. After I did the requirements, the insurance approved all in a day or two. It was great. Paid for virtually everything.
