There is nothing on the website. The best thing is to call every couple of days. I was approved in 4 days. My best friend was able to find out if they didn't have something they needed before the drs office my calling so often. It still took her 2-3 months for approval with the same insurance, but different employer. Good luck.

My PCP submitted it to my surgeon. My PCP included my weight over the past 5 yrs, any weight plans I had tried over the years, and any other issues I was having because of my weight. Don't give up! -

I have UHC Choice Plus and didn't have the five years of a BMI over 40. I think it was three of the five though. I was approved in 4 days. Good luck! -
First, Congrats to all! I have UHC Choice Plus also and was approved in 4 days. I had to submit a 5 yr weight history, PCP letter of approval/support with any and all information to help my case, my surgeon office required a psych evaluation before submitting, I think that is it. It was just done the first of this year and I have forgotten already. I have been banded now for almost six weeks. I love it already. Good luck!

Yes. I count myself as one of the lucky ones. My best friend has UHC also and with are with the same dr and it took her 2 or 2 1/2 months for her approval. A lot of problems with the office not sending what they had. Went to the wrong fax number twice, then missing something, etc. She was finally approved and was just banded yesterday. Our goal when we went out to do this was to have it on the same day. We ending up being two weeks apart. I have heard that UHC is still one of the fastest ones.

No. I am in VA and the insurance is with my dh's job.

The requirements may come down to the doctors office also. I had to have the first seminar, nut seminar, pysch evaluation down before they would submit to UHC Choice Plus. Mine required a BMI 40+, 5 yr weight history, and a letter from my PCP. I was approved in 4 days. Call them and ask what they need. All are different. Good luck!

Hi Guys, Do any of you have a flutter feeling around your port cut? I would say there is a baby in there if I didn't know better. I can't feel my port so I don't have any idea what this is.

I was not morbidly obese all 5 years. It depends on weight what they submitted if it was two or three years out of the 5. Good Luck!

I had to do two weeks, but ate what is listed above and was able to have a sandwich with 4 oz of lean meat and dressings. My Aunt Flow came in my second week on the diet, but I lost 12 1/2 lbs all together. -
Hey! I am doing pretty good. I feel like the swelling has gone down some. I am stiff and a little sore. I still have some gas pain in my shoulders and in the front between my breast. I finally got the heating pad out and that seems to be help. I have need the pain medicine. I am still having a hard time getting all of the liquids and Protein in. I have been taking short walks. I did walk around the block yesterday very slow. I am so scared to push the liquids. I always ate just to eat and I know I don't want to eat. I had my first tummy rumble this my morning to actually want something to eat (drink). It is so nice to have other people at the same stage you are for feedback.

I have UHC. I didn't have to do a 6 month diet. I submitted a 5 yr weight history and have a BMI over 40. I got my weight from my PCP and my OB/GYN. I have the beginnings of a couple of comorbidities. I was approved in four days. Call your insurance company and find out what they require. All plans are different. Good Luck!

I am done on this end also. I came home today. I am feeling pretty good. Some gas pain in the shoulders, but my stomach is okay. I have numing medicine going to port cut. I will run out on Sunday and I have to remove it. Did any of you get this also? Congrats to all the 20th banders.

I would call the insurance company by the middle of next week to make sure they reached the information. After they tell you they have received it, call about a week later to check the status. If not approved yet, I would call every couple of days or everyday if you want. Good Luck!

I would call. I had to talk with the Care Coordination Dept with my insurance UHC. They can tell you if the papers have been received and where the process is. Go for it! Oh, I called everyday day.

From what I understand, if the company you work for as it as an exclusion there is no turning it over. I don't think an appeal will work. You can try going to your HR dept and see if they can do something. Maybe someone else will have something different to say. Good luck! -

I have UHC and my BMI was not over 40 for all five years. It has been over 40 for the past two years, but between 35-39 for the over three. I got approved in four days. Done give up! -
