    Big Decision

I am one of the *lucky* that have dumping with the sleeve. I can't eat very sweet stuff like ice cream or cheesecake or other high sugar foods. About 1 bite is all I can tolerate. RNY slight edge for diabetics. If you are pre diabetic sleeve may be enough. RNY still gold standard. Whatever choice you make you can and will be successful. Personally I did I not have comorbidities and saw no reason to have the re-routing part done.

Beef is higher in calories and so that is why. If you pick beef, use the leanest cut possible. I recommend you be open to new foods as you go thru the process. I eat all kinds of stuff I never did I before. My doctor had a pretty simple pre op to follow and I knew I had to do it to be in the best shape for surgery so I was very compliant. Depending on how big a person was especially in the belly area they do it for 1 or 2 weeks. I was told 1 week and did it for 10 days. I had to substitute a Protein shake for 2 meals. I used premiere protein or GNC Lean shake ready made. Then I could have 1 "healthy" meal with upto 4oz lean meat or chicken or fish and non starch vegetables. I was also allowed 1 Greek yogurt or a ounce of cheese or a piece of fruit for a snack. I could have 2 Snacks a day. Sometimes I had the Greek yogurt for Breakfast and the shake for a snack. After first couple days I did not even want 2 snacks as hunger was not too high probably because there was no bread, rice or grains to make me more hungry. Overall it was about 800 calories a day. I lost about 11 lbs in 10 days.

Would you do it again if you knew....?

I am 17 months out and I don't mourn food at all. Mostly because at this stage I can have pretty much anything. But do I have everything. No I don't. Do I have as much as before. Nope. Can I get 3 courses at a restaurant. Nope. But can I have a slice of pizza. Yes did that Saturday. Can I have a cheeseburger. Yes just had half an angus burger I made at home on a slice of low carb bread. It was delish. Some days I can eat a whole one. Had a couple French fries too. The only thing I can't have is high sugar stuff like ice cream or cheese cake or other sweet things. Makes my stomach ache. A type of dumping for sleevers. I am glad I have it so I stay away from it. But can I have a cookie or brownie. Yes 1 only. So no, for me at this stage there is no need to mourn. Pretty much anything can be eaten. But do I choose to eat that junk. 95% of the time I do not. I fuel my body now and I want high quality fuel. Yes I do get stressed and I feel like I want to go binge but I physically cannot. I am so glad I cannot! So be patient. Life is pretty normal 17 months out for me. Still have to be a mindful eater but knowing I can have pretty much anything makes me choose the right thing most of the time and I am not tempted by the "forbidden" because for me nothing is forbidden, but I choose not to have it (95% of the time!)

    Personal trainer

After losing 100 lbs in 9 months I wants to get under the 200 pound mark. I signed up with a personal trainer for 2x week for 6 months. I just did not know what to do on my own. Pick have lost 26 more pounds and went from 36 to 24 percent body fat. It cost a ton of money but it got me where I am now. I just finished my 6 months and he set me up a plan to do on my own and I can review with him again for free in 6-8 weeks. I hope I don't screw this up on my own. It is really expensive but I wonder if I will have to go back again and consider it a necessity to stay healthly. I will give it a few months and then check. I really recommend it for a newbie though. It is an investment in your future health.

New Dating Site for Bariatric Patients!

You may have to expand it to non WLS big losers or even to any people looking to get control of their diets and bodies and health such as those fighting diabetes with diet, fitness. Although it would be nice to have enough bariatric patients the number of woman vs men is so high on the female side. Anyone who has lost a lot of weight or struggling with diet and exercise has the same issues with choosing the right foods, exercising etc even if no surgery. They just shouldn't join if they dont want to meet anyone who had WLS! It would sort of be a myfitnesspal for dating. Just a thought that you may need to think about in future...

    Exercise? Diet?

For the first 9 months after surgery my exercise was very basic. I walked 2 times a week, I took stairs instead of escalators. I parked farther away at a store. I tried to add everyday activity and make it part of my life. I did not want to get crazy on any exercise only to get bored of it or pick something too hard or that I did not like. I did not want it to be an obligation to exercise. I lost about 100 lbs in that time but was still over 200. I knew that to get the rest of e way and to ensure I kept muscle mass I would need to start an exercise program. I started to go to the gym. I paid a ton for personal training for 6 months. I started spin class 2 times a week. I now go to gym 4 times a week most of the time and still do 2 days of walking. I take Saturdays off! I did not burn out. I now enjoy he gym. On vacation I went biking every day (for fun!). I went from 50% body fat pre surgery to 36% after losing 100 pounds and now after the gym, it have lost 25 more in 6 months and my body fat is 24%. So in the beginning do Something. Stay active. As you lose you can add more exercise. Make it something you enjoy. Something fun. If you hate exercise now just start by walking a few minutes a day and challenge yourself to add a couple minutes a day. Your body needs to move even a little. It will make you feel great. I promise!
  7. You have a choice with your every behavior. It is in your control to succeed and make the choice that leads to success. With every bite you have a choice. With every food purchase you have a choice. Even after you buy it you have a choice whether to eat it. No one else makes that choice for you. The choice does not make you a bad, good, smart, or weak person. It is simply a choice. Guilt or regrets never got anyone anywhere. That is the past which can not change. Stay in the moment and do not worry about the future. The only thing to be concerned about is the choice you make right now and the future will take care of itself. Every success will build up and lead to more. A walk of a thousand miles or 20 lbs begins with a single step or more properly the choice to take that first step.

    Sleevers over 300lbs?

My losses also are 1-3 lbs a month near goal. I had to really push to get under 200 lbs including spending A LOT of money for personal training. That got me the last 27 lbs and down to 182ish. I am still not a normal BMI and may never be there. I have 9 lbs more to go. In fact I met my personal goal of 187 and am just going to see if I end up a bit lower by continuing this slow but sustained effort. Not sure if I ever will have plastics though my arms are like umbrellas for small children and my boobs like slinkies that have been over stretched. Oh well. I am 23% BF and for a 48 year old woman that is damn good. So be happy - keep striving if you want but I for one am happy and at a weight my 315 lb self never thought to see again.

Do you have a weight chart?

I am at goal and decided to show you several. 1 over the full 18 months. 1 for last 12, 6, and 3 months. I wanted to show all because it IS a bumpy ride so don't let the gains bum you out. I had surgery march 2013. Also I just got back from vacation so am up 4 lbs but I am still under goal and I know I will take that back off. These are screen shots from myfitnesspal. I also track in excel.

Husband says no to me having surgery!

60% is the average amount of excess weight loss not the success rate. so if you are average and 100 lbs overweight you lose 60 pounds ON AVERAGE. someone as hardworking and diligent as you will blow right past average! I have lost 97% of my excess weight and I plan on getting to 100% with diet alone the percentage lost by the time people gain it back is much much worse.

Question for those that met their goals...

Took me 16 months to reach my personal goal. Lost most in first 9 months (90lbs) then took me 7 months to lose 30 more. So don't compare. It's not a race and there is no finish. Even though I am "at goal" every day is pretty much like the losing phase but with some more calories allowed. I will always need to be vigilant.

I thought I was (fill in the blank), but really, I was just fat.

I thought my tanita scale was broken because it stopped working and when it did it said I was 55% BF. Turns out I was over the 305 limit and i was just fat. now I am 182 and 26% BF.

    Stalled :(

I have to share that once I limited my sugar grams to less than 25g a day weight just comes off. Slowly but off. I allow myself 50 total sugar grams to account for sugar in Greek yogurt and any milk sugars. I use dannon Greek 80 calorie yogurt and mix it with fage to get tons of Protein. I strive for at least 80g protein a day. I also added strength training so built muscle. this has gotten me too the finish. I lost 110 lbs in 9 months and this change got me the next 25 in 6 more months. I don't worry about fat or other carbs too much. my diet is usually 45-50% fat a day. my losing calories were 1300 a day. I am 5-10 but I think it is best for most people to eat at least 1200 to avoid deficiencies. just my opinion though. good luck.

Will My Stomach Stretch? Interesting information

Sounds like you are all pretty early out when the stomach is still swollen. It takes about 6 weeks for that to go down and then over time you will develop more capacity. By a few months liquids are not restricted at all so watch those liquid calories. Liquids will not stretch the sleeve. The stomach will stretch somewhat over time but most of the part that was stretchy was removed so you will never be able to eat the volume of solid food that you did before. I remember in the beginning eating solid meat and chewing forever and praying for the day when it would not hurt to eat after 2 bites. But those are the days when you are losing the most so don't wish it away. In time you will be able to eat a small entree at a restaurant and feel full but not uncomfortable when eating enough to not feel hungry. I am 16 months out and can eat everything but portions are what they should be not enormous like my old life. I can eat 4 ounces of steak and some broccoli etc when I got out to dinner. My daughter is happy to eat the rest of the steak. Yesterday when I went out to lunch I had a personal 8 inch brick oven style pizza (left off the end crust). I was able to eat most though focused on the toppings. I felt full after but not uncomfortable. I am in maintenance so fit it into my plan. I will never be eating 3-4 slices of a pizza like my old life. I will never go back for seconds. Take advantage of your restriction. Choose dense Protein. Stop a bite or two before you think it is enough. Chew a lot and eat slow. These are things I have to remember every day.

It hurts every time i eat :(

Completely normal. Sad to say. But it will get better gradually. Also there is nothing stopping you from staying on mushy/puréed longer. My surgeon/nut said not to worry if cannot progress right away. Take your time.
  16. I just hate the BMI. I am 10 lbs over being "normal" weight at a BMI of 26.4. But if I lose more it would be with some muscle as well.

Sleeve Leaks ... Any fatalities reported?

My surgeon did a web chat on the sleeve a few months back and talked about leaks and why they use a 40 fr bougie instead of smaller since it less likely to have leak issues. You may want to see what size your surgeon uses since it appears less chance of leak with larger size and weight loss is not decreased with size 40fr. Since sleeve only a few years and not as much research on optimal size bougie yet they are only beginning to determine what is the optimal size to improve weight loss vs. prevent complications. http://www.ustream.tv/recorded/44477278 Also http://www.ncbi.nlm.nih.gov/pubmed/23023201 or http://www.eaes.eu/getmedia/8fb93687-5ea0-4e71-b144-84e6abbd69fc/letter_for_Parikh-_review_-_Marius_NEDELCU_-_eaesAF-final.pdf Also http://link.springer.com/article/10.1007%2Fs11695-013-1047-3
  18. at 5-10 you can eat 1200-1300 a day and still lose weight. I know because that is me. in fact I eat 1300-2000 a day in my weight loss phase before taking out exercise calories. the more cardio exercise I do the more hungry I am and that's okay. I do two days strength training and 2 cardio. for my height, since calorie budget is high side carb intake is also higher than people on 800 a day. I eat over 100 carbs a day. I also eat 80-100 grams of Protein a day I don't check fat grams but those are also around 90-100 a day so my macros are often about 50% fat. I lost 130 lbs over 16 months on this so not everyone needs or should eat 800 cals or stay under 50g carbs especially taller persons. I will advise though to limit SUGAR carbs to < 25g per day. when I do that I lose the fastest. Also do not graze. Eat 3 meals and 2 Snacks a day with at least 2-4 hours in between. Protein first, fat second and carbs last.

    Sleevers over 300lbs?

it took me 18 months in total to get from peak of 315 to around mid 180s and size 12. I am 5-10 and carries most on legs. you look like you carried more on middle. a lot you have left will always droop without plastics so stop beating yourself up. if you are getting rashes document with you primary care doc and some plastics may be covered by insurance. most surgeons want you a couple years out from surgery before plastics so weight is stablized but you can always get a consult. you are doing great though. no boot camp will get rid of extra skin but it will burn more calories and help you lose. it will also make you look great if you ever decide to get skin removal/tummy tuck. my arms are like yours. I want them done someday. have to save $$$.

    Sleevers over 300lbs?

I lost about 90 lbs from surgery in 9 months but was still around 208 (started 315 before pre op) I really wanted that Onederland. in order to make it I had to ratchet up training. I had just been walking a couple times a week. I saved the heavy artillery for the end. I started with 6 months of personal training 2x a week and spinning class 2x a week. in the past 6 months I lost another 25 and safely under 200 at around 183 or so. at my height this is good as I am so under 25% body fat though still 26 BMI. this required a lot money and focus. I don't think you need to get personal training but you may need to really commit yourself to get past the finish line. the sleeve helps but you need to decide how much you want to make your goal, create a plan and constantly recommit.

    Dumping Syndrome....... Advice?

Reactive hypoglycemia is a form of dumping. There are 2 basic forms http://www.urmc.rochester.edu/encyclopedia/content.aspx?ContentTypeID=134&ContentID=107 Also http://en.m.wikipedia.org/wiki/Reactive_hypoglycemia Dumping more common for bypass especially if sugary foods gets "dumped" into small intestine quickly without usual stomach to digest it. But is happens to 10-15% of people who have had a partial gastrectomy (which the sleeve is) whether for weight loss or other medical reason. True dumping would not last overnight. It resolves itself in a few hours at most. Hopefully sooner! And it would have cardiovascular symptoms like palpitations and also flushing, etc. Falafel a are deep fries and not full of sugar so may not have been dumping precisely. This could have been just over eating for the sleeve which would also make you feel yucky but not dumping. I ate a falafel too quick a couple weeks ago and felt pretty yucky for a while. In any case hope you feel better soon and remember to eat very slowly any new food.

    Wedding Dress Shopping

25 years ago I bought my dress before inflated sizes. When I ordered it I was 195 at 5-10" and I ordered a 16. They wanted me to order the 18 but I was on a dedicated weightless plan. 3 months later I was 172 and when it was fitted they took it in a LOT to the point that it was more like a 12. Dress boutiques love to order large so they can make money on the alterations so don't worry if the number sounds big. They will make it fit! I have a cousin who needed even more alterations 2 days before her wedding and got them done not that I recommend that but if you get it fitted in September and get smaller you can get it altered again in October. Just find a seamstress who can accommodate your changing body.

    Can't stop losing!

I think your body will slow weight loss and stop when it needs to. You are pretty young and so getting back to your pre children weight would not be too thin. So I don't think you should worry over much. Add some healthy higher calories Snacks. My goto is a handful of almonds, dark chocolate m&ms and almond m&ms. It is fat and tiny bit of sweetness and also fills me up. I would not recommend in Weight loss phase but in maintenance it really hits the spot. I also have a banana or other fruit a couple times a week when I never did I in weight loss phase plus 2-3 Alice's of bread a week. Just make sure to track so that a little extra doesn't add up to more and then you get hooked on carbs or sugar which is what causes weight gain. Remember Protein then fat then carbs - and very rarely sugar.
