Our protocol with this type of problem is a significant unfil with rest of your pouch for a couple of weeks then slowly refilling. This usually cures the problem. If not, then we do a scope or an upper GI. It is very normal not to have restriction at this point. It takes 3-5 fills to get there - just be patient and follow the diet no more cheating! This band hasn't been released in the US yet so I don't know anything about it. I'm sure it's a good band but the most important thing is that you have a band--whether it's an AP band, Swedish band, Midband, whatever. The differences between them in weight loss will be minimal. Question for Dr.C -compression garment?

I don't really see a need for them after a lap band, I don't think they would help that much - seems like my patients have more pain that is gas-related which the garments wouldn't help at all. To be honest I think we know very little about erosions. My guess is that they are either due to technique or to bands being too tight or possibly a chronic slip. As far as the technique goes sometimes these cases are very difficult and it is impossible to end up with a perfect result every time, try as we might! Early erosions are probably more likely technique related, late erosions the opposite. Hard to say Amy, sounds like it could be a hernia to me. I did have one patient who developed a hernia right under the port so it was very difficult to detect as there was already a lump there. In my experience low BMI bandsters do well BUT - the weight loss is very slow. Whether you weigh 500 pounds or 200 pounds, weight loss with the band is about a 2 year process. If you are only needing to lose 50 lbs with your band, that will still come off over a 2 year period. You would probably in this scenario lose 30 lbs the first year and 20 lbs the second year. So that's roughly about a half pound a week at best. The moral of the story is - the weight WILL come off, just not overnight! Anything that irritates the diaphragm can cause shoulder pain (google Kehr's sign). The band lays right up against the diaphgram. Sorry - I thought it would have the opposite effect! You should know that I've already sprayed her skin with some numbing medicine so she doesn't feel the stick. There is sound to the video so you can hear there is no moaning in agony!! Plus, it only lasts 8 seconds... My guess is that either you ripped one of the sutures or you have a hernia there. If it is the former it will eventually get better. First of all I don't think what you're describing is bulimia at all, it sounds like the symptoms of an overtight band. It doesn't matter what it looks like on fluoro, if you have symptoms of being overtight, you're overtight. I've never seen one single bit of research to substantiate this claim. Regular pop is evil because of all the calories. A small amount of diet pop is ok in my book as long as it's not coming at the expense of your protein intake. Most likely you just need another fill. Could be a slip, or more rarely an erosion. If you were my patient I'd fill you, if no luck then get an upper GI or do a scope. Good luck! Shay, I appreciate the kind words from other patients. Couple of things about CareSource: 1. If your BMI is below 50 they rarely approve any type of weight loss surgery 2. Even if your BMI is over 50 you're going to need some major comorbidities (such as diabetes, hypertension, sleep apnea, arthritis) 3. Even if you do have all of the above they don't approve the band very often only RNY (although we have had one approval) I wish it weren't so, but that's what we have to deal with. On the plus side, if you meet the above criteria you really don't have to have a referral from your PCP. On the down side CareSource will make you do at least a 6 month supervised diet prior to approval. It's a real pain in the butt!
