Example of Appeal Letter (UHC-Choice Plus)
Jenn B. replied to Cindy_Wisc's topic in Insurance & Financing
Omg this thread is making me nervous I have UHC choice plus they my husbands company and my surgeons office said they love working with UHC. Easiest approval but according to this UHC has been a nightmare. I'm just starting my journey so now I'm feeling a little paranoid they could deny me. -
That's sweet. Mine does talk about the 6 months but dosnt say consecutive months so I'm hoping my trips to Ky every other month will work.

Did you have to do the six month diet plan

And best of luck to you dear!!

Just called to remind Angela that I want to be put on the cancellation list but nothing yet. Keeping my fingers crossed I get in earlier. The sooner I can start the better!!!

I am also working with Dr. Custer's team. I have my first consultation on August 11th, so I am just in the starting phase of my journey, but am hopeful that with the insurance I have it will be a short and sweet process. I have met with Dr. Custer once or twice before and loved him, but until now never had the opportunity to get the surgery. I am looking into the gastric bypass RNY surgery vs. other options. I am hopeful that I can get in for my initial consultation early due to cancellations. Good luck in your journey with Dr. Custer.

Thank you both so much for your reply. This is a journey and one that I am looking forward to tackling. It's scary, exciting, and nerve racking but something I know I can do. I look forward to hearing about your surgery and success!

Hello I just started my weight loss journey and I have UHC Choice Plus thru my husbands employer Saia. I talked to the insurance rep and so did the bariatric nurse and YES they do cover the surgery. I have my consultation on August 11th, but hoping for sooner depending on cancellations. I meet the criteria as far as my BMI goes, but I am a little confused regarding the diet plan. Do I have to show 5 years of weight history or do I have to do the 6 months of doctor monitored diet and exercise. Seems like that is a common questions among everyone. I automatically have to do a 3 month program thru my surgeon, but not sure about the rest. When I asked the insurance rep if there was any additional requirements she said no, but when I look on UHC's sight it says 5 years weight history and 6 months doctor monitored diet plan. So IDK what is what???

Experiences with Phentermine?
Jenn B. replied to RellaBelle's topic in GLP-1 & Other Weight Loss Medications (NEW!)
I have used for a long time in and off. At first your not hungry and totally wired but after being on it for a while it starts to lose that effect. I have always gained the weight back and then some. If you have tried loosing weight and are struggling between surgery and using then I would recommend surgery. -
You can google bariatric doctors in your area but you may need a specialist referral depending on your insurance.i would do my own research before I decided on a doctor. Check credentials and ask lots of questions.
-
I would check with Molina and see what the qualifications are. If you have to do six months then yes I would start now and make sure it's doctor monitored or it won't count.
-
If Molina covers it and your are doing all the requirements then I don't see why they would deny you.

Thank you. I can't wait to meet all of them!!!

Misty- I have United health care thru my husband work. I also have to do six months but I have been seeing a diet doctor every other month for the last fourteen so I'm hoping insurance will allow that as some of my time. Regardless I have to go thru my surgeons program for three months. I made my notebook up last night and started writing down some questions I have. Also received my health packet from clinic and filled it out yesterday. Hoping and praying they can get me in sooner than my scheduled appointment. The sooner the better. I'm so excited for you and thank you for your support. Good luck to you!!

Good for you and congratulations. Please keep us posted on your progress.

Misty- thank you for the information. I will most certainly put a folder/binder together. Wow, July 15th isn't that far away. Congrats!!! What surgery are you having and what criteria did your insurance have? I have UHC choice plus so I'm hoping the three month program will be sufficient and not have to do the six months. I would get the surgery today if I could. How long has your process taken?

Thank you for your input. I've learned almost everything I can over the last 7 years of research. I've teetered between lap band and gastric bypass but I think I'm just going to go for it and do the gastric bypass. Go big or go home right...lol. I'm nervous about quitting smoking and don't know if I have the will power but will do whatever needed to have the surgery. Although I might lose my sanity in the process...I think giving up diet Pepsi might also be the death of me. I just wondered about the insurance because it doesn't state anything about doing six months of a doctor supervised weight loss program. Just wondering if the three months thru the program will be enough?

Hello Groveport Ohio here. I meet with Dr. Custer on August 11th but hoping to have my consultation earlier due to cancellations. I am so excited as I have been wanting this surgery for about 7yrs. I have UHC choice plus plan. I meet the criteria but still have to go thru the 3 month program through the doctor. Anyone else use Dr. Custer? How did you like him and the facility? Did anyone have UHC? How was the approval process. Looking forward to any advice or feedback.
