I also saw Dr. Oliak and was hit with his $3500 "Program fee". Sound pretty outrageous since my PPO pays very well. Obviously whatever the "program fee" is for is not considerable medically necessary by my PPO, otherwise they would pay it. I am going to keep shopping. -
