Self-paid several years ago, when most insurances wouldn't cover the surgery. I've had two babies since then, and am currently on a very good downward trend. I'm very pleased with the adjustability of the band, which has allowed me to take a step back for pregnancy and breast-feeding without feeling that everything is wasted and also allowed me to start back up again when I'm ready.
