I have anthem BCBS through my husbands employer. After my deductible of $300 I pay 20% of the negotiated rate for the surgery (co-insurance) until I reach my maximum out of pocket which is $2,000 dollars. after I have reached $2,000 BCBS covers all covered costs at 100%. I would call your plan provider in the AM to ask what is your deductible, out of pocket maximum, and co-insurance amount. This will better help you understand and get an estimate of your costs per calendar year.

Good luck! I am an ER/PACU Nurse also, I am just worried about dehydration issues at work. I have to start really getting used to drinking fluids. I am taking off 4-6 weeks. I am afraid of returning to work too soon, then having to take time off again because I returned too soon. I am not worried about PACU days it's the ER days that scare me -

Yeah I am new also, there are internet Trolls everywhere. I HATE the ones that talk about going to McDonalds or some other FF joint. What's scary are the number of positive words of encouragement they get, It makes me wonder Really who is the sick one. -

On my Quest to getting a gastric sleeve, sometimes I have felt lost and confuse with Sooooo much conflicting information. So I went digging and have found some documents that are plan specific to Anthem BCBS of CA. Remember each individual employer may add or eliminate requirements from these guidelines(also there are different requirements between PPO and H M O plans). Just when my fears are subsiding I hear another story, Yikes!! I called my carrier and they told me the information I needed, my issue was with the physicians office. One of the coordinators told me All plans have a MD supervised weight loss waiting period. As if I weren't a wreck already my carrier doesn't require it, I told her I would not come to see the surgeon until she knew this for sure. I couldn't imagine going through all of this for nothing. SCARY I have to say the waiting and not knowing of this process is almost too much to bear. I wish you good luck on your Journey! My appointment with the Surgeon is Monday my knees are shaking as I sit here. Wish me luck -

I found a great link that breaks down the requirements, of most national Insurance carriers. I hope this helps those of us who are waiting, I found my carrier and the requirements are indeed correct. I can't say that this site is 100% accurate I just stumbled upon it, but since they are providing info on MD's that do surgery I would imagine they are legitimate. They provide online live chat also. I just hope this helps relieve some anxiety while you are waiting, or it provides you with additional information so you can have it prior to submitting to your insurance carrier. Yes it is always best to call your carrier, however I have seen here on this site people trying to contact their carrier and still not getting information (as if the insurance they are paying for is a Top Secret document) Yes different employers can modify from the carriers options. At least this service can help if anyone is interested. -

My appointment is on the 7th, I can't wait!!! I have orientation fist, then the appointment with the Surgeon in the afternoon. Come Sunday morning I will be a wreck waiting for Monday. -

I don't have a time requirement, I just have to have one Nut and one Psych visit. The office coordinator said when I came in on the 7th, that's my first Md visit they then submit the paperwork to the Insurance. She also said that approval with Anthem BCBS is very fast, she said after they receive the go ahead I can schedule the date. I have been exhausted and nervously excited just waiting to get this over with! It's nice to see you are at the end of your preop phase. You have Real patience in regards to the 6 months. I would be pulling my teeth out by now. Well good luck can't wait to hear your post op stories. -
I was wondering how detailed the diet history sheets have to be, I have well over 5 years of a BMI over 35 with 3 comorbidities. However with the diet history I can't remember the exact years and weight loss and gains from all of the Non-Physician supervised diets. I do have medical records from Physician Supervised weight loss however. Please can someone give me any advice on filling this out. I would really appreciate the help.

It's for identification purposes,weight fluctuates but for most people their weight is generally stable. Suppose you were in an accident and multiple people were in the car, your ID would help them identify you. There are soooo many other scenarios where this information would be helpful. -

I am glad my state doesn't require weight, saves me from telling a Lie!! Can't wait for the day when I no longer have to tell the nurse in my doctors office, to not to say my weight out loud cause I don't want my husband to hear. -

I have very long and thick Dreadlocks, it won't be so cute when they are falling on the floor . Hey but to return to my former self, it will be worth every lock that falls on the floor. I can always buy a wig, as my mom says it's just hair and it grows back. I thought about getting the surgery years ago but my husband didn't want me to get it, he kept saying just try diet and exercise. (he's 130lbs easy for him to say) He kept saying "I still love you regardless of what you weigh" and don't do elective surgeries "you could die".The problem is I don't love my outer self,as far as diets when wasn't I on one and I am going to die from my comorbidities. Now that he see's my health and my energy taking a nose dive, and these issues could become serious he has been 100% on board. Also my youngest has never seen me thin, I don't want him to ever feel embarrassed of me because kids do talk. Again I can't say enough how Great you look congratulations to you, it wasn't the surgery alone that got you there it was your hard work!
