Hi! It was easy to get approved, so long as you Dr. addresses each and every thing on that checklist. My Dr. did not on the first submission and I was denied. My Dr. resubmitted with the correct info being addressed and I was approved within 3 days. Good luck to you! Let me know if you have other questions.
