Not sure which horizon bcbsnj plan you use but I am using seth kipnis through jsumc... There is also a really great group affiliated with meridian who work through bayshore and two other affiliated hospials (I think) -

I had no problems. I attended the seminar in December and am scheduled for march 2nd. I had to take care of a lot of prerequisites in that time and had to show proof of prior weight loss attempts. -

Finally got approval from the insurance company today so my surgery date is confirmed for March 2nd!
