Lexi, The one and only one patient I banded that I put a 4cc band in when I probably should have put a VG was in a patient just like you describe. I think there is some merit there, you can't go wrong with a VG, the worst case scenario is that you might have to overfill it to get adequate restriction. I posted on the ObesityHelp forum that I would put the abstract for this article here - it's a way to treat port infections without removing the port. I have personally tried it an it worked great. A 3cc fill is a pretty small one in my book for a VG. I usually do 4cc. If you had an early slip and never really had any trouble vomiting, it could be surgical technique. If you, like many bandsters, have had your share of PB'ing, there is no way to know. The bottom line is - slips are rare in patients who don't PB. Proper eating techniques are very important! Pars flaccida makes posterior slips almost impossible, meaning the back wall of the stomach. The anterior (front) wall can still slip. It's very easy, you have to have local anesthesia with some sedation. Takes about 30 minutes. Can't speculate on cost, depends on your center. The short answer is yes, I probably would prescribe an appetite suppressant if asked. As far as complications 3 years out, you can slip a band or dilate a pouch any time, the risk really doesn't go down with time. I wouldn't expect too much out of the first fill - it takes 2-3 to get decent restriction usually. Nancy, I don't know anything about your surgeons per se so this has nothing to do with them personally... There are surgeons out there who are not "believers" in the band who do it every now and then anyway rather than losing patients who refuse to have a bypass. You want to make sure that is not the situation wherever you choose to go. As far as how to figure that out, that's up to you! Certainly concerning that there might be a leak, but until you have 2 consecutive fills where there is zero aspirated, I wouldn't say for sure that's what it is. If you were my patient I'd say come back in a month, if there is zero in there again then inject some dye in the port and see if there is a leak.
