I am in a wheel chair and have spent a lot of time in the hospital and nursing facilities.  In any medical facility, how you're treated can have a lot to do w/ how difficult it is to "treat" you.  I am sedentary and as big as I was, it was hard for nurses, etc., to turn me in bed, lift me, and stuff.   So I decided to get lapband surgery (inToronto, because Medicare wouldn't pay here); that was in 2007.  I lost ≈80 lbs. in the first 1-1 1/2 years.  I'd like to lose another 30 #'s, or so, but I've gotten kind of sloppy with my food choices.  I haven't had any fills and an (overzealous?) doctor removed the couple of cc's(?) that were in the band early on, because I was throwing up a couple (or more) times a week. It took me awhile to get in the habit of stopping 1 or 2 bites before I felt full (no matter how good it tasted) =P.  
I wish I knew how to add photos but since I don't,  you'll just have to imagine a tall, athletic, buxom, 22 y.o. w/ thick chestnut hair.  No, that's not me, but isn't it nice to imagine?  ^_______^  e.
