Anyone have a sample PCP "medical necessity" letter?

Sharon, My PCP gve a very simple one and it was accepted. He simply stated Greg has been under my care for 15 years during which time he has been overweight. He has been under my care in an attempt to lose weight and has not been successful. Many of his other health problems are a direct result of his weight. I feel as his PCP that he wold benifit from the lapband surgery and the loss of weight that wold result would help in the maintanance of his other health issues. Not sure on the eact wording but it was very similar to this. I don't think I ket a copy of the letter but if I did I'll let you know and I can fax a copy to you if possible or Il send it word for word in an e-mail. I'll let you know if I find it'

    Favorite Grandparent Memories

I had the fortune of knowing all my grandparents (one grandfather was not maternal but you'd never know it) and three of the four sets of great grandparents well into my late teens /early twenties. The stories I could tell would expend past the time I have left on this earth. Three of my children will only know two grandmothers. My oldest son had the pleasure of knowing my father but unfortuneatley he was dying of cancer the whole time but my son says he was still a great inspiration to him. But one short story. My dad's father was 22 when he died in 1922. My grandmother then left her two son's with family in the upper part of the State of Michigan and moved to Detroit for a factory job in those days. She got employeement in a Chrome Plating factory. When my grandmother re-married she was able to bring her boys to Detroit to live with her again. Well as it turns out the factory she worked in is in the suburb of Detroit where I live and was closed about 20 yrs ago. The building is still there but the busniess have changed several times. Once I was having a difficult time (personal issues) and I was working the midnight shift as a cop. I was feeling pretty depressed and I had drove my scout car to the rear of the building so I was hidden from the public in order to relax and think things out( ok so I would catch a few zzzzz there now and then). It was there at least 8 yrs after her death that I actually found myself hearing her voice. I talked to her and she gave me advise. As it turns out I did take her advise and shortly there after my personal problems began to work out and I got over my depression. After that when ever I had a problem I could go there and for some unknown reason after I pulled out from behind the building I could figure out what I need to do to handle any problem. I know she has guided me thru many times of troubles. She was always known by family members to be the one to go to when she was alive for advise. She'd take hours to talk to anyone who need help. The strange thing is that I live directly across a freeway from the building. every single day I leave my home and point my car to the end of the street I see the building and I remember her and thank her. The building now houses a new gym...and I think she is calling me to come join it ...lol It won't surprise me on bit.

    For those who have religion

Well Kathy as fate would have it. I was just called and told one of my cousins passed away yesterday. My family thinks e-mail is proper way to make notifications of these things and they called because I didn't acknowledge the e-mail. They sent it to an old e-mail address I used to use. Can't figure them out sometime. I must send them two or three e-mails a day from the new addy and they send important stuff to me on the old one. He is being cremated this afternoon and a memorial service is being held Saturday Morning. So Kathy you and I will share a week of sorrow here. Dick had cancer and was suffering terribly so he is in a better place now...no less sad but easier to justify I guess.

    I'm Managing My Morbid Obesity

a medical device implanted ...Alex let me tell you the truth about this device. God made some white, black, brown, yellow and red people. He made some male and some female. He made some tall and some short. But he intended to make us a proper weight. Then he realized every invention has its flaws. And some of us we eating up everyone elses food supply. So he had a recall on some of the models with the defect and sent and army of angels (the Dr's) to put a stop to repair our flaw. Now he needs to g=figure out how to fix it for those of us that grew taller then our hairlines.

    Cancer Free and now Lapband

Renee, Praise God for your recovery and We'll pray for as great a success with your banding. My 23 yr old daughter just had cancer surgery this past June and although its still recent she has been doing very well. She Recently had further problems as the Dr's thought she had Colon cancer. I know its hard to believe anyone would be happy to find out they have chrones but she was glad it wasn't cancer. Take care and keep us posted

    New fill is a challenge

Tracie, I have had three fills and have yet to feel a single thing during them. And at two of the fills they had trouble getting into the port yet I still didn't feel it. Ports not that deep under the skin So its not like getting a shot into the muscle where it hurts. You'll be fine...don't worry.

    Tuna Fish

Shamrock, We can have crackers and breads(no white breads) so you can use then=m if they fit into that meal plan. I take solid white albacore packed in Water, mix it with fat free Mircle Whip, Sweet pickle realish a dab of mustard and a little squeez of lemon. Some chopped onions, sprinkle a few imitation bacon bites on it then wrap it in lettcue leafs and make a Lettuce Wrap meal. I've gotten into wrapping a lot of stuff into lettuce wrap style. Had that for the first time when in Vegas in Dec. At a Chinesse Rrest. (can't remember the name) and it was great. SO I experiment with stuff now.

    Michigan Blue cross/sheild Network HMO

Once I had all my paperwwork into the office it took less then two weeks to get the approval. Was banded two weeks later. but I have BC/BS traditional and I don't know if that makes a difference. But its my understanding that BC/BS of Michigan is approaving this surgery almost across the board. Good luck I know personally you been struggling in this process.

    weekly scale challenge

Zoe, Sorry to hear you were under the weather. Glad your better. But thanks for the tip on how to bring up calorie count when you feel it may be to low. One of the guys at work just won a free trip to Vegas at a cop's donut eating contest. He ate 7 Krispy Creams in one minute. Hell I could do that banded LOL But I didn't enter cause I was afraid my Dr. would throw me out of his office window if he heard I did ...lol...Might have chanced it if Dr wasn't on the fourth floor

    MY port is an outtie

Sharon, you have to remember that the port is attached to the top of a muscle just under the skin so you shold be able to feel it. Some can feel it with no problem others have to press. But like Darcy says the Dr needs to feel it to fill or unfill it. I had a fall shortly after my surgery where my protruding belly scraped down the side of a shopping cart and now my port is tipped. They think I maky have pulled the bottom two stiches off. But its not any problem at this time so they are going to leave it alone. It actually sticks out a tiny bit when I'm standing but then I wasn't planning on showing off any mens fashions in the near future lol.

How much weight have you lost?

I'd like to keep it going but I just realized if you go back and edit the original one we did here you can't then go and fix all the post after you so it just needs to be a new post or we could close the thread at the end of each month and start a new one. One for every month. But I do think this is good for all to see that total. its amazing...I own a car that weights less then this amount.

    weekly scale challenge

Betty , Don't get discouraged. I just went threw (and may still be in a slump). I stayed the same for almost 6 weeks. Then last week I decided that even though I was moving inches around I wanted to do a reality check and I started writing every thing down I was eating and in the honest amounts. In tracking my food again I found that I had gotten off on what I was supposed to be doing so now I'm back following it well and I'm seeing the benifits. Last week I lost my first pound in 6 weeks and today I lost another. So it appears I may have broken threw. But the important thing is I feel great and doing things I wasn't before and I know these pauses in weight lost, as upsetting as they are, are only temporary. The band helps cause in other diet attempts this is when I'd get upset and stuff myself back to higher weight. Band won't let me stuff so I just keep trying to follow my program. Good luck and don't give up the faith

How to get my Brain to go with the weight loss

Nancy, You hit the nail on the head.(are you a carpenter???) The scale used to be my enimy and then after the band as fast as I was losing it became my constant companion. Now its like a distant friend cause I only get on it on Mondays Good luck and remember the band only works if you want it too. The diets we used to fail at had a reason for the failure. We never felt full. Now the band will allow you to feel full you just have to make the right choices of what you put in the stomach.
