Medical Records Headache

My PCP from another state has been really slow in sending my medical records for 2006 (the only year missing from 5yr history and Its been 6week since my second request for my records.). So I requsted records from an ER visit I had that year for heart palpitations and the Surgeons office got them already :) But the dag blasted ER failed to record my weight for that visit!:blink: How do I find out what the hold up is with the PCP if all they do is refer me to the medical records voice mail. I only have 3 more months to go before we submit to insurance and this is the only hold up!

I work in records so I will tell you how we handle it. You will need to fill out a ROI (release of information form). If your provider is part of a bigger company with a website you can usually find the ROI form on the site. Fill it out and send it back to them. Once they receive the form they have 15 days to comply. This does mean they can send them to you on the 15th day. If they use an outside service to copy your records ask them for there name and phone number so you can follow up with them.

If you haven't heard back from them at all ask to speak to the clinic manager and he/she will be able to help.

It should NEVER take longer than 20 days if you factor in the 15 days they have and maybe 5 for the mail (depending on how far away you live). If you live close go into the clinic and ask them for the form. Fill it out right then and there and give it back to them.

Thanks a bunch. I sent the ROI form about 6wks ago, now I'll follow your advice first thing tomorrow. I so appreciate your response.
