I have been heavy my entire life.  I decided to have the lap band in 2007, I weighed 318 and I am 5'9''.  The approval process was easy, my insurance paid all but $100.  My surgery was 1/30/2008.   My surgeon is amazing.  I got filled once after my surgery, then I moved an hour away.  I lost about 30 pounds, then I started to fall off the wagon, now I weigh 337.  :-*(  I am trying to get back on track desperately and have an appointment to see my surgeon later this month.  Hopefully I can get filled and continue to lose despite this setback!
