    Hi!!! Here you go! A. The beneficiary must be between 18 and 65 years of age. B. The beneficiary has a documented body-mass index >35 and has at least one co- morbidity related to obesity. C. The beneficiary must be free of endocrine disease as supported by an endocrine study consisting of a T3, T4, blood sugar and a 17-Keto Steroid or Plasma Cortisol. D. Under the supervision of a physician, the beneficiary has made at least one documented attempt to lose weight in the past. The medically supervised weight loss attempt(s) as defined above must have been at least six months in duration. E. Medical and psychiatric contraindications to the surgical procedure have been ruled out (and referrals made as necessary) 1. A complete history and physical, documenting the beneficiaries: a. Height, Weight, and BMI; b. The exclusion or diagnosis of genetic or syndromic obesity, such as Prader-Willi Syndrome 2. A psychiatric evaluation no more than three months prior to requesting authorization. The evaluation should address the following: a. Ability to provide, without coercion, informed consent; b.     I have to say that this is not always true for all managed care programs. I have Caresource in Ohio. Ohio Medicaid requires 6 months of doctor supervised diet, and comorbidities if your BMI is below 40. Caresource, however, requires 9 months of supervised diet, and will only approve you with no comorbidities if your BMI is over 50. BMI 45-50 requires one comorbidity, 35-45 requires two or more. The bottom line is that your Managed Care Plan dictates what you are required to do, and that it is always a good idea to get those requirements in writing if at all possible.     I appreciate it. I had been thinking about getting the sleeve for years, but unfortunately I lived in Arkansas and I didnt meet requirements. I'm actually in Tacoma area. I have Molina-Apple Health and trying to get a feel of what it does and doesn't cover. I did the 6 mos. Diet, Psych eval, TSH..you name it. (back in AR ) found out a simple BMI of 40 wasn't enough no comorbidities. I was devastated because it was time consuming and lack of navigation from the surgeons office. I'm awaiting my appt. currently to pitch the idea to my new PCP here in WA. I will obviously have to go through the UW-CENTER of Excellence ????. What did you need to be approved? Any other health conditions? They require years of proof of obesity?( 5 years ) I appreciate you reaching out..there aren't many with our insurance on here that I can find. So thanks.. !!!!     There's a letter sent from the hospital that's I'm planning on going to stating the requirements by state law. And it says 35 with top 4 Co Morbid or rare comorbid. But when I call Molina they say there are three ways to get approved. BMI 40,BMI 35 with comorbidities,or 30-34 with DM II or metabolic syndrome. The only way to understand i guess is to just allow the process and cut out the guess work.. I hate doing stuff for nothing. Ya know? Sent from my SM-G900T using the BariatricPal App

    Update: I'm using Molina BTW I've heard conflicting info from everyone about what Molina WILL cover. I called Rockwood in Spokane and the representative said that with Molina, all you need is an BMI of 40.. that's enough. She explained that they do not take plain ole' medicaid..only Molina medicaid. So, unconvinced, I called Apple Health Molina and was actually told by a representative there (including the Lead Rep) That there is 3 different ways to get approved. A. BMI of 40 or over 100kg Or B. BMI of 35 or higher with one or more comorbidities. Arthritis sleep Apnea High Blood Pressure High Cholesterol Type 2 Diabetes Venous Stasis Disease (Collection of Blood in the Lower Limbs) Congestive Heart Failure Fatty Liver Syndrome Gall Bladder Disease Depression Stroke or Stroke Risk GERD (Acid Reflux Disease) Or C. BMI 30-35 with DM II or metabolic syndrome. I asked the representative if I could have the bariatric program requirements in a file or letter.. she says that it couldn't be given to me as its used for them to review each case..and that I could obtain it through my PCP.
