    hi, did you go to the site and fill out the information? After that is when you cut and paste. I just did mine today and it worked. I hope that helps if not tell me what you did try and I will ask the resident computer geek. Good luck with your surgery!

    Good Morning, There is a fill center USA in GA I have used the fill center in FL. I looked on the data base we keep for patients and we have referred several to them and I've received no bad reports. Hope it helps. fillcenterusa.com
