I just completed pre-auth evals and classes and received in the mail today a denial letter. I'm confused. My insurance co-ordinator stated she would submit my psych eval and request for surgery on Oct 19, and the insurance company states they didn't receive the psych eval nor do they have any info like my BMI and co-morbidities. I have uncontrolled Diabetes type 2 and hypertension. My BMI is 39.42. Why didn't my insurance co-ordinator submit all the necessary info and if she did, why am I being denied? I haven't recieved medical clearance yet, not at that stage but I see a cardiologist for cardiology clearance this month. What's my next step?  I'm really concerned.
